Connecting ORAL- and home health care services (CORAL)

Oral health problems for homebound patients have been neglected in health care services. This research project aim to include oral health in the integrated care perspectives for these patients.

One of the greatest challenges faced by our society is an ever-increasing number of homebound elderly patients suffering from multi-morbidity.

Oral health problems for homebound patients have been neglected in health care services due to knowledge gaps, disintegrated and fragmented healthcare services, blurred responsibility between different levels of care.

It is urgent to include oral health in the integrated care perspective for these patients to avoid unnecessary health deterioration and increased care dependency and utilisation of health care services.

About the project

In CORAL, we will apply methods and framework that have not been applied previously within the oral health- and home care service research. We will provide knowledge beyond the state of the art regarding the validity, applicability and methodological approaches within the field and across services and boundaries.

We aim to expand our understanding of the underlying mechanisms of collaborative practices, facilitators, barriers and opportunities by applying innovative approaches to improve oral health services for homebound patients.

Stakeholders from different disciplines and scientific collaborators will together design solutions for connecting home health care and oral health care.

Objectives

In CORAL, we will build on a multilevel approach on integrated care framework for complex intervention as the main theoretical and methodological approaches for organising our project. The overarching goal is to provide a scientific-based generic integrated care model for connecting home health care and oral health services.Throughout three work packages the objective is to:

Design a scientific-based intervention for connecting oral health and home care services
Test the model for connecting public dental service with home health care services
Provide guidelines for best practice and innovative digital decision support that enable patient-centred health care across service levels.

Outcomes

We will evaluate (1) the service utilization as a measure of accessibility, (2) the number of acute referrals and (3) health-related quality of life. Health economic evaluation will include a cost-utility. Cost will be estimated from the societal perspective, including both medical and non-medical costs.

Background

Disconnected services and uncoordinated inter-professional collaboration are two fundamental barriers for establishing integrated oral care for elderly receiving home health care. Today, collaborative communication channels between home care and public dental services include telephones, letters, emails, and meetings. This type of communication is time-consuming, vulnerable and associated with reduced efficiency.

Oral diseases are the most common of the chronic diseases for elderly people and represent an important public health problem due to prevalence and subsequent impact on individuals and society. Oral health problems lead to severe pain, have negative effects on nutrition and quality of life. Due to oral diseases, an increased bacterial systemic exposure and increased levels of inflammatory factors, may lead to adverse health outcomes such as uncontrolled diabetes, cardiovascular disease, respiratory disease and premature death. In addition to the somatic consequences of oral disease, psychosocial aspects such as a reduced ability to speak and interact socially often influence the quality of life of these patients.

Previous research on integrate care has addressed collaboration between hospital (governmental level) and municipalities. In oral health services, we are faced with another structural obstacle because public dental servoces is organized at the county level. Collaboration between the healthcare providers are hampered by layers of organizational complexity and administrative levels, as well as by a lack of collaboration and guidelines. We lack in-depth knowledge as well as scientific-based interventions about how to improve the oral health service on individual and organizational level.

Objective: To map dental health service utilization (DSU) and associated factors among older adults receiving home care services (HCS) in south-eastern Norway, using Andersen’s model. Materials and methods: In this cross-sectional study of older adults receiving HCS aged ≥ 65 years, data on oral health practices, general health, and predisposing, enabling, and need-based factors were collected via interviewer-administered questionnaire. Results: Of 116 participants (mean age 83, 53.6% female), 67.2% reported annual DSU, while 24.1% used dental health services only for acute problems. Although 95.6% were entitled to free dental care in the public dental service (PDS), 66 were aware of this, and 56.5% made use of their rights. Awareness of entitlements was associated with more frequent PDS use. A higher self-reported number of teeth and using private dental services were also linked to higher DSU. Conclusions: The present study showed underutilization of dental health services among older adults receiving HCS and unawareness of entitlement of their rights in PDS. However, higher self-reported number of teeth and use of private dental services were linked to more frequent DSU. To address underuse, the PDS should improve awareness of entitlements, enhance information delivery, and further investigate reasons for non-utilization.

Objective: To map self-reported oral health status, oral health-related quality of life (OHRQoL), and associated factors among older adults receiving home care services (HCS) in south-eastern Norway. Material and methods: For this cross-sectional study, older adult users of HCS, aged ≥ 65 years were interviewed using an interviewer-administered questionnaire. Information about demographics, number of medicines, and Activity of Daily Living (ADL-score) was obtained. Self-reported oral health status and subjective dry mouth (Summated Xerostomia Inventory-Duch version, SXI-D) were assessed. OHRQoL was measured using Oral Health Impact Profile-14 (OHIP-14). Results: Of 116 participants (mean age 83 years), 52.6% were female. Half of the participants reported missing < 5 of their natural teeth and 6.9% were edentulous. Pain and discomfort were reported by 16.4%, and subjective dry mouth was common (40.7%). Poor oral health status was reported by 8.6%, and experiencing problems or discomfort fairly often or very often was reported by 20.7%. Good self-perceived oral health was reported by 61%. Younger (< 75 years) and less dependent (ADL < 2) individuals and those missing ≥ 5 natural teeth reported a negative impact on OHRQoL more often. Conclusions: Half of older adults receiving HCS in Norway retain most of their natural teeth, few are edentulous and xerostomia is common. Younger and less dependent individuals and those missing ≥ 5 natural teeth reported a negative impact on OHRQoL more often.

Background An increasing number of older adults receive home healthcare in their homes, some of whom lose their capacity and ability to take care of their own health, including oral health. Poor oral health may have serious consequences that can lead to poor nutrition, diseases, isolation, and even death. There is an urgent need to involve users in developing targeted interventions to ensure that eligible older adults exercise their right to receive appropriate dental care. The purpose of the study was to explore what user groups emphasised in co-creating a targeted dental care intervention for older adults receiving home healthcare. Methods In this study, an exploratory design with an inductive and interpretive approach was employed. In two six-hour workshops, eleven participants representing three older adults, one next of kin, two nurses, and five dental care providers exchanged and discussed their perspectives on dental care as part of a co-creation process. Data were collected from group and plenary discussions during the workshops and were analysed using reflexive thematic analysis. Results The different user groups conveyed confusing and contradictory understandings and interpretations of the dental healthcare landscape. This uncertainty resulted in missed information regarding dental care, leading to situations where they do not always seek dental healthcare services until an acute issue arises. A blurred perception of older adults’ needs for support, combined with shortcomings in collaboration, communication, and the exchange of digital information systems, has led to a lack of knowledge about one another and inadequate dental healthcare for this vulnerable group. The various user groups suggested different solutions; however, several key issues emerged as urgent to address. There is a need to build and integrate systems into home healthcare’s digital administrative and health record systems to support the clarification and identification of eligible patients, as well as to develop a digital coordination and communication system to connect the two levels of service. Conclusions Unpacking the perspectives of the different user groups uncovered that an intervention must take into account the views of all groups. Understanding and incorporating the perceptions of all the different user groups were crucial for developing an intervention that enables a preventive and collaborative dental care approach.

Background There is little knowledge about home care services personnel competencies and beliefs concerning oral healthcare for home-dwelling, dependent older adults. This study aims to map oral healthcare beliefs among home care services personnel, and have the following question: How do the education level, years of work experience and training in oral health have impact on home care services personnel oral healthcare beliefs? Methods A cross-sectional study among home care services personnel working with older people receiving home care services was conducted across four municipalities in south-eastern Norway. The questionnaire consisted of background information (gender, education level, years of work experience, training in oral healthcare, employment status) and the nursing Dental Coping Beliefs scale. Ethics approval for this study was obtained from the Norwegian Centre for Research Data. Results Two hundred and sixty-two homecare services personnel responded to the questionnaire, 16.5% males and 83.5% females; 40.5% had had training in oral healthcare. Home care services personnel believed that gum diseases and cavities can be prevented by dental flossing (61.4%) and toothbrushing (98.4%). 59% disagreed that preventing sickness and medicines from destroying teeth is impossible. However, the majority of the home care services personnel were uncertain about how oral mucosal disorders can be treated. Having more than three years of higher education was positively associated with being in higher quartiles of oral healthcare beliefs, and external locus of control, and having training in oral healthcare was positively associated with being in the lower quartiles of internal locus of control and self-efficacy dimensions. Males were more likely to be in the lower quartile of oral healthcare beliefs, which wasn’t positive. Conclusion In the population studied, the home care services personnel beliefs about oral healthcare improved with an increasing level of education and having had training in oral healthcare. This suggests that home care services personnel need more education and training in oral healthcare.

Background Providing dental services to dependent older adults might be challenging because of physical and cognitive decline. The present study aimed to explore current practices, knowledge, and experienced challenges related to the treatment of older adults in home health care services (HHCS) among dentists and dental hygienists in Norway. Methods An electronic questionnaire survey was distributed to Norwegian dentists and dental hygienists, inquiring about background characteristics, current practices, self-perceived knowledge, and challenges when providing oral health care for older HHCS patients. Results Four hundred and sixty-six dentists and 244 dental hygienists treating older HHCS patients responded to the survey. The majority were female (n=620; 87.3%) and worked in the public dental service (PDS) (n=639; 90%). When older HHCS adults attended the dental practice, the treatments provided were most frequently aimed at relieving acute oral problems, although dental hygienists reported to focus on improving oral health more often than dentists. Dentists reported to have more self-perceived knowledge than dental hygienists regarding patients with complex treatment needs, cognitive or physical impairment. Exploratory Factor Analysis (EFA) was carried out on the 16 items related to challenges, three factors were extracted and Structural Equation Models (SEMs) were performed. Challenges related to dental care for older HHCS adults were related to time, practical organization and communication. Variation within these categories was associated with sex, graduation year and country, as well as time used per patient and work sector, but not with professional status. Conclusions The results indicate that dental care for older HHCS patients is time-demanding and more often aimed at relieving symptoms than improving oral health. A substantial proportion of dentists and dental hygienists in Norway lack confidence when providing dental care for frail elderly.

Aim To examine factors that affect the performance of oral health care (OHC) for older people receiving nursing care at home. Background Oral health is often neglected by health care providers caring for older people. Research shows that health care providers' provision of OHC may be influenced by various factors (barriers and facilitators). When this research was conducted, health care providers from home healthcare services (HHCS) and nursing homes were grouped together despite setting differences; therefore, this study focuses on the performance of OHC by home health care providers (HHCPs) as a single group. Design Explorative design with a qualitative approach. Methods The managers of four HHCS units recruited 17 HHCPs to participate in focus group interviews. One interview was conducted per unit, and there were four to five participants in each interview. The analysis of interviews was based on theoretical thematic analysis and the PRECEDE constructs in the PRECEDE–PROCEED model. Consolidated Criteria for Reporting Qualitative Research (COREQ) guidelines were used in reporting this study. Findings The analysis resulted in two themes with predisposing factors (HHCPs' professional responsibilities, older people's attitude), five themes with enabling factors (knowledge and skills, older people/carer trust, available time, available equipment and collaboration with public dental service (PDS)), and two themes with reinforcing factors (routines and OHC focus on the workplace) that affect the provision of OHC. The factors were categorised as individual, organisational and collaboration factors. Conclusions In addition to individual factors found in previous studies, factors related to the organisation of services and communication between HHCPs and PDS seem to affect HHCPs' provision of OHC for adults receiving HHCS. Implications for Practice This study provides in-depth knowledge that can contribute to increasing HHCPs' provision of OHC and thereby prevent oral and dental disease among older people receiving HHCS.

Objectives: Through a scoping review, we examine in this survey what ways health equity has been promoted in clinical research informatics with patient implications and especially published in the year of 2021 (and some in 2022). Method: A scoping review was conducted guided by using methods described in the Joanna Briggs Institute Manual. The review process consisted of five stages: 1) development of aim and research question, 2) literature search, 3) literature screening and selection, 4) data extraction, and 5) accumulate and report results. Results: From the 478 identified papers in 2021 on the topic of clinical research informatics with focus on health equity as a patient implication, 8 papers met our inclusion criteria. All included papers focused on artificial intelligence (AI) technology. The papers addressed health equity in clinical research informatics either through the exposure of inequity in AI-based solutions or using AI as a tool for promoting health equity in the delivery of healthcare services. While algorithmic bias poses a risk to health equity within AI-based solutions, AI has also uncovered inequity in traditional treatment and demonstrated effective complements and alternatives that promotes health equity. Conclusions: Clinical research informatics with implications for patients still face challenges of ethical nature and clinical value. However, used prudently—for the right purpose in the right context—clinical research informatics could bring powerful tools in advancing health equity in patient care.

Objective To map the literature on subjective and objective oral health indicators and oral health-related quality of life (OHRQoL) in older adults receiving home health care services (HHCS). Background The proportion of older adults in need of HHCS will increase in the coming years. Previous studies indicate that frail and dependent older adults are at increased risk for oral diseases, due to challenges with daily oral hygiene and regular access to dental services. Materials and methods Four databases were searched in November 2020 for relevant literature. Search terms included a comprehensive list of terms for adults 65 years or older receiving HHCS, clinical and subjective oral health indicators, and OHRQoL. The literature was reviewed based on inclusion and exclusion criteria. Results Of the 3114 sources identified, 18 were included. Data on oral diseases and symptoms among older adults receiving HHCS were limited and heterogeneous. Overall, older adults often lacked some of their natural teeth and often had removable dentures that needed repair. In addition, plaque, caries, xerostomia, and chewing and swallowing problems were common among the population group. Data on OHRQoL were scarce and indicated a positive association with a higher number of present teeth, while decayed teeth, root remnants, and dry mouth had substantial negative impacts on the daily activities of older adults receiving HHCS. Conclusion This scoping review show that older adults above 65 years receiving HHCS generally have poor oral health status and that there is a knowledge gap regarding their OHRQoL.

Background: Older adults (≥65 years) receiving municipal home health care services (HHCS) in Norway face substantial oral‑health challenges. One important reason is that HHCS and public dental services (PDS), organized at the county level, operate in separate organizational structures and use different digital systems. Currently, no integrated digital communication solution exists to support collaboration, and there is a clear need for improved cross‑sector coordination. Recent cost‑of‑illness evidence shows that poor oral health in this population entails considerable economic burden, including direct costs related to oral disease, indirect costs from increased risk of systemic illness, and productivity losses among caregivers. SYNCORAL is a digital platform embedded in electronic patient records designed to streamline patient identification, consent, and information exchange between HHCS and PDS. By enabling better coordination and earlier intervention, SYNCORAL may help shift care from costly acute treatment toward prevention. However, the key effects and costs of SYNCORAL remain uncertain. A Value of Information (VOI) analysis is therefore needed to assess whether additional evidence is required before large‑scale implementation. Aim: The aim of this VOI analysis is to quantify remaining decision‑critical uncertainty in the expected effects and costs of SYNCORAL, and to assess whether collecting additional evidence would be worthwhile. Method: We developed a Markov model (see Figure 2) and performed a probabilistic sensitivity analysis (PSA) using Monte Carlo simulation with 10 000 iterations to quantify parameter uncertainty. The model applied a lifetime time horizon, following older adults receiving HHCS from the age of 65 years old to death. Two separate analyses were conducted for adults aged 65–79 years and 80+ years. Data were obtained from epidemiological studies, national health registries, reimbursement databases, and expert assessments. Preliminary results: Substantial uncertainty across several key parameters is already evident while the VOI analysis remains in progress. Figure 3 shows the preliminary incremental cost‑effectiveness scatterplot of SYNCORAL versus current practice. Each point is one PSA simulation; the dashed line indicates a 275 000 NOK/QALY threshold, with points below the line considered cost‑effective. In these preliminary results, SYNCORAL appears cost‑effective in around 60% of simulations at this threshold. Further research: Our planned VOI work will estimate Expected Value of Sampling Information (EVSI) to quantify the value of reducing the uncertainty through new data, and Expected Net Benefit of Sampling (ENBS) to assess whether additional evidence would justify the cost of further research

Introduction: Older adults (65+) receiving home health care services (HHCS) represent about 2% of Norway’s population. By 2050, the population aged 80+ is projected to double, which is expected to increase the demand for HHCS. Old age is associated with a high prevalence of oral diseases and poor oral health, which in turn is associated with elevated risks of systemic conditions. Despite these implications, the societal economic cost of oral health among older adults receiving HHCS is insufficiently documented. This study aimed to estimate current and future societal costs of reduced oral health among older adults receiving HHCS in Norway. Method: A cost-of-illness analysis was conducted. Direct costs included routine examinations and dental treatment, while indirect costs covered caregiver productivity losses and systemic diseases associated with reduced oral health. Data were obtained from epidemiological studies, national health registers, reimbursement registries, and expert elicitation. Results/conclusion: Older adults receiving HHCS accounted for 4.8% of dental health expenditures (NOK 1.1 billion) in 2026. In 2026, adults aged 80 years and more made up about 60% of total costs (NOK 919 million), compared with 40% for those aged 65–79 years (NOK 619 million). By 2060, this distribution is projected to shift to roughly 70% (NOK 2.3 billion) versus 30% (NOK 1 billion) Poor oral health among older adults receiving HHCS constitutes a substantial economic burden.
