  Background and Objectives: Evaluating osteoarthritis (OA) care quality is increasingly relevant for service improvement and benchmarking purposes. The Osteoarthritis Quality Indicator questionnaire (OA-QI) measures patient-reported guideline-concordant care; however, no version has been tested in Italian primary care or bilingual contexts. This study aimed to introduce the OA-QI version 3 (OA-QI v3) in German and Italian, assess its applicability in practice, and examine its acceptability and reliability. Materials and Methods: A cross-sectional survey was conducted using the South Tyrolean General Practice Research Network. Thirty-eight general practitioners recruited 266 patients with hip or knee OA. Patients completed the OA-QI v3 in German or Italian, with subsamples for comprehensibility testing (n = 38) and retest reliability after 14 days (n = 36). Test–retest reliability was analyzed using percent agreement, Cohen’s κ, intraclass correlation coefficients (ICC), and standard error of measurement. The smallest detectable change was analyzed to estimate factual change. Results: Response rate reached 95% of the targeted patients. Patient feedback showed good comprehensibility and ease of use in both languages. Adherence to recommended quality indicators varied, with strengths in physical activity advice, NSAID prescription, and pain assessment, but gaps in weight management, occupational counseling, and assistive devices. Test–retest reliability ranged from fair to substantial at the item level (κ = 0.33–0.69) and was moderate for the total score (ICC = 0.55, 95% CI 0.28–0.74). While measurement error restricted individual-level interpretation, reliability at the practice or institutional level supports application for benchmarking and quality monitoring. Conclusions: The OA-QI v3 was feasible, acceptable, and reliable for group-level assessments in South Tyrol. These findings position OA-QI v3 as a practical tool for identifying care gaps and guiding quality improvement, while providing important lessons for the full validation of the German and Italian versions in larger cross-national samples.

  BACKGROUND: Workplace violence towards healthcare workers is increasing worldwide, with significant consequences for the healthcare worker, healthcare organization, and the quality of care. Healthcare workers in psychiatric inpatient units are particularly exposed, as the frequency and severity of violence is high. This study aimed to examine the prevalence of workplace violence towards healthcare workers in psychiatric inpatient units in Norway and the rate of formal reporting of such incidents. MATERIALS AND METHODS: A cross-sectional, questionnaire-based survey was conducted among healthcare workers across ten psychiatric inpatient units in Norway. The survey investigated the 12-month prevalence of workplace violence towards healthcare workers and the extent of formal reporting by type and severity. RESULTS: A total of 237 (36%) healthcare workers completed the questionnaire. The overall 12-month prevalence of workplace violence from patients was 93%, with 91% of respondents reported being exposed to threats and 80% to violence. The median formal reporting rate was 7% (IQR 0-50) for threats, 40% (IQR 1-69) for violence without injury, and 70% (IQR 20-100) for violence resulting in injury. Formal reporting was generally regarded as important and not perceived as a professional defeat, but it was not seen as leading to change. CONCLUSIONS: This study confirmed a high prevalence of workplace violence in Norwegian psychiatric inpatient units, with the majority of incidents not formally reported.

  Background Management of noncommunicable diseases (NCDs) is an increasing challenge for health care systems. Although remote patient monitoring presents a promising solution by utilizing technology to monitor patients outside clinical settings, there is a lack of knowledge about the effect on resource utilization. Objective This systematic review aimed to review the effects of remote patient monitoring on health care resource utilization by patients with NCDs. Methods Eligible randomized controlled trials (RCTs) involved digital transmission of health data from patients to health care personnel. Outcomes included hospitalizations, length of stay, outpatient visits, and emergency visits. A systematic literature search was performed in Medline, Embase, and Cochrane Central Register of Controlled Trials in June 2024. Titles, abstracts, and full texts were screened individually by 2 authors. Risk of bias was assessed, and data were extracted, analyzed, and pooled in meta-analysis when possible. Confidence in the estimates was assessed using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach. Results We included 40 RCTs published between 2017 and 2024. The largest group of NCDs was cardiovascular disease (16 studies). Remote patient monitoring may slightly decrease the proportion of hospitalizations compared with usual care (risk ratio [RR] 0.86, 95% CI 0.77 to 0.95; low certainty). Compared with usual care, remote patient monitoring had fewer or an equal number of hospitalizations (mean difference –0.13, 95% CI –0.29 to 0.03; low certainty). Hospital length of stay may be slightly reduced with remote patient monitoring compared with usual care (mean difference –0.84, 95% CI –1.61 to –0.06 days; low certainty). The proportion of outpatient visits showed probably little to no difference between remote patient monitoring and usual care (RR 0.94, 95% CI 0.87 to 1.02; moderate certainty). Compared with usual care, remote patient monitoring had slightly more outpatient visits, but the CI was wide (mean difference 0.41, 95% CI –0.22 to 1.03; low certainty). The results indicate a small or no difference between remote patient monitoring and usual care regarding proportion of emergency visits (RR 0.91, 95% CI 0.79 to 1.05; low certainty). We are uncertain whether remote patient monitoring increases or decreases the number of emergency visits, as the evidence was of very low certainty. Conclusions This systematic review showed that remote patient monitoring possibly led to lower proportions of patients being hospitalized, fewer hospitalizations, and shorter hospital length of stay compared with usual care. Patients undergoing remote monitoring had possibly more outpatient visits compared with usual care. The proportions of patients with outpatient visits or emergency visits were probably similar. Finally, we had very low certainty in the number of emergency visits. The results should be considered with caution as the certainty of evidence was moderate to very low. We did not find results regarding institutional stay.

  Abstract Background  Exercise is recommended for patients with hip/knee osteoarthritis (OA), however, adherence to exercise tends to be poor. This study aimed to (1) compare in-person versus app-based adherence to exercise therapy, (2) identify patient characteristics associated with adherence, and (3) examine associations between adherence and change in disease-specific outcomes in hip/knee OA patients. Method  Data were collected as part of a randomized controlled trial. In-person physiotherapy treatment involved supervised exercise therapy twice weekly for 6 weeks, complemented by an additional weekly home-exercise session. The app group received an individually tailored exercise program in the Virtual Training-app for 6 weeks and were advised to exercise three times weekly. Adherence was recorded as number of exercise sessions attended. Patient characteristics were reported at baseline. Changes in disease-specific outcomes were evaluated at 6-weeks follow-up using Hip/Knee Injury and Osteoarthritis Outcome Score. Associations were assessed using multiple logistic and linear regression analyses. Results In total 68 patients, 34 in each group were included (63 years, 69% female). The odds of adherence were 4.2 times higher when exercise was supervised by physiotherapists (95% CI 1.5, 12.4; p = 0.008). High fatigue was associated with lower adherence (OR 0.8, p = 0.049), whereas higher education (OR 5.2, p = 0.02) and self-efficacy (OR 3.2, p = 0.008) were associated with higher adherence. No significant associations were found between disease-specific outcomes and adherence. Conclusion App-based exercise did not improve adherence compared to supervised exercise. Apart from fatigue, education, and self-efficacy, no factors were found to influence adherence to exercise in hip/knee OA patients. Trial registration ClinicalTrials.gov (NCT04767854).

  Background: Axial spondyloarthritis (axSpA) is a chronic inflammatory joint disease affecting the spine and sacroiliac joints, requiring frequent, lifelong monitoring and treatment. This involves regular symptom monitoring, assessing medication tolerance and side effects, and prompt therapy adjustments. Typically, patients with axSpA attend prescheduled hospital visits, but once stable disease has been attained, these seldom align with periods of high disease activity. Remote monitoring and patient-initiated care offer flexible, need-based, follow-up options. However, knowledge about how patients with axSpA perceive and experience these approaches is limited. To effectively implement these strategies in clinical practice, understanding patient perspectives is crucial. Objective: This study aims to explore how patients with axSpA perceive and experience remote monitoring and patient-initiated care. Methods: Our qualitative study was embedded in a randomized controlled trial. Participants were allocated to either usual care, remote monitoring, or patient-initiated care. The 2 intervention groups had no prescheduled visits and used a remote monitoring app, but only the remote monitoring group received monitoring by health care professionals. Semistructured interviews were conducted with 18 participants from the intervention groups to explore their experiences. The interviews were audio recorded, transcribed, anonymized, and analyzed using thematic analysis. Participants provided informed consent. Results: Eighteen patients (11 men and 7 women, aged 26-65 years) participated, 10 from the remote monitoring group and 8 from the patient-initiated care group. Transcripts were analyzed into four key themes: (1) “I don’t need to go to the hospital just to report I’m doing well.” When patients felt well, they perceived in-person consultations as less important. They acknowledged health care resource challenges and were willing to adapt but expressed concerns about rapid technological advancement, fearing it could exclude vulnerable groups. They emphasized the need for shared decision-making in determining follow-up strategies; (2) “It feels safer to meet healthcare personnel in person” highlighted participants’preference for in-person interactions as a safety net for detecting changes or signs of disease. They felt more secure when communicating face-to-face with health care professionals; (3) Remote monitoring can promote a sense of freedom and self-efficacy.   Background: People with hand osteoarthritis (OA) often have poor access to recommended treatments. To enhance care quality, quality indicators (QIs) based on clinical recommendations are essential. Current QI sets, like the Osteoarthritis Quality Indicator Questionnaire (OA-QI v.2), primarily address hip- and knee OA, and not hand OA. Objectives: To adapt the OA-QI v.2 for assessing patient-reported quality of hand OA care. Design: We used the OA-QI v.2. set as a starting point and adapted it to reflect hand OA care. A literature search was performed to identify potential QIs for hand OA following the Rand/UCLA Appropriateness method. A European expert panel, comprising researchers, clinicians, and patient research partners, participated in online meetings to discuss adaptation and suggest new QIs based on treatment recommendations for hand OA, and anonymously rated each suggested QI regarding its importance, validity, usefulness, and feasibility. Consensus was defined by predefined rating cut-off scores. The adapted questionnaire was translated from English into Norwegian. Cognitive debriefing interviews with Norwegian and UK hand OA patients were conducted to ensure clarity. Results: Our initial literature search provided 1670 articles, with none describing relevant QIs. After three voting rounds, sixteen QI items reached consensus, reflecting current hand OA care standards. Items were generally well understood, requiring only minor clarity amendments after patient interviews (N ¼ 28).

  Clinical trials are essential for evaluating the efficacy and safety of new treatments and health interventions. However, while pharmacological trials are well-established, non-pharmacological trials face unique challenges related to their complexity and difficulties such as recruitment, retention, intervention standardisation, selection of outcome measures and blinding of clinicians, participants and data collectors. This communication paper describes the objectives, implementation steps and bylaws of the ‘Trials foR heAlth Care inTerventIONs’ Network (TRACTION), established by an international multiprofessional task force of experts to foster high-quality non-pharmacological research, ultimately improving patient care and healthcare outcomes. The TRACTION research network will provide information and resources through a collaborative hub for researchers, health professionals, patient research partners and stakeholders in diverse biomedical and healthcare areas, connecting people with different levels of expertise but with the same interests (eg, to evaluate the effect of non-pharmacological interventions, recruiting participants). This open network will support researchers in optimising trial design, participant recruitment, data management and analysis, and disseminating and implementing trial results. The network will also facilitate specialisation training and provide educational materials and mentoring.

  Remote monitoring using electronic patient reported outcomes (ePROs) in axial spondyloarthritis (axSpA) may improve self-management and reduce the need for consultations. However, knowledge regarding patients’ willingness to use remote care and adherence to reporting ePROs is scarce. The objective of this study was to assess axSpA patients’ willingness to use remote care and adherence to reporting of ePROs. The study was part of a three-armed randomized controlled trial testing digital follow-up strategies (The ReMonit study, NCT: 05031767). AxSpA patients in low disease activity were randomized to usual care, remote monitoring, or patient-initiated care. Demographics, clinical data, and patients’ willingness to use remote care were collected at baseline. EPROs were reported either monthly or quarterly by the remote monitoring- and patient-initiated care group over 18 months, respectively. Adherence to reporting was calculated as number of ePROs completed divided by the total number requested. Mixed model logistic regression was utilized to assess factors associated with adherence to reporting of ePROs. In total 242 patients (median age 43 years, 75% males) were included. The majority (96%) reported high willingness to use remote care. Adherence to reporting ePROs remained high over 18 months by remote monitoring and patient-initiated care groups [median (IQR): 88% (77–100) vs. 83% (66–100)]. No patient characteristics were significantly associated with adherence to reporting of ePROs. The high degree of willingness and adherence to reporting ePROs over time indicates that the majority of axSpA patients with low disease activity are motivated to use remote care.

  Objective: To evaluate the efficacy of strength exercise or aerobic exercise compared to usual care on knee-related quality of life (QoL) and knee function at 4 months and 1 year in individuals with knee osteoarthritis. Methods: A three-arm randomized controlled trial (RCT) compared 12 weeks of strength exercise or aerobic exercise (stationary cycling) to usual care supervised by physiotherapists in primary care. We recruited 168 participants aged 35–70 years with symptomatic knee osteoarthritis. The primary outcome was The Knee Injury and Osteoarthritis Outcome Score (KOOS) QoL at 1 year. Secondary outcomes were self-reported function, pain, and self-efficacy, muscle strength and maximal oxygen uptake (VO2max) at 4 months and 1 year. Results: There were no differences between strength exercise and usual care on KOOS QoL (6.5, 95% CI -0.9 to 14), or for aerobic exercise and usual care (5.0, 95% CI -2.7 to 12.8), at 1 year. The two exercise groups showed better quadriceps muscle strength, and VO2max at 4 months, compared to usual care. Conclusion: This trial found no statistically significant effects of two exercise programs compared to usual care on KOOS QoL at 1 year in individuals with symptomatic and radiographic knee osteoarthritis, but an underpowered sample size may explain lack of efficacy between the intervention groups and the usual care group.

  Objective To assess the quality of care, effectiveness, and cost-effectiveness over 12 months after implementing a structured model of care for hip and knee osteoarthritis (OA) in primary healthcare as compared to usual care. Design In this pragmatic cluster-randomized, controlled trial with a stepped-wedge cohort design, we recruited 40 general practitioners (GPs), 37 physiotherapists (PTs), and 393 patients with symptomatic hip or knee OA from six municipalities (clusters) in Norway. The model included the delivery of a 3-hour patient education and 8–12 weeks individually tailored exercise programs, and interactive workshops for GPs and PTs. At 12 months, the patient-reported quality of care was assessed by the OsteoArthritis Quality Indicator questionnaire (16 items, pass rate 0–100%, 100%=best). Costs were obtained from patient-reported and national register data. Cost-effectiveness at the healthcare perspective was evaluated using incremental net monetary benefit (INMB). Results Of 393 patients, 109 were recruited during the control periods (control group) and 284 were recruited during interventions periods (intervention group). At 12 months the intervention group reported statistically significant higher quality of care compared to the control group (59% vs. 40%; mean difference: 17.6 (95% confidence interval [CI] 11.1, 24.0)). Cost-effectiveness analyses showed that the model of care resulted in quality-adjusted life-years gained and cost-savings compared to usual care with mean INMB €2020 (95% CI 611, 3492) over 12 months. Conclusions This study showed that implementing the model of care for OA in primary healthcare, improved quality of care and showed cost-effectiveness over 12 months compared to usual care.

  Background: Patients with chronic inflammatory joint diseases such as axial spondyloarthritis have traditionally received regular follow-up in specialist health care to maintain low disease activity. The follow-up has been organized as prescheduled face-to-face visits, which are time-consuming for both patients and health care professionals. Technology has enabled the remote monitoring of disease activity, allowing patients to self-monitor their disease and contact health care professionals when needed. Remote monitoring or self-monitoring may provide a more personalized follow-up, but there is limited research on how these follow-up strategies perform in maintaining low disease activity, patient satisfaction, safety, and cost-effectiveness.<p> <p>Objective: The Remote Monitoring in Axial Spondyloarthritis (ReMonit) study aimed to assess the effectiveness of digital remote monitoring and self-monitoring in maintaining low disease activity in patients with axial spondyloarthritis.<p> <p>Methods: The ReMonit study is a 3-armed, single-site, randomized, controlled, open-label noninferiority trial including patients with axial spondyloarthritis with low disease activity (Ankylosing Spondylitis Disease Activity Score <2.1) and on stable treatment with a tumor necrosis factor inhibitor. Participants were randomized 1:1:1 to arm A (usual care, face-to-face visits every sixth month), arm B (remote monitoring, monthly digital registration of patient-reported outcomes), or arm C (patient-initiated care, self-monitoring, no planned visits during the study period). The primary end point was disease activity measured with the Ankylosing Spondylitis Disease Activity Score, evaluated at 6, 12, and 18 months. We aimed to include 240 patients, 80 in each arm. Secondary end points included other measures of disease activity, patient satisfaction, safety, and cost-effectiveness.<p> <p>Results: The project is funded by the South-Eastern Norway Regional Health Authority and Centre for the treatment of Rheumatic and Musculoskeletal Diseases (REMEDY), Diakonhjemmet Hospital, Norway. Enrollment started in September 2021 and was completed with 242 patients by June 2022. The data collection will be completed in December 2023.<p> <p>Conclusions: To our knowledge, this trial will be among the first to evaluate the effectiveness, safety, and cost-effectiveness of remote digital monitoring and self-monitoring of patients with axial spondyloarthritis compared with usual care. Hence, the ReMonit study will contribute important knowledge to personalized follow-up strategies for patients with axial spondyloarthritis. These results may also be relevant for other patient groups with inflammatory joint diseases.

  BAKGRUNN OG FORMÅL Urinsyregikt er den vanligste inflammatoriske revmatiske sykdommene blant voksne, og forekomsten er økende. Det finnes effektiv medikamentell behandling for å senke urinsyrenivået i blod, og dermed unngå anfall med smertefulle leddbetennelser. Dessverre er det mange pasienter som ikke får oppfølging i henhold til anbefalinger og/eller har lav etterlevelse av behandlingen. ReMonit Gout er et pågående forskingsprosjekt med mål om å utvikle og teste en mestringsapp for pasienter med urinsyregikt. Appen kan potensielt bidra til å gi flere pasienter oppfølging i tråd med anbefalte retningslinjer, og spare pasienter og helsepersonell for tid og ressurser. Prosjektet har fått støtte fra Helse Sør-Øst, Stiftelsen Dam og REMEDY. Delmålene for prosjektet er: 1) Å utvikle og teste anvendbarheten av mestringsappen Urika, for pasienter som følges opp i spesialisthelsetjenesten. 2) Å teste om digital oppfølging med Urika-appen ikke er underlegen ‘state-of-the art’ oppfølging i spesialisthelsetjenesten, i å oppnå og vedlikeholde et lavt serum urinsyrenivå (<360/<300 μmol/L). 3) Å tilpasse Urika-appen til pasienter som kun følges opp av fastlegen, og teste om appen er anvendbar til bruk i primærhelsetjenesten. METODE Brukermedvirkere, forskere og klinikere har utviklet innholdet i Urika-appen sammen med firmaet Youwell. Urika-appen inneholder bl.a. informasjon om urinsyregikt, påminnelser om medisin og blodprøver, oversikt over urinsyrenivå, og en algoritme for utregning av medisindose til den enkelte pasient. Appen ble testet i en anvendbarhetsstudie ved Diakonhjemmet sykehus av pasienter som startet urinsyresenkende behandling. Pasientene svarte på spørreskjema ved studiestart, månedlig, og etter 3 måneder. I tillegg deltok 10 pasienter i semistrukturerte intervjuer om erfaringer med appen, og vi samlet data fra appen om bruk og registreringer. Videre skal vi gjennomføre en randomisert kontrollert ikke-underlegenhetsstudie (RCT) ved Diakonhjemmet sykehus. Totalt 168 pasienter med urinsyregikt vil bli randomisert til enten digital oppfølging med Urika-appen, eller til dagens telefonbaserte oppfølging fra sykepleier. Hovedutfallsmålet er andelen pasienter med lavt serum urinsyrenivå (<360/<300 μmol/L) ved 12 måneder. Vi vil også gjøre en helseøkonomisk evaluering. Urika-appen skal videreutvikles for primærhelsetjenesten i samarbeid med brukerrepresentanter og fastleger. Deretter vil vi rekruttere 30 pasienter og 10 fastleger til å teste appen i en anvendbarhetsstudie, for å evaluere anvendbarheten av appen for pasienter med urinsyregikt som følges opp av fastlegen. Pasientene og fastlegene vil bli spurt om å svare på spørreskjema og delta i semistrukturerte intervjuer om erfaringer med bruk av Urika-appen som støtte i behandlingen i primærhelsetjenesten. RESULTATER Etter utvikling og teknisk testing av Urika-appen, ble 21 pasienter inkludert i anvendbarhetsstudien ved Diakonhjemmet Sykehus fra mars-juni 2024, alle var menn med en gjennomsnittsalder på 50 år. Appen skåret høyt på brukervennlighet, og det ble rapportert få uønskede hendelser/feil med appen. Totalt 87% av endepunktene for anvendbarhet ble nådd. Det var over den prespesifiserte akseptable grensen for anvendbarhet (≥70%), og inkluderte kliniske utfallsmål, teknisk og operasjonell anvendbarhet. Urika-appen ble forbedret basert på resultatene fra anvendbarhetsstudien. Inklusjon av deltakere til RCTen startet i juni 2025, det var inkludert 5 pasienter i studien ved utgangen av august 2025. Vi har fått innspill fra fastleger og brukerrepresentanter med forslag om tilpasninger av Urika-appen for bruk i primærhelsetjenesten, og praktisk gjennomføring av anvendbarhetsstudien blant pasienter som følges opp av fastlegen. Vi vil gjøre nødvendige endringer og teknisk prøving av appen før vi begynner rekruttering av pasienter og fastleger til studien. KONKLUSJON I ReMonit Gout prosjektet har vi utviklet mestringsappen Urika for å støtte pasienter som skal starte urinsyresenkende behandling. Vi skal teste appen blant pasienter med urinsyregikt som følges opp i spesialist- og primærhelsetjenesten.
