  Research Type: Level 1 - Randomized controlled trial (RCT), Meta-analysis of randomized trials with homogeneous results Introduction/Purpose: Displaced or unstable Lisfranc injuries typically require surgery. Primary arthrodesis (PA) of the medial tarsometatarsal (TMT) joints provides stabilization but compromises motion. In contrast, open reduction and internal fixation (ORIF) with bridge plating (BP) offers extra-articular stabilization, preserving joint surfaces and mobility, though requiring later hardware removal. Preservation might be particularly advantageous for the first TMT joint, which has greater physiological motion than TMT 2 and 3. This study evaluates the long-term clinical and radiographic outcomes of PA versus BP for first TMT joint stabilization in unstable Lisfranc injuries. We hypothesized that BP would result in superior clinical outcomes and lower adjacent joint osteoarthritis rates compared to PA, given its potential to maintain joint mobility. Methods: This study presents the long-term follow-up of an IRB-approved randomized controlled trial comparing PA to BP of the first TMT joint in patients with acute, unstable Lisfranc injuries. Eligible patients, aged 18-65, had unstable Lisfranc injuries without fractures in relation to the first TMT joint. Instability was defined as either primary displacement or a positive manual stress test under fluoroscopy. Exclusion criteria were polytrauma, open injuries, diabetes, and neuropathy. Patients were randomized to PA of the first TMT joint or BP with plate removal at four months; all underwent PA of the second and third TMT joints. Blinded examiners assessed AOFAS midfoot score, MOxFQ, VAS pain scores, SF-36, return to activity, work, and orthotic use. CT scans were evaluated for union and secondary midfoot osteoarthritis. We used Wilcoxon rank-sum and t-tests for continuous variables, chi-square or Fisher’s exact tests for categorical data, and Spearman’s correlation for associations (P <.05 significant). Results: Of 48 enrolled patients, 41 (85%) completed clinical and radiographic follow-up at a mean of 10.8 years (SD 1.2) post-injury. Groups did not differ in age, BMI, or gender. All clinical scores, complication/ reoperation rates, return to activity, occupational status, and orthotic use showed no significant differences. Osteoarthritis was most prevalent in the naviculocuneiform (NC) joints, without between-group differences. NC2/3 osteoarthritis severity correlated with poorer clinical outcomes, strongest for AOFAS (ρ= -0.57, p=0.0002) and VAS at activity (ρ=+0.51, p=0.0009). Orthotic use increased with osteoarthritis severity (ρ=0.3612, p=0.0152). In the BP group, one patient underwent secondary TMT1 fusion, however, per-protocol analysis showed no altered results; 75% had mild/moderate and 10% severe TMT1 osteoarthritis, with a negative correlation between return to activity and TMT1 osteoarthritis (ρ=-0.6292, p=0.0072). Conclusion: This 10-year follow-up of a randomized controlled trial found no significant differences in clinical or radiographic outcomes between PA and BP for unstable Lisfranc injuries. Based on our results, preserving the first TMT joint which requires additional surgery (hardware removal) does not appear to provide added benefit. Given the comparable clinical outcomes and the potential for progressive TMT1 degeneration with BP, PA should be considered the preferred treatment for displaced or unstable Lisfranc injuries.

  Abstract Purpose A tendon transfer is a common method of treating ankle plantar flexion weakness and tendon end non‐union following chronic Achilles tendon rupture and delayed representation following Achilles tendon re‐rupture. Commonly, the transferred tendon is fixed into a bone tunnel on the postero‐superior surface of the calcaneum close to the distal Achilles tendon insertion. To date, there is no standardised description or measurement of calcaneal tunnel position. The aim of this study is to describe the anatomic location for calcaneal tunnel placement and to determine the reliability of a method of measuring tunnel position and direction within the calcaneum. Methods The routine post‐operative lateral ankle radiographs from 40 patients (40 ft) following Achilles tendon reconstruction using tendon transfer into the calcaneum: calcaneal tunnel zone (CTZ), calcaneal tunnel ratio (CTR) and calcaneal tunnel angle (CTA) were tested for reliability using test‐retest between three observers. Additionally, CTR and CTA were compared in cases where a calcaneoplasty was performed or not. Results The intraclass correlation coefficient (ICC) of the CTR and CTA was found to be 0.86–0.95 (95% confidence interval [CI]: 0.75‐0.98) and 0.95–0.99 (95% CI: 0.92–0.99), respectively, indicating good and excellent reliability. Patients who received a calcaneoplasty had a significantly greater CTR of 0.74 (0.1) and a lower CTA of 76.1° (10.8) compared to those who did not have a CTR of 0.61 (0.1) and 100.9 (12.4), Diff 95% CI: 0.13 (0.08–0.18) and −25 (−32 to −17), respectively, both p  < 0.001. Conclusions The CTR and CTA were reliable measures for the calcaneal tunnel following Achilles tendon reconstruction using tendon transfer within the limitations of the sagittal radiographic view. When a calcaneoplasty was performed, it resulted in a significantly greater CTR. These measurements should be used to describe calcaneal tunnels rather than a description of tunnel placement to optimise predictive factors following Achilles tendon reconstruction.   Background: Evidence from prospective short-term studies suggest that proximal medial gastrocnemius recession is a safe and efficient procedure to treat chronic plantar fasciitis resistant to nonoperative treatment. The aim of this study was to evaluate the long-term clinical outcomes of proximal medial gastrocnemius recession and stretching compared to a stretching exercise protocol for patients with chronic plantar fasciitis and an isolated gastrocnemius contracture (IGC). Methods: Forty patients with plantar fasciitis lasting more than 1 year were prospectively randomized to a home stretching exercise program only, or to proximal medial gastrocnemius recession in addition to the stretching program. Clinical and functional data in this study were obtained at baseline and 6-year follow-up. The main outcome was the American Orthopaedic Foot & Ankle Society (AOFAS) ankle-hindfoot score. Secondary outcomes were the visual analog scale (VAS) for pain, the Manchester Oxford Foot Questionnaire (MOxFQ), ankle dorsiflexion, and Achilles complex performance. Results: Thirty-three of 40 patients completed the 6-year follow-up. Seven patients had crossed over from nonoperative treatment to operative treatment. At 6 years, the operative group demonstrated significantly better outcomes with AOFAS (88.9 vs 78.6, P = .012), for pain measured by VAS (2.5 vs 5.5, P < .001) and with the MOxFQ total score (24.4 vs 45.9, P = .05) (per protocol analysis excluding crossovers). No between-group differences were observed for ankle dorsiflexion or Achilles complex performance at 6 years. Conclusion: This study demonstrates that the improved function and reduced level of pain by proximal medial gastrocnemius recession and stretching is better compared to stretching alone after 6 years of follow-up for patients with chronic plantar fasciitis and a concomitant isolated gastrocnemius contracture.

  Objectives: Trans-syndesmotic fixation with suture buttons, posterior malleolar fixation (PMF) with screws and anterior inferior tibiofibular ligament (AITFL) augmentation with suture tape have all been suggested as potential treatments in the setting of a malleolar fracture. However, there is no consensus on the optimal treatment for small vs. large malleolar fractures. The purpose of this study was to determine which combination of: 1) posterior malleolar screw fixation, 2) syndesmotic fixation with suture button (SB), and 3) AITFL augmentation with suture tape (ST) best restored native tibio-fibular and ankle joint kinematics following a small and large posterior malleolar fracture. Methods: Twenty fresh frozen cadaveric lower leg specimens were divided into two groups and underwent biomechanical testing using a 6-degrees-of-freedom robotic arm in 7 states: 1) Native, 2) Syndesmosis Injury + Malleolar Fracture (Group 1: small fracture, Group 2: large fracture) 3) Screw fixation, 4) Screw + Suture Tape Augmentation, 5) Screw + Suture Tape Augmentation + Suture Button, 6) Suture Button + Suture Tape Augmentation, 7) Suture Button. Four biomechanical tests were performed at neutral and at 30 degrees of plantarflexion: 1) Internal Rotation, 2) External Rotation, 3) Lateral Drawer, 4) Posterior Drawer. The position of the tibia, fibula and talus were continuously recorded using a 5-camera motion capture system. Results: No differences were found in the efficacy of treatments between the small fracture and large fracture groups. In the external rotation test, screws with ST augmentation resulted in best stability of the fibula and ankle joint. In the internal rotation test, all repairs that included posterior malleolar screws stabilized the fibula and ankle. Posterior and lateral drawer of the foot resulted in only small differences between the intact and malleolar fracture states. Conclusions: Posterior malleolar fixation resulted in higher syndesmotic stability compared to trans-syndesmotic fixation with SBs. AITFL augmentation with ST provided additional external rotation stability when combined with screw fixation. Posterior malleolar screw fixation with AITFL augmentation using ST may be the preferred surgical method when treating patients with acute ankle injury involving an unstable syndesmosis injury and a posterior malleolar fragment.   Background The aim of this study was to evaluate the outcome after nondisplaced and stable Lisfranc injuries. Methods 26 patients with injuries to the Lisfranc joint complex detected on CT scans, but without displacement were tested to be stable using a fluoroscopic stress test. The patients were immobilized in a non-weightbearing short leg cast for 6 weeks. The final follow-up was 55 (IQR 53–60) months after injury. Results All the Lisfranc injuries were confirmed to be stable on follow-up weightbearing radiographs at a minimum of 3 months after injury. Median American Foot and Ankle Society (AOFAS) midfoot score at 1-year follow-up was 89 (IQR 84–97) and at final follow-up 100 (IQR 90–100); The AOFAS score continued to improve after 1-year (P=.005). The median visual analog scale (VAS) for pain was 0 (IQR 0–0) at the final follow-up. One patient had radiological signs of osteoarthritis at 1-year follow-up. Conclusion Stable Lisfranc injuries treated nonoperatively had an excellent outcome in this study with a median follow-up of 55 months. The AOFAS score continued to improve after 1 year.

  Background. Chronic syndesmotic injury may cause long-term pain and reduced ankle function. Today, there is no consensus about the best surgical treatment of these injuries. We present the technique and results of revision and fixation with a suture button and a quadricortical screw. Methods. Eleven patients treated for chronic syndesmotic injury were included. The patients completed questionnaires regarding ankle function, and computed tomography scans were obtained to evaluate tibiofibular distance and osteoarthritis. Complications were registered. Results. At mean 45 months follow-up, the mean American Orthopaedic Foot and Ankle Society ankle-hindfoot score was 87 points. There were 5 complications, including 3 hardware-related pain. The tibiofibular distance was significantly reduced with surgery. Seven patients had progression of osteoarthritis. Conclusions. Even though the majority of the patients had progression of radiological signs of osteoarthritis, the functional outcome after revision and fixation with a suture button and a quadricortical screw in chronic syndesmotic ruptures is good and comparable to the results presented in other studies.   Background: Unstable Lisfranc injuries are best treated with anatomic reduction and stable fixation. There are controversies regarding which type of stabilization is best. In the present study, we compared primary arthrodesis of the first tarsometatarsal (TMT) joint to temporary bridge plating in unstable Lisfranc injuries. Methods: Forty-eight patients with Lisfranc injuries were included and followed for 2 years. Twenty-four patients were randomized to primary arthrodesis (PA) of the medial 3 TMT joints, whereas 24 patients were randomized to temporary bridge plate (BP) over the first TMT joint and primary arthrodesis of the second and third TMT joints. The main outcome parameter was the American Orthopaedic Foot &amp; Ankle Society (AOFAS) midfoot scale and the secondary outcome parameters were the 36-Item Short Form Health Survey (SF-36) and visual analog scale for pain (VAS pain). Computed tomography (CT) scans pre- and postoperatively were obtained. Radiographs were obtained at follow-ups. Pedobarographic examination was performed at the 2-year follow-up. Twenty-two of 24 patients in the PA and 23/24 in the BP group completed the 2-year follow-up. Results: The mean AOFAS midfoot score 2 years postoperatively was 89 (SD 9) in the PA group and 85 (SD 15) in the BP group ( P = .32). There were no significant differences between the groups with regard to SF-36 or VAS pain scores. The alignment of the first metatarsal was better in the BP group than in the PA group measured by the anteroposterior Meary angle ( P = .04). The PA group had a reduced peak pressure under the fifth metatarsal ( P = .047). In the BP group, 11/24 patients had radiologic signs of osteoarthritis in the first TMT joint. Conclusion: Both treatment groups had good outcome scores. The first metatarsal was better aligned in the BP group; however, there was a high incidence of radiographic osteoarthritis in this group.   Lisfranc injuries consist of a wide spectrum of injuries, ranging from subtle injuries to severe fracture-dislocations. Injuries with instability of the tarsometatarsal, intercuneiform, or naviculocuneiform joints should be treated with anatomic reduction and stable fixation. The best method of fixation is debated. Transarticular screw fixation has the disadvantage of damaging the tarsometatarsal joints. Bridging the tarsometatarsal joints with use of low-profile locking plates avoids the placement of screws through the joint and potentially reduces the risk of posttraumatic arthritis. Primary arthrodesis of the 3 medial tarsometatarsal joints is also an option in treating Lisfranc injuries and has been shown to lead to better outcomes compared with transarticular screw fixation in ligamentous Lisfranc injuries. In this article, we show the technique of open reduction and internal fixation of Lisfranc fracture-dislocation with use of dorsal bridging locking plates. The following steps are presented in the video: (1) incision technique with use of a dorsomedial incision and a dorsolateral incision, (2) open reduction and temporary fixation of the tarsometatarsal joints with use of Kirschner wires, (3) confirmation of anatomic reduction of the tarsometatarsal joints with direct visualization and fluoroscopy, (4) fixation of the medial 3 tarsometatarsal joints with dorsal bridging locking plates, (5) placement of a “homerun” screw from the medial cuneiform to the base of the second metatarsal, (6) fixation of the fourth and fifth tarsometatarsal joints with Kirschner wires, and (7) checking of reduction and fixation with use of fluoroscopy and performance of wound closure. Postoperatively, the foot is kept non-weight-bearing in a below-the-knee cast for 6 weeks, followed by 6 weeks of protected weight-bearing in a walker boot. Any Kirschner wires fixating the fourth and fifth tarsometatarsal joints are removed 6 weeks postoperatively. We prefer to remove the dorsal bridging plates 4 to 6 months postoperatively. Anatomic reduction and stable fixation is associated with better functional outcomes. Hardware failure and loss of reduction are potential complications that can lead to worse outcomes.

  Background In Lisfranc injuries the stability of the tarsometatarsal joints guides the treatment of the injury. Determining the stability, especially in the subtle Lisfranc injuries, can be challenging. The purpose of this study was to identify incidence, mechanisms of injury and predictors for instability in Lisfranc injuries. Methods Eighty-four Lisfranc injuries presenting at Oslo University Hospital between September 2014 and August 2015 were included. The diagnosis was based on radiologically verified injuries to the tarsometatarsal joints. Associations between radiographic findings and stability were examined. Results The incidence of Lisfranc injuries was 14/100,000 person-years, and only 31% were high-energy injuries. The incidence of unstable injuries was 6/100,000 person–years, and these were more common in women than men (P = 0.016). Intraarticular fractures in the two lateral tarsometatarsal joints increased the risk of instability (P = 0.007). The height of the second tarsometatarsal joint was less in the unstable injuries than in the stable injuries (P = 0.036). Conclusion The incidence of Lisfranc injuries in the present study is higher than previously published. The most common mechanism of injury is low-energy trauma. Intraarticular fractures in the two lateral tarsometatarsal joints, female gender and shorter second tarsometatarsal joint height increase the risk of an unstable injury.

  Background The transfer of Flexor Hallucis Longus Tendon (FHL) is an established method for the treatment of chronic Achilles tendon ruptures. An extensive examination of power, strength, endurance and complications related to this procedure is presented. Methods 21 patients treated with open FHL transfer for chronic Achilles tendon rupture were studied retrospectively. Medical records were reviewed. The patients were examined with a test battery for triceps surae strength, functional tests and PROMs. Results The median maximal concentric strength was equal,1300 vs 1336 W, comparing affected with unaffected side. The endurance tests showed a larger difference, 219 J vs. 2398 J, respectively. The median AOFAS score was 87. 11 of 21 patients sustained one or more complications; the most common were infection, disturbed wound healing, and clawing of small toes. Conclusions Patients achieve almost normal maximal strength after open FHL transfer, but endurance is notably lower. The complication rate was high.   Background Early intramedullary nailing (IMN) of long bone fractures in severely injured patients has been evaluated as beneficial, but has also been associated with increased inflammation, multi organ failure (MOF) and morbidity. This study was initiated to evaluate the impact of primary femoral IMN on coagulation-, fibrinolysis-, inflammatory- and cardiopulmonary responses in polytraumatized patients. Methods Twelve adult polytraumatized patients with femoral shaft fractures were included. Serial blood samples were collected to evaluate coagulation-, fibrinolytic-, and cytokine activation in arterial blood. A flow-directed pulmonary artery (PA) catheter was inserted prior to IMN. Cardiopulmonary function parameters were recorded peri- and postoperatively. The clinical course of the patients and complications were monitored and recorded daily. Results Mean Injury Severity Score (ISS) was 31 ± 2.6. No procedure-related effect of the primary IMN on coagulation- and fibrinolysis activation was evident. Tumor necrosis factor alpha (TNF-α) increased significantly from 6 hours post procedure to peak levels on the third postoperative day. Interleukin-6 (IL-6) increased from the first to the third postoperative day. Interleukin-10 (IL-10) peaked on the first postoperative day. A procedure-related transient hemodynamic response was observed on indexed pulmonary vascular resistance (PVRI) two hours post procedure. 11/12 patients developed systemic inflammatory response syndrome (SIRS), 7/12 pneumonia, 3/12 acute lung injury (ALI), 3/12 adult respiratory distress syndrome (ARDS), 3/12 sepsis, 0/12 wound infection. Conclusion In the polytraumatized patients with femoral shaft fractures operated with primary IMN we observed a substantial response related to the initial trauma. We could not demonstrate any major additional IMN-related impact on the inflammatory responses or on the cardiopulmonary function parameters. These results have to be interpreted carefully due to the relatively few patients included. Trial Registration ClinicalTrials.gov: NCT00981877
