  .Papirløse migranter utgjør en liten gruppe som lever marginaliserte liv, med spesielle behov i møte med helsetjenesten. De navigerer i en kompleks hverdag preget av uformelt arbeid og midlertidige boforhold samtidig som de forsøker å ivareta egen og andres helse. Mange har sammensatte helseutfordringer og behov for langvarig oppfølging. Rettighetsbegrensninger, språkproblemer, manglende personnummer og frykt for utsendelse påvirker behandlingsforløp og gir praktiske hindringer i klinisk arbeid. I denne kronikken belyser vi konkrete kliniske og organisatoriske utfordringer med utgangspunkt i en kasuistikk og egne erfaringer fra Helsesenter for papirløse migranter, og vi peker på løsninger som kan bidra til tryggere og mer likeverdige møter mellom leger og denne pasientgruppen.

  Background and aim The aim of the study was to investigate patients' attitudes to and experiences of consultations in general practice where medical students acted as observers or conducted independent consultations. Participants were also asked about the advantages and disadvantages of student involvement. Material and method We conducted a cross-sectional study in which patients completed an online questionnaire in general practitioners' (GPs') waiting rooms, having been invited to participate by medical students in a clinical placement. The results are presented descriptively. Results A total of 2263 out of 5576 (41 %) invited patients participated. Of these, 1287/2263 (57 %) experienced consultations involving students. Of these, 890/1277 (70 %) reported that they valued the experience and 1009/1248 (81 %) agreed that it was a positive experience. Of the 847 patients who clearly recalled a consultation involving a student, 763/837 (91 %) reported feeling safe and 806/846 (95 %) felt they received adequate care. Interpretation Although the vast majority of patients in the study felt well cared for in consultations involving medical students in clinical placement in general practice, special attention should be paid to vulnerable patient groups.

  Introduction Long-lasting pain in adolescents may affect education, social interactions, and is associated with mental health challenges. Current interventions are often suboptimal due to insufficient understanding of the challenges faced by adolescents with long-lasting pain and those who support them. Understanding the management challenges experienced by adolescents with long-lasting pain, along with those faced by their parents, education professionals, and healthcare professionals (HCPs), is crucial for informing person-centered interventions and improving care and outcomes. Objectives This study aimed to gather insights and visions from adolescents with long-lasting pain, along with their parents, HCPs, and teachers, to develop a person-centered intervention for managing pain. Methods We used a qualitative Action Research approach, employing three workshops with 1) adolescents with long-lasting pain, 2) HCPs and teachers, and 3) parents. Workshops incorporated case vignettes and design-card exercises to foster dialogue, knowledge construction and articulation of insights and visions to inform intervention design. Data were collected through audio recordings, participant artifacts, and field notes, then analyzed using Reflexive Thematic Analysis and matrix synthesis to create a conceptual model highlighting tension points for future interventions. Results In three separate workshops, 13 adolescents with long-lasting pain, 16 HCPs and teachers (four physiotherapists, four senior high school teachers, three psychologists, three school health nurses, and two General Practitioners), and four parents participated. Adolescents described pain’s pervasive impact on their education, social lives, and self-identity. Barriers to improvement included limited coordination between healthcare and school, as well as a lack of communication. The school setting and school health services were identified as an ideal setting for interventions. Key visions for interventions included early holistic assessments, enhanced interdisciplinary collaboration with dedicated coordination roles, specialized adolescent-focused expertise, and the use of digital tools for personalized management. Conclusion This study brought new insights into the development of a person-centered intervention for adolescents with long-lasting pain, highlighting the impact of pain on those affected and barriers to optimal care. It emphasized the need for including education professionals and school health services in interdisciplinary collaboration, holistic assessments, and improved expertise in adolescent pain management.

  Background Point-of-care ultrasound (POCUS) has long been a diagnostic tool in family medicine, although most Norwegian general practitioners (GPs) who use POCUS, scans infrequently. The broad scope of family medicine, the relatively low prevalence of illnesses and infrequent use of POCUS imply that GPs may experience diagnostic uncertainty regularly. Aim To explore how GPs perceived and managed diagnostic uncertainty when using POCUS. Design and setting A qualitative focus group study among Norwegian GPs using POCUS. Methods Four focus group discussions were conducted. Total number of participants were 21. The interview guide was piloted, the focus group discussions were audio-recorded and transcribed, and Systematic Text Condensation, a thematic cross-case analysis, was used to analyse the data. Results Diagnostic uncertainty using POCUS was considered as aligning to the general clinical uncertainties in family medicine, but there were also POCUS-specific uncertainties in clinical decision-making. We generated six themes: emotional, cognitive, and ethical uncertainty using POCUS, communicating uncertainty to patients, interaction with specialists when using POCUS, and coping strategies of participants. POCUS results were the only results the participants sometimes withheld when communicating with other specialists. POCUS itself stimulated a renewed interest in family medicine. Scanning enough patients was the recommended coping strategy. Conclusion POCUS-using GPs experienced diagnostic uncertainty when using POCUS that aligned with other diagnostic uncertainties they experienced in everyday practice. However, they did not treat the results like other findings, as the GPs at times withheld their POCUS findings when interacting with secondary care specialists. This requires further investigation.

  Bakgrunn: Pasientenes legesøkningsatferd under covid-19-pandemien er lite studert. Formålet med studien var å undersøke norske pasienters erfaringer med fastlegenes tilgjengelighet og endringer i legesøkning og egenomsorg under pandemien. Materiale og metode: I denne tverrsnittsstudien delte medisinstudenter ut spørreskjemaer til pasienter på fastlegers venterom høsten 2021. Vi mottok 639 anonyme svar. Resultater: 78% av deltakerne opplevde tilgjengeligheten til fastlegekontoret som før, mens 74% oppgav egen terskel til å ta kontakt som uendret. 16% utsatte kontakt med helsetjenesten for sine plager, hvorav halvparten opplevde negative konsekvenser. Konklusjon: Selv om erfaringene fra covid-19-pandemien er at fastlegetjenesten i stor grad var omstillingsdyktig og leverte tilpassede tjenester, er det rom for å gi tydeligere informasjon om tilgjengelighet til fastlegetjenesten ved framtidige pandemier.

  Objective: To assess the cost-effectiveness of a single treatment session of acupuncture, when applied in addition to usual care for acute low back pain (ALBP). Methods: Secondary analysis of a multicentre randomised controlled trial in Norwegian general practice. In total, 171 participants with ALBP ⩽14 days were randomised to a control group (CG) receiving usual care or to an acupuncture group (AG) receiving one additional session of Western medical acupuncture alongside usual care. Primary outcome measures for this cost-effectiveness analysis were quality-adjusted life years (QALYs), health care costs and societal costs at days 28 and 365, the incremental cost-effectiveness ratio (ICER) and net monetary benefit (NMB). The NMB was calculated on the basis of the Norwegian cost-effectiveness threshold of NOK 275,000 (USD 35,628) per QALY gained. Missing data were replaced by multiple chained imputation. Results: Eighty-six participants in the CG and 81 in the AG were included in the analysis. We found no QALY gain at day 28. At day 365, the incremental QALY of 0.035 was statistically significant. The differences in health care costs and societal costs were not statistically significant. Three out of four calculations led to negative ICERs (cost saving) and positive NMBs. For the health care perspective at day 365, the ICER was USD –568 per QALY and the NMB was USD 1265, with 95.9% probability of acupuncture being cost-effective. Conclusion: To our knowledge, this is the first cost-effectiveness analysis of acupuncture for ALBP. The findings indicate that acupuncture may be cost-effective from a 1-year perspective, but more studies are needed. Trial registration number: NCT01439412 (ClinicalTrials.gov).

  Objective: To assess the use of point-of-care ultrasound (POCUS) in Norwegian general practice. Design: Retrospective register study based on general practitioners’ (GPs’) reimbursement claims. Setting: Norwegian general practice excluding out-of-hours clinics in 2009, 2012 and 2016. Subjects: GPs who scanned patients for a given set of symptoms and medical conditions. Main outcome measures: Number and characteristics of GPs performing POCUS. Number and type of scans carried out. Results: The number of scanning GPs increased from 479 in 2009 to 2078 in 2016. The number of registered scans increased from 8962 to 55921. In 2016, approximately 30% of Norwegian GPs sent at least one reimbursement claim for POCUS. Seven out of 10 GPs did not scan every month. The gender distribution of scanning GPs was equal to that of the total GP population. Male GPs scanned four times more frequent than female GPs. Specialist in family medicine scanned twice as much as non-specialist. The use of POCUS among GPs in different counties varied from 31.6 to 198.5 per 10,000 citizens. Conclusions: The number of Norwegian GPs using POCUS and the number of scans have increased substantially from 2009 to 2016. The use of the various scans, based on the use of reimbursement claims, have evolved differently. The reasons for this are not known. The low number of scans carried out by most GPs raises a concern when it comes to the quality of the performed scans.

  Objectives: The aim of this study was to evaluate whether a single treatment session of acupuncture, when applied in addition to standard treatment for acute low back pain (ALBP), reduces the time to recovery compared with standard treatment alone. Design: A multicentre, randomised, controlled trial. Setting: Conducted at 11 Norwegian general practitioners’ (GPs’) offices. Participants: 171 adults aged 20–55 years seeking their GP for ALBP (≤14 days) between March 2014 and March 2017. Patients with secondary back pain and previous sick leave and acupuncture treatment was excluded. Interventions: The participants were randomised to either the control group (CG) or the acupuncture group (AG) by online software. The CG received standard treatment according to the Norwegian guidelines, while the AG received one session of Western medical acupuncture treatment in addition to standard treatment. The statistician was blinded to group status. Primary and secondary outcome measures: The primary outcome was median days to recovery. Secondary outcomes were pain intensity, global improvement, back- specific functional status, sick leave, medication and adverse effects. Results: 185 participants were randomised, 95 in the CG and 90 in the AG. 14 participants did not receive the allocated intervention and 4 were excluded from the analysis. Thus, 167 participants were included in the analysis, 86 in the CG and 81 in the AG. The groups were similar according to baseline characteristics. The median time to recovery was 14 days for the CG and 9 days for the AG, HR 1.37 (95% CI 0.95 to 1.96), (p=0.089). No serious adverse effects were reported. Conclusions: We did not find any statistically significant reduction in time- to- recovery after a single session of acupuncture for ALBP compared with standard care.

  Objective: Needle acupuncture in small children has gained some acceptance in Western medicine. It is controversial, as infants and toddlers are unable to consent to treatment. We aimed to assess its efficacy for treating infantile colic. Design: A systematic review and a blinding-test validation based on individual patient data from randomised controlled trials. Primary end-points were crying time at mid-treatment, at the end of treatment and at a 1-month follow-up. A 30-min mean difference (MD) in crying time between acupuncture and control was predefined as a clinically important difference. Pearson’s chi-squared test and the James and Bang indices were used to test the success of blinding of the outcome assessors [parents]. Eligibility criteria and data sources: We included randomised controlled trials of acupuncture treatments of infantile colic. Systematic searches were conducted in Cochrane CENTRAL, MEDLINE, EMBASE, CINAHL and AMED, and in the Chinese language databases CNKI, VIP, Wang fang, SinoMed and Chinese Clinical Trial Registry. Results: We included three randomised controlled trials with data from 307 participants. Only one of the included trials obtained a successful blinding of the outcome assessors in both the acupuncture and control groups. The MD in crying time between acupuncture intervention and no acupuncture control was −24.9 min [95% confidence interval, CI −46.2 to −3.6; three trials] at mid-treatment, −11.4 min [95% CI −31.8 to 9.0; three trials] at the end of treatment and −11.8 min [95% CI −62.9 to 39.2; one trial] at the 4-week follow-up. The corresponding standardised mean differences [SMDs] were −0.23 [95% CI −0.42 to −0.06], −0.10 [95% CI −0.29 to 0.08] and −0.09 [95% CI −0.48 to 0.30]. The heterogeneity was negligible in all analyses. The statistically significant result at mid-treatment was lost when excluding the apparently unblinded study in a sensitivity analysis: MD −13.8 min [95%CI −37.5 to 9.9] and SMD −0.13 [95%CI −0.35 to 0.09]. The registration of crying during treatment suggested more crying during acupuncture [odds ratio 7.7; 95% CI 2.7–20.6; one trial]. GRADE-Moderate quality evidence. Conclusions: Percutaneous needle acupuncture treatments should not be recommended for infantile colic on a general basis.

  Background: Electronic questionnaires can ease data collection in randomized controlled trials (RCTs) in clinical practice. We found no existing software that could automate the sending of emails to participants enrolled into an RCT at different study participant inclusion time points. Objective: Our aim was to develop suitable software to facilitate data collection in an ongoing multicenter RCT of low back pain (the Acuback study). For the Acuback study, we determined that we would need to send a total of 5130 emails to 270 patients recruited at different centers and at 19 different time points. Methods: The first version of the software was tested in a pilot study in November 2013 but was unable to deliver multiuser or Web-based access. We resolved these shortcomings in the next version, which we tested on the Web in February 2014. Our new version was able to schedule and send the required emails in the full-scale Acuback trial that started in March 2014. The system architecture evolved through an iterative, inductive process between the project study leader and the software programmer. The program was tested and updated when errors occurred. To evaluate the development of the software, we used a logbook, a research assistant dialogue, and Acuback trial participant queries. Results: We have developed a Web-based app, Survey Email Scheduling and Monitoring in eRCTs (SESAMe), that monitors responses in electronic surveys and sends reminders by emails or text messages (short message service, SMS) to participants. The overall response rate for the 19 surveys in the Acuback study increased from 76.4% (655/857) before we introduced reminders to 93.11% (1149/1234) after the new function (P<.001). Further development will aim at securing encryption and data storage. Conclusions: The SESAMe software facilitates consecutive patient data collection in RCTs and can be used to increase response rates and quality of research, both in general practice and in other clinical trial settings.

  Objective. Infantile colic is a painful condition in the first months of infancy. Acupuncture is used in Scandinavia as a treatment for infantile colic. A randomized controlled trial was carried out with the aim of testing the hypothesis that acupuncture treatment has a clinically relevant effect for this condition. Design. A prospective, blinding-validated, randomized controlled multicentre trial in general practice. Research assistants and parents were blinded. Setting. 13 GPs’ offices in Southern Norway. Intervention. Three days of bilateral needling of the acupuncture point ST36, with no treatment as control. Subjects. 113 patients were recruited; 23 patients were excluded, and 90 randomized; 79 diaries and 84 interviews were analysed. Main outcome measures. Difference in changes in crying time during the trial period between the intervention and control group. Results. The blinding validation questions showed a random distribution with p = 0.41 and 0.60, indicating true blinding. We found no statistically significant difference in crying time reduction between acupuncture and control group at any of the measured intervals, nor in the main analysis of differences in changes over time (p = 0.26). There was a tendency in favour of the acupuncture group, with a non-significant total baseline-corrected mean of 13 minutes (95% CI –24 to + 51) difference in crying time between the groups. This was not considered clinically relevant, according to protocol. Conclusion. This trial of acupuncture treatment for infantile colic showed no statistically significant or clinically relevant effect. With the current evidence, the authors suggest that acupuncture for infantile colic should be restricted to clinical trials.

  Introduction Some general practitioners (GPs) treat acute low back pain (LBP) with acupuncture, despite lacking evidence of its effectiveness for this condition. The aim of this study was to evaluate whether a single treatment session with acupuncture can reduce time to recovery when applied in addition to standard LBP treatment according to the Norwegian national guidelines. Analyses of prognostic factors for recovery and cost-effectiveness will also be carried out. Methods and analysis In this randomised, controlled multicentre study in general practice in Southern Norway, 270 patients will be allocated into one of two treatment groups, using a web-based application based on block randomisation. Outcome assessor will be blinded for group allocation of the patients. The control group will receive standard treatment, while the intervention group will receive standard treatment plus acupuncture treatment. There will be different GPs treating the two groups, and both groups will just have one consultation. Adults who consult their GP because of acute LBP will be included. Patients with nerve root affection, ‘red flags’, pregnancy, previous sick leave more than 14 days and disability pension will be excluded. The primary outcome of the study is the median time to recovery (in days). The secondary outcomes are rated global improvement, back-specific functional status, sick leave, medication, GP visits and side effects. A pilot study will be conducted. Ethics and dissemination Participation is based on informed written consent. The authors will apply for an ethical approval from the Regional Committee for Medical and Health Research Ethics when the study protocol is published. Results from this study, positive or negative, will be disseminated in scientific medical journals.

  Vi vet nok til å kunne hjelpe langt flere pasienter med langvarige symptomer. Likevel holder vi tilbake håpet, av frykt for å skuffe. Det kan koste pasientene store deler av livet. Denne kronikken er et faglig opprop fra 63 klinikere, forskere og fagpersoner fra hele landet. Vi har klinisk og forskningsmessig erfaring med vedvarende fysiske symptomer, som smerter, utmattelse, «hjernetåke», svimmelhet og mageplager og har samlet oss om et felles budskap: Ny kunnskap om hvordan slike symptomer oppstår og opprettholdes gir grunn til langt større håp om bedring enn mange pasienter får formidlet i dag.

  Bakgrunn og formål: Langvarige smerter hos ungdom er et utbredt og komplekst helseproblem, ofte forbundet med andre helseplager og skolefravær. Det er fortsatt begrenset kunnskap om persontilpassede tiltak for denne aldersgruppen, og tidligere tiltak har i liten grad involvert sluttbrukere i utviklingen av relevante og målrettede intervensjoner. Smertehåndtering påvirkes av en rekke faktorer, inkludert skolesituasjon, hjem- og fritidsforhold. Innsikt fra helsepersonell, lærere og foreldre er derfor viktig for en helhetlig tilnærming i utviklingen av intervensjoner. HEYoung-intervensjonsstudien har som mål å samskape en persontilpasset intervensjon for ungdom med langvarige smerter, som senere skal evalueres i en pilot-randomisert kontrollert studie på videregående skoler i Norge. Formålet med første steg i prosjektet var å identifisere utfordringer og barrierer i hvordan ungdommer, helsepersonell, lærere og foreldre opplever dagens praksis for smertehåndtering, samt å utforske visjoner for en persontilpasset intervensjon gjennom en brukerdrevet tilnærming. Metode: I februar 2024 gjennomførte vi det første steget i HEYoung-intervensjonsstudien ved å arrangere workshops med tre grupper: 1) ungdommer med smerter (n=13), 2) helsepersonell og lærere (n=16) og 3) foreldre (n=4). Workshopene var delt inn i tre faser — kritikk, fantasi og visjon — for å støtte deltakerne i refleksjoner rundt smertehåndtering, og i å utvikle konkrete tiltak til en persontilpasset intervensjon. Vi brukte casevignetter og inspirasjonskort for å stimulere diskusjonene, som ble tatt opp med lydopptak, transkribert og analysert ved hjelp av refleksiv tematisk analyse (Braun og Clarke). Resultater: Ungdom beskrev hvordan smertene førte til fravær, redusert konsentrasjon, behov for tilrettelegging og begrenset sosialt liv, samtidig som smertene utfordret deres selvbilde og identitet. Foreldre, helsepersonell og lærere fremhevet at smertene medførte komplekse utfordringer og krevde koordinering og tilpasning mellom skole og helsevesen. Barrierer inkluderte manglende koordinering mellom helsevesen og skole om fravær og undervisningstilpasning, samt utilstrekkelig kommunikasjon om symptomer, behandlingsplaner og tilretteleggingsbehov. Systemiske faktorer, som ventetider for psykologisk støtte og krav om medisinsk diagnose for tilrettelegging, forsterket utfordringene. Skolen ble identifisert som en sentral arena for en intervensjon. Nøkkelvisjoner inkluderte tidlige helhetlige vurderinger, tverrfaglig samarbeid med dedikerte koordinatorer, tilgang til helsepersonell med spesialisert ungdomskompetanse og digitale verktøy for tilpasset oppfølging. Konklusjon: Studien avdekket nye innsikter i smertehåndtering for ungdom, inkludert smertens påvirkning på de berørte og barrierer for håndtering og oppfølging. Visjonene understreket viktigheten av bedre tverrfaglig samarbeid, grundigere undersøkelser og forbedret ekspertise i smertehåndtering for ungdom.

  The status of primary care education in the Nordic countries is high. Could we strive for even better? New data is emerging in a European survey from 32 countries by the Basic Medical Education Committee of EURACT. 1. The first presentation offers an overview of primary care undergraduate education in Europe in 2023, reflecting the requirements and reality in universities. Especially the northern data is extracted for comparison. 2. Numerous European examples will create hope. Emphasis is placed on Nordic innovations. 3. Ideas about resources and provision of primary care education will be debated in a panel discussion. Is the GP specialist training and continuous professional development in line with undergraduate education, to strengthen the core values as a bedrock? Interactive methods will be used to activate participants during the symposium.

  Background: Understanding how to navigate medical guidelines is an essential skill for future doctors. Recent emphasis has been placed on the importance of prioritization and time management when implementing guidelines into clinical practice. However, traditional medical education is offered topic by topic, with less emphasis given to patient-centred care, grading of recommendations and sometimes conflicting guidelines. Methods: Our objective was to develop a two-week elective subject titled "The Medical Profession: Navigating Medical Guidelines" for third-year medical students in Oslo. Through an intensive application of the nominal group technique (NGT), a group of five experienced primary care physicians and teachers collaborated during a two-day session to design the subject and its curriculum. Initially, we had concerns about the complexity of this topic for students at the third-year level due to their limited clinical experience. To assess the subject, the examination was designed to include an individual reflection note and a group presentation. Out of 16 students, 14 consented to having their reflection notes used for educational research purposes. Results: The NGT process led to development of a subject with interactive lectures, group and plenary discussions, as well as a clinical rotation day. Key topics included research guidelines, sensible choices and sustainability, patient-centered care, medical uncertainty, multi-morbidity, and navigating conflicting guidelines. A longitudinal patient-case video presentation accompanied the teaching with increasing complexity each day. Students were divided into two groups for the clinical rotation day at out-of-hour emergency care and nursing home facilities. Two teachers were present each day of the course to facilitate skill development and discussions. Students provided positive feedback, appreciating the reflection on their future professional roles. Conclusion(s): The NGT process proved effective in creating an elective subject well-received by both teachers and students. We encourage others to consider using similar models when designing new curricula.

  Background Practicing the role of a doctor with real patients is essential to integrate theoretical knowledge and practical skills. With an increasing number of medical students and longer durations of clinical placements for each student, more patients will encounter a student in the doctor's chair in the future. How do they experience it? Previous studies have focused on students and supervisors in clinical practice. Fewer have explored the patient's perspective. Patients may perceive that students have more time and are more thorough in their examinations, and they feel that they contribute to the students' learning by sharing their own stories and experiences. However, they may be hesitant with regards to mental illnesses or intimate examinations. Research question In this study we aim to explore patients’ attitudes and experiences meeting medical students at the General practitioners (GP’s) office. Methods Students in the later stages of their studies in Oslo and Bergen invite patients to respond to an anonymous questionnaire about their attitudes towards meeting students at their GP's office. Patients are recruited in the waiting rooms of the GP practices, where the students are in placements, and they respond digitally on their own smartphones or an available tablet. Responses are stratified based on whether respondents have had a personal experience of a student encounter or not. Those who have recently had a student consultation will also be asked detailed questions about their experience. Results The survey has been piloted at two doctor's offices in Bergen and data collection will be performed throughout 2023. Preliminary results will be presented at the conference. Conclusion To facilitate best possible clinical learning, we need to learn more about how patients experience meeting students. Through this study, we aim to facilitate good learning experiences while ensuring that the patients are well taken care of.

  Regarding the recently published review ”Looking for new treatments of Infantile Colic“ by Savino et al. we want to add that positive effects of acupuncture have been demonstrated to release pain and agitation and that acupuncture seems to be a safe treatment when performed by trained acupuncturists. Inconclusive results in the few published articles on the subject can be due to different acupuncture points, different insertion time, different needling methods, differences in the outcome variables, in how the crying was measured and insufficient sample sizes. Further research is needed on understanding the utility, safety, and effectiveness of acupuncture in infants with colic.
