We have investigated whether closer collaboration at general practitioner centres in the treatment of mental disorders has affected the number of days with sickness absence and work assessment allowance. The collaboration model consisted of a psychologist and a psychiatrist working part-time at the centre and in collaboration with services in the borough. The model was tested at one general practitioner centre in each of three boroughs in Groruddalen in Oslo, while three centres in the same boroughs constitute the control group.
The purpose of the collaboration model is for patients to have a better overall health service, with earlier identification of mental health problems, faster access to health care, more effective dialogue between the health services and increased assistance in the primary health care without having to be referred to the specialist health services. Identification of mental health problems in more patients and earlier intervention can probably contribute to a greater proportion of sickness absence being linked to diagnoses of mental health problems, but more adequate treatment can probably also limit long-term sick leave and drop-out from working life.
We find that fewer people are on sick leave in the intervention group, but still see an increase in the number of sick days of almost 4 per cent, due to increased duration of sick leave and reduced use of graded sick leave. We also find an 8 percent reduction in the number of days with work clarification money. Both the increase in the number of sick days and the reduction in days with work assessment allowance are among those with mental disorders, which suggests that the change is due to the intervention.
Closer collaboration between the primary health care and mental health services may be able to improve the health service, with earlier identification and treatment of mental health problems. Although sickness absence is increased, it is conceivable that earlier and more targeted interventions will prevent mental health problems from developing into more serious and prolonged disorders and to prevent transition to work clarification or disability benefits.

Background and Objectives: Adolescence is a time of substantial change, bringing about the transition from childhood into adulthood. Patterns for future health behaviors are developed and continue into adult life. Little is known about adolescents’ perceptions of health and help-seeking within healthcare services. The objective of this study was to explore adolescents’ attitudes to health and seeking help for health-related problems.
Methods: A qualitative design was chosen. Interviews were conducted with 5 adolescents visiting a youth health clinic in Moss, Norway. The interviews were audiotaped, transcribed and analyzed according to systematic text condensation.
Results: The participants were 2 boys and 3 girls, ranging from 17-19 years of age. We found that all adolescents incorporated psychological and social qualities in their view on health. Positive social connections with family and friends were the most important factors for good health for all participants. They all conveyed resistance to disclosing mental health issues, although this was recognized as the most important barrier for good health. Establishing a trusting relationship with a healthcare provider was necessary before disclosing mental health issues and receiving help.
Conclusion: In our small qualitative assessment of adolescents’ attitudes to health and seeking help for health-related problems, we found that adolescents’ focus was on mental and social aspects of health and that a trusting relationship with healthcare providers was necessary for the adolescents to seek help within the healthcare system. Future research should be conducted to study more deeply what characterizes the relationship between adolescents and healthcare providers in order to provide the most effective and appropriate healthcare to patients during this particular period of life.

Designing a new elective subject for third-year medical students on navigating medical guidelines.

Background:
Understanding how to navigate medical guidelines is an essential skill for future doctors. Recent emphasis has been placed on the importance of prioritization and time management when implementing guidelines into clinical practice. However, traditional medical education is offered topic by topic, with less emphasis given to patient-centred care, grading of recommendations and sometimes conflicting guidelines.
Methods:
Our objective was to develop a two-week elective subject titled "The Medical Profession: Navigating Medical Guidelines" for third-year medical students in Oslo. Through an intensive application of the nominal group technique (NGT), a group of five experienced primary care physicians and teachers collaborated during a two-day session to design the subject and its curriculum.
Initially, we had concerns about the complexity of this topic for students at the third-year level due to their limited clinical experience. To assess the subject, the examination was designed to include an individual reflection note and a group presentation. Out of 16 students, 14 consented to having their reflection notes used for educational research purposes.
Results:
The NGT process led to development of a subject with interactive lectures, group and plenary discussions, as well as a clinical rotation day. Key topics included research guidelines, sensible choices and sustainability, patient-centered care, medical uncertainty, multi-morbidity, and navigating conflicting guidelines. A longitudinal patient-case video presentation accompanied the teaching with increasing complexity each day. Students were divided into two groups for the clinical rotation day at out-of-hour emergency care and nursing home facilities. Two teachers were present each day of the course to facilitate skill development and discussions. Students provided positive feedback, appreciating the reflection on their future professional roles.
Conclusion(s):
The NGT process proved effective in creating an elective subject well-received by both teachers and students. We encourage others to consider using similar models when designing new curricula.
