Advance care planning for seriously ill elderly persons living at home

The overarching goal of the research project is to improve health care for seriously ill older patients who live at home, and their next of kin through better implementation of advance care planning (ACP). 

ACP can improve health services by safeguarding user involvement and patient autonomy, strengthening communication between patient, next of kin and health personnel, and by avoiding under and overtreatment. The aim of the project is to map the current level of implementation of ACP, and explore the barriers and facilitators for full implementation, as well as the experiences, needs and preferences among the stakeholders involved in ACP.

Methods:

 

  • Developing a fidelity scale for ACP as an evidence-based method to measure the current level of implementation of ACP in selected hospital wards and community services.
  • Semi-structured interview guides to investigate needs and preferences, barriers and facilitators, and experiences among leaders, health care professionals, patients and next of kin involved in ACP.
