Although a significant proportion of adults recover from substance use disorders (SUDs), little is known about how they reach this turning point or why they stop using. The purpose of the study was to explore the factors that influence reasoning and decision making about quitting substance use after a long-term SUD. Semistructured interviews were conducted with 18 participants, each of whom had been diagnosed with a SUD and had been abstinent for at least 5 years. A resource group of peer consultants in long-term recovery from SUDs contributed to the study’s planning, preparation, and initial analyses. Participants recalled harmful consequences and significant events during their years of substance use. Pressure and concern from close family members were important in their initial efforts to abstain from substance use. Being able to imagine a different life, and the awareness of existing treatment options, promoted hope and further reinforced their motivation to quit. Greater focus on why those with SUDs want to quit may help direct treatment matching; treatment completion may be more likely if the person’s reasons for seeking help are addressed.

Recovery in co-occurring mental health and substance use disorders often involves relationships with professional helpers, yet little is known about how these are experienced by service users. The aim of this study was to explore and describe behaviour and attributes of professional helpers that support recovery, as experienced by persons with co-occurring disorders. Within a collaborative approach, in-depth individual interviews with eight persons with lived experience of co-occurring disorders were analysed using systematic text condensation. The analysis yielded four categories of recovery-supporting behaviour and attributes of professional helpers and the ability to build trust cuts across all of them: Building trust through (a) hopefulness and loving concern, (b) commitment, (c) direct honesty and expectation and (d) action and courage. Services should allow for flexibility and continuity, and training should recognise the importance of establishing trust in order to reach out to this group.

Purpose The purpose of this paper is to explore and describe experiences of recovery among people with co-occurring mental health and substance use conditions (co-occurring conditions) in a rural community in Norway. Design/methodology/approach In-depth individual interviews with eight persons with co-occurring conditions were conducted, audiotaped, transcribed and analysed using a phenomenological approach. This study is part of a research project investigating recovery orientation of services in a Norwegian district. Findings The analysis yielded four dimensions of recovery: feeling useful and accepted; coming to love oneself; mastering life; and emerging as a person. Insecure and inadequate housing and limited solutions to financial problems were described as major obstacles to recovery. Research limitations/implications Further research into the facilitation of recovery as defined by persons with concurrent disorders is needed, particularly regarding the facilitation of community participation. Practical implications This study supports an increased focus on societal and community factors in promoting recovery for persons with co-occurring conditions, as well as service designs that allow for an integration of social services and health care, and for collaboration among services. Social implications The results suggest that the community can aid recovery by accepting persons with co-occurring conditions as fellow citizens and welcoming their contributions. Originality/value The paper provides an enhanced understanding of how persons with co-occurring conditions may experience recovery.

Basert på tverrfaglige fokusgrupper og individuelle intervjuer med til sammen 11 sosialarbeidere beskriver denne artikkelen deres arbeidserfaringer med overdoser og overdosedødsfall. Data ble analysert med systematisk tekstkondensering med henblikk på å utvikle en ny beskrivelse av erfaringene. Den nye beskrivelsen samarbeid noen dør med henspeiler på det komplekse i at ansvaret for å forebygge overdosedødsfall dels ligger i sosialtjenesten som struktur, dels hos sosialarbeiderne som fagfolk og dels hos klientene selv. Særlig i akutte kriser så samarbeidet ut til å bli fragmentert og utydelig. Studiens funn drøftes i lys av noen av prinsipper i Ottawa-charteret om helsefremmende arbeid.

Studien beskriver brukertilfredsheten med samarbeid før, under og etter en intervensjon for å øke miljøarbeideres kompetanse i språk og språkobservasjoner i et lavterskel hybelhus. Beboere fra to andre hybelhus uten intervensjon var kontrollgruppe. Forskjeller i gjennomsnittsskår mellom beboere i hybelhus med og uten intervensjon ble undersøkt med Students t-test. Før og under intervensjonen var beboerne ved hybelhusene uten intervensjon i gjennomsnitt litt mer fornøyd med samarbeidet med ansatte. Etter intervensjonen var det beboerne fra hybelhuset med intervensjon som var mest fornøyd. Opplæ- ring av ansatte i språk og språkobservasjoner kan gi økt tilfredshet blant beboerne når det gjelder samarbeidet med ansatte

In this article, we suggest that collaboration can be specified as a set of processes built around turntaking as the key process, with the processes of negotiation for common goals and putting differences to work as the two-dimensional focus in collaborative practice. Turntaking connects therapeutic work to establishing relationships instead of focusing on motivational states. Common goals connect collaborative practice to the therapeutic alliance. Putting difference at the centre places collaborative practice within a dialogical perspective. We specify dialogue as the overarching perspective in this article, differentiating two different types of conversation within the concept: dialogical and dialectical as the processes of change. The dialectical leads to an agreement of goals and perspectives, while the dialogical is oriented to accepting differences in goals and perspectives. These are connected to two different paths to change within the present moment. The first describes a process of moving along in present moments that turn into now-moments and are resolved in a moment of meeting. This could be seen as a description of dialectical conversations at a micro level. The second path to change occurs during quieter moments of therapeutic processes when participants are engaged in an intersubjective quest to negotiate the best fit between one's own intentions and those of the other. The therapist's skills to realise this include the use of service user feedback and following the lead of the family.

Recovery ved samtidige lidelser er lite dokumentert i norsk kontekst. Denne studien baserer seg på individuelle intervjuer med 13 brukere med samtidige lidelser som mottok psykososial oppfølging fra et psykisk helse- og rusteam i en bydel i en stor by i Norge. Intervjuene ble analysert ved hjelp av systematisk tekstkondensering. Hovedfunnet var at deltakernes erfaringer med recovery-orienteringen i tjenesten kunne uttrykkes som Her tar de tingene i henda og gjør noe med det. Funnet antyder at livskvaliteten for brukerne er knyttet til relasjonene og samarbeidet med teamet om de områdene og de målene som brukerne synes er viktige.

Psychoeducational multi-family groups (MFG) are recommended in the new national guidelines for the treatment of psychosis: What are the participants’ subjective experiences? The correlation between family intervention and an improved course of psychotic disorders is well documented. There are, however, few studies which have specifically examined participants’ experiences. This study explores the subjective experiences of three girls (aged 19–26) suffering from a psychosis and five of their familymembers (aged 42–61). Data collection was done through qualitative research interviews, and data was analysed using a phenomenological-hermeneutic approach. The findings suggest that knowledge, particularly experiential knowledge, contributes towards understanding, management and a sense of accomplishment. Purpose – Hope is regarded central to recovery in a broad range of health conditions including mental health and substance use problems. Still the phenomenon, along with its implications to research and practice, has gained limited attention. The purpose of this paper is to explore first-person accounts of how practitioners nurture and inspire hope. Design/methodology/approach – This qualitative study is part of a larger action research project. Data were collected using in-depth interviews with eight participants. The interviews were transcribed verbatim and analyzed applying thematic analysis. Findings – Hope was perceived as a central phenomenon to practitioners within mental health and substance use services. The following overarching themes were identified through thematic analysis: “Believing in oneself and others,” “Seeing and acknowledging opportunities” and “maneuvering towards hope.” Research limitations/implications – It seems that there is a need for extending knowledge in how practitioners may inspire hope. In addition there seems to be a need for developing more knowledge on how practitioners’ own hope may be nurtured within a system that is constantly undergoing change and new demands. Practical implications – Participants considered their own hope as a prerequisite in being able to hope on behalf of others and strategies for inspiring hope were presented on both an individual and system level. Working conditions and challenges within the service system in itself entailed challenges, which at times made nurturing hope difficult. Systems need to embrace flexibility and openness, allowing efforts aimed at inspiring hope to be made. In addition, practitioner’s own hope and the importance of nurturing it, needs to be acknowledged within services. Hope needs to be viewed as a joint venture, in context and in relation to others. Originality/value – The paper contributes to the knowledge base on hope inspiring practices within a field sometimes associated with challenges and despair. It points out some important preconditions in relation to the challenges practitioners might encounter, both on an individual level and in relation to the service system per se.

Background: Norwegian health authorities place emphasis on recovery oriented practices in mental health services. Recovery is described as an active process with a focus on personal resources and supportive contexts. In the recovery process, the relationship between the person and the carer is of great importance. Conversation is a meaningful approach for developing a trusting relationship. Conversation also has importance in itself, because it establishes the foundation for human contact and gives the client the opportunity to be acknowledged as a person. Aim: The aim of the study was to describe and explore what health professionals focused on in recovery oriented conversations with patients in a Norwegian mental healthcare centre. Methods: This study was part of an action research project and had a qualitative and explorative design. Data were collected in multistage focus groups and were analysed using qualitative content analysis. Findings: The findings highlighted the prerequisites for conversation, the content of conversation and different views on the topics of conversation. Conclusions: The findings contribute knowledge about what promotes or inhibits recovery oriented conversations. Such conversations focus on the patients’ everyday life, appreciating them as actors in their own lives, and facilitate shared decision making processes and working with hope. The study demonstrates that individual, cultural and contextual aspects play an important part in recovery oriented conversations. Implications for practice: Practice development involves acknowledging and re-evaluating the possibilities for using conversations with patients as an approach and as a tool in person-centred and recovery oriented practices Relational competence is an essential part of enhancing recovery oriented conversation, and needs to be attended to in skills training and competence building Awareness and critical analysis of the clinical context is important to promote an active and participative patient role. Authoritarian cultures with concern about what is permitted or not may well be a barrier to shared decision making

The aim of the study was to describe nurses’ experiences from situations in which patients suffering from dissociative disorders needed stabilization. The data were collected from individual research interviews and were analyzed using Malterud’s systematic text condensation. The analysis led to new descriptions of the experiences: to create inner peace, in order to feel at peace and to give peace. “Creating inner peace” describes the conditions that allow nurses to feel secure in the situation. “Feeling and giving peace” is about common traits in the behavior of the nurses. The study will provide inspiration to the development of phenomenological and health promoting practice for nurses who work in similar situations.

Hope is a central concept in nursing and other fields of health care. However,there is no consensus about the concept of hope.We argue that seeking consensus is futile given the multifaceted and multidimensional nature of the concept, but instead we encourage in-depth studies of the assumptions behind talk about hope in specific contexts. Our approach to the ‘science of hope’ is inspired by philosophical pragmatism. We argue that hope is a concept that opens different rooms for action in different contexts and that accordingly, all hope interventions are con- textually sensitive. Careful attention to how the relative positions and power of nurses and patients influence what can be inferred from their different ways of talking about hope may make hopeful conversations more meaningful in health care relationships.

Collaborative practice in health care is hailed by Zwarenstein, Goldman,and Reeves (2009), and Hopkin (2010), as an approach that can produce improved health outcomes by preventing fragmentation in healthcare and strengthening healthcare systems. A great deal of efforts is being extended by international organizations such as WHO and national Health directorates to improve collaboration in healthcare provision. As a key approach to enhance collaborative practice at the service level, a model of collaborative practice focusing on collaborative processes is proposed in this paper. A model of collaborative practice in community mental Health care is based on the results of two case studies and supported by relevant literature. The model has four components – (a) the frameworks for practice consisting of person-centered practice and recovery orientation; (b) the structure of collaborative practice that includes intrasystem collaboration and collaboration in service processes; (c) Three principles for collaboration as self-understanding, utual understanding,and shared decision-making; and (d) the key processes in Collaboration identified as open dialogue and participatory engagement. Collaborative practice in community mental health care depicted in this model puts service users at the center of service provision and processes with the goal of addressing service users' needs from their perspectives. The model provides a framework to develop and test practice guidelines for community mental health care.

To assess demographic characteristics, treatment utilization and circumstances of death among those who died from drug-induced deaths in an urban setting and to identify possible subpopulations that should be targeted specifically to further develop preventive public health policies. Subjects (N = 231) who died, from drug-induced deaths, in the Norwegian capital Oslo (2006–2008) were identified through the National Cause of Death Registry. Data on toxicology, prison release and contact with health and social services in Oslo were collected. Majority of cases were men (78%) and the mean age was 37 years. Nearly all cases (90%) were polydrug intoxications. Heroin was implicated in 67%. Residential address was the most common place of death (67%). Most cases (82%) had been in contact with health and social services in the year before death. Women were 4 years older, more often Oslo residents (82% vs. 64%) and fewer died from heroin intoxication. Non-Oslo residents were younger and more likely to have been found outdoors with heroin as the main intoxicant. Other identified subpopulations were those who died after prison release and those discharged from drug treatment. The findings suggest that the majority of cases could have been available for preventive measures through their contacts with health and social services. Yet, the heterogeneity among cases indicates that such measures need to be multifaceted. Finally, it is important for policymakers and health and social workers in various countries to consider subpopulations such as women and non-city residents when developing public health interventions to prevent overdose deaths.

Hvordan beskrives håpefull praksis av ansatte i ambulante akuttpsykiatriske team, og hvordan kan denne praksisen forstås? For å besvare dette ble det gjort fl ersteg fokusgruppeintervjuer med ansatte i et slikt team. En fenomenologisk-hermeneutisk metode inspirert av Ricoeur ble brukt i analysen av de transkriberte intervjuene. Vår helhetlige forståelse er at ansattes håpefulle praksis innebærer at de veksler mellom å inspirere og tilrettelegge for å skape bevegelser i fastlåste situasjoner. Vi fant at handlingene kan inndeles i «å komme i posisjon for å skape bevegelse», «å sette pasienten i bevegelse» og «å støtte pasientens bevegelse». Vi diskuterer hvorvidt håpefull praksis er et faglig ideal, om det har faglig legitimitet, samt kulturelle forutsetninger som forutsigbarhet, framskrittstro og selvstendighet, og det etiske dilemma som oppstår når pasienter beveger seg mot selvmord.

En tilnærming til levde erfaringer basert på et innsideperspektiv kan åpne for forståelse og kunnskap om fenomenene slik de er opplevd. Hensikten med artikkelen er å beskrive, undersøke og tolke metaforer om opplevelsen av seg selv hos yngre norske menn med heroinavhengighet og selvmordsadferd, samt å bidra til at subjektive erfaringer vurderes som et grunnlag for kunnskap i tilfriskningen. Analysen av metaforene konstruerte tre tema; å være isolert, å være nær punktet uten returmuligheter og å fortsatt være på grensen. Den helhetlige forståelsen kan tolkes som å leve med en balanse mellom å være offer eller aktør. I tilfriskningsprosessen kan økt oppmerksomhet rettet mot klientens metaforiske språk om opplevelsen av seg selv brukes som et bidrag i en flerfaglig vurdering av selvmordsrisiko.

For de ikke-vestlige mennene som deltok i vår studie, kan meningen med emigrasjon, rusavhengighet og selvmordsatferd forstås som å leve i en labyrint. Livet i labyrinten var preget av en alvorlig usikkerhet. Denne opplevelsen av usikkerhet var knyttet til uklar identitet, eksterne konflikter og mangel på tilhørighet. Diss forholdene påvirket deltakernes helse, velvære og mestring negativt. De opplevde hjelp og behandlig konstruktivt når rusavhengigheten ble fokusert samtidig med de psykososiale problemene. da kunne deltakerne også få kontroll over selvmordsatferden.

The aim of the study was to illuminate and interpret the lived experiences of emigration, substance abuse and sucidal behaviour in young non-western men in Scandinavia. A phenomenological-hermeneutic approach was used to analyse the data. A comprehensive understanding of the data was formulated as "living in the maze". Ill-health involved having a sense of liminality, which impaired one's possibilities to define and re-define goals. Substance abuse and suicidal behaviour were explicit expressions of not being well, when living in a maze that was perceived as closed. Both problem-focused and emotion-focused coping were used by the participants. These provided the motivation for substance abuse and suicidal behaviour.

The aim of this study was to illuminate and interpret the experiences of professionals and men living with life-threatening heroin overdoses in Oslo. A case study design was chosen for data collection. A phenomenological hermeneutic approach and triangulation was used to analyse the data. The naive reading involved awareness of the perceived sense of social isolation, which had consequences for how life-threatening heroin overdoses were managed, comprehended and motivated by the men. Professionals also experienced isolation, since they had no specific goals or guidelines for cooperation, and relevant political documents did not address the phenomenon of life-threatening heroin overdoses. Non-participant observation revealed different types of isolated interventions, involving very short interventions by several kinds of actors. The structural analysis identified three themes: The meaning of being rescued, the meaning of longing for communion and the meaning of being confused. A comprehensive understanding of the complexity in experiencing and preventing life-threatening heroin overdoses in men in Oslo indicated that the meaning of these social phenomena could be understood as bridging the gaps between barriers to treatment, interaction and motivation.

The purpose of this descriptive study was to explore and interpret men's experience of sence of self within the context of recovery from substance abuse and suicidal behavior. A secondary analysis of data from a previous study of four young Norwegian men revealed how a shifting sence of self was constructed through the use of metaphors. The analysis identified three themes related to the participant's sence of self: The meaning of Being Isolated, The Meaning of Being Close to the Point of No Return, and the meaning of Still Being on the Edge. By acknowledging metaphorical expressions as a source of important knowledge, this study reflects on how mental helath professionals can use metaphors in the recovery process. As part of the treatment alliance and the therapeutic communication, we suggest that metaphors may aid the assessment of suicidal ideation, promote a sence of more active self, and signal acceptance and recognition of subjectivity.

Abstract Introduction: In the research literature there is documentation that some overdose deaths may be suicide. This study is based on data from a review of all fatal overdoses in Oslo during the period 2006-2008. Methods: Based on a case comprising two of these deaths that were assumed to be suicide, either based on specific statements or suicide notes, we describe by a qualitative exploration how relatives and professionals experienced the deceased persons’ life situation during the last year they lived. The phenomenon of overdose as suicide is also illustrated using the experiences of other participants in the Oslo study group. Results: By individual research interviews and analyzing the text by systematic text condensation, we found five important elements for overdose as suicide: Danger afoot all the time, Physical changes, Indicating distance, Relating differently to life and death, and Overdoses are more than an accident. Discussion: The elements are discussed in the light of two of the principles for health promotion in the WHO´s Ottawa declaration, to create supportive environments and to develop personal skills. Conclusions: From the experiences of relatives and professionals, we suggest the importance of taking on a participant-position in relation to the persons' life contexts and the meaning of creating hope.

Bakgrunn Rusavhengighet og selvmordsadferd representerer store folkehelseproblemer. Fenomenene er hovedsakelig studert med kvantitative design. Åpne dybdeintervjuer kan få fram kunnskap om heroinavhengighet og selvmordsadferd slik fenomenene er opplevd av mennene selv. Ricoeur hevder metaforer er bærerer av meningsoverskudd gjennom å inneholde en språklig spenning som åpner for ny forståelse. Tolkningen av meningen med denne spenningen blir da sentralt å utforske mellom klient og hjelper. Hensikt Med målet om økt kompetanse til å vurdere mennesker i krise, og deltakelse i forebygging og behandling av selvmordshandlinger, er hensikten med forelesningen å erkjenne subjektive erfaringer som grunnlag for kunnskap og innsikt. Det antas at kunnskap konstrueres i en gitt kontekst, der klient og hjelper begge blir viktige med-aktører. Metode En fenomenologisk hermeneutisk analyse ble brukt til å tolke meningen med metaforene i fortellingene om heroinavhengighet og selvmordsadferd. Resultat Analyseprosessen konstruerte en omskiftelig, endret og negativ opplevelse av seg selv ettersom heroinavhengigheten og selvmordsadferden tiltok. Dette forstås som en balanse mellom å oppleve seg selv som aktør eller offer. En tiltagende sosial isolasjon, fanget i en situasjon uten synlige muligheter for å vende tilbake til livet og å fortsatt å befinne seg på en grense mellom liv og død påvirket denne balansen. Konklusjon Metaforer brukes når hverdagsspråket ikke lenger strekker til. Behandlingsteamet bør rette oppmerksomheten mot klientens metaforiske språk, identifisere vekslende metaforisk språk om selvoppfattelsen hos selvmordsnære personer spesielt, samt alltid reagere på metaforisk språk. Dette kan anses som et viktig bidrag til klientens forståelse av og kommunikasjon om relevante levde erfaringer, og dermed deres tilfriskning.
