Abstract Background Chronic pain is a global public health issue, severely affecting quality of life, functional capacity, and healthcare resources. Effective management requires adequate health literacy, yet limited health literacy is associated with greater pain burden and poorer outcomes. This underlines the need for interventions that can strengthen patients’ health literacy skills. Thus, the aim of the present study was to investigate longitudinal changes in health literacy following participation in a multidisciplinary primary care pain management intervention. Methods This longitudinal study reports health literacy outcomes from a multidisciplinary, personalized pain management intervention that was developed in accordance with the Medical Research Council framework. Forty-three participants with chronic non-cancer pain completed the full intervention. Health literacy was measured using the Norwegian version of the Health Literacy Questionnaire (HLQ) at baseline, midpoint, and post-intervention. Changes over time were analyzed using paired-sample t-tests. Results Statistically significant improvements were observed in five HLQ domains from baseline to midpoint and in eight domains from baseline to post-intervention. “Having sufficient information to manage my health” showed the largest increase among Domains 1–5, whereas “Navigating the healthcare system” showed the largest increase among Domains 6–9 and improved significantly across all time-point comparisons. Conclusion Participants demonstrated improvements in several dimensions of health literacy during participation in the intervention, particularly in domains related to having sufficient information to manage health and active self-management. These findings support further investigation of tailored, multidisciplinary approaches to strengthening health literacy among people with chronic pain. Trial registration NCT05828121 Date: 04-25-2023.

ABSTRACT Background Suicide risk is elevated after prison release, but population‐level evidence on temporal risk patterns and associated factors remains limited. We aimed to describe characteristics of individuals who die by suicide after release from Norwegian prisons, compare their suicide mortality with the general population, and examine risk factors over time. Material and Methods Data from the Norwegian Prison Release Study (nPRIS), including all people in prison in Norway (2000–2022), were linked to the Norwegian Cause of Death Registry, the Norwegian Prison Registry, the Norwegian Patient Registry, and Statistics Norway. We calculated crude mortality rates (CMR) per 100,000 person‐years, standardized mortality ratios (SMR), annual age‐standardized years of life lost rate (ASYR), and Cox proportional hazards models. Results From 2000 to 2022, 1183 suicides (9.1% of all deaths; CMR 81; 95% CI 76–85) occurred after release (90% male). The SMR was higher in women (SMR 8.64; 95% CI 7.17–10.34) than men (SMR 3.98; 95% CI 3.76–4.22). In multivariable models (2009–2022; n = 57,034; 516 suicides), strong independent associations with suicide 6 months post‐release included self‐harm (adjusted HR [aHR] 4.76; 95% CI 2.60–8.72), severe mental illness (aHR 1.95; 95% CI 1.16–3.27), release from high‐security prisons (aHR 1.93; 95% CI 1.12–3.33), and pretrial detention (aHR 1.82; 95% CI 1.06–3.13). Conclusions Suicide risk is substantially and persistently elevated relative to the general population, peaks immediately after release, and is concentrated among people with prior self‐harm, severe mental illness, and those released from high‐security settings.

Background Preventing episodes involving aggression and reducing coercive measures are important goals in in-patient mental health settings. Antecedents may provide insight into how episodes are described to develop. However, most studies reflect staff perspectives, and often describe and temporally specify antecedents inconsistently. Aims This scoping review mapped reported antecedents to episodes involving aggression and episodes involving coercive measures, and identified which appeared shared or specific to either phenomenon. Method This review followed the Joanna Briggs Institute methodology and Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews guidance, and included 39 studies from systematic searches across Ovid Medline, APA PsycINFO and Scopus. Reported antecedents were extracted, standardised and mapped to six analytical domains. Results The review mapped 240 antecedents, many of which appeared shared across the phenomena. Patient studies emphasised experiences of restrictive care, loss of autonomy and poor interaction quality. Staff studies emphasised immediate, observable situations related to risk management, such as patient aggression, denied requests and interpersonal conflict. Most studies did not specify the temporal boundaries of antecedents. Conclusions The perspectives represented in studies and the ways antecedents are conceptualised influence which antecedents become visible in the literature. Overlapping antecedents indicate that episodes involving aggression and episodes involving coercive measures are often described as part of the same escalation process, although patient and staff studies seem to highlight different stages. Prevention efforts therefore require attention to the entire trajectory, and to the point at which staff perceive intervention as necessary. Future studies should incorporate both perspectives, prioritise patient involvement, and more clearly define and temporally specify antecedents.

Abstract Background Delirium is common and age, frailty and dementia are risk-factors. The pathophysiology is complex and poorly understood, but age-related immune system changes may be key. This study aimed to measure markers of immune cell migration, accelerated immune ageing and blood–brain barrier permeability in community-dwelling, older people comparing those who developed ≥1 episode of delirium over 2 years with those that did not. Methods Baseline samples were collected from CASCADE participants who were followed up for 2 years. Delirium screening was performed weekly; positive screens or hospital admission triggered a delirium assessment. Serum was isolated, stored and analysed using a multiplex panel comparing levels between participants with and without delirium episodes. Results About 98 participants were included. Median age was 88 (IQR: 81.8–92.0), with 90.8% classified as frail. Forty-three participants (43.9%) experienced ≥1 delirium episodes. CCL2 (MCP-1) and CXCL2 (MIP-2α), measured in a stable state at baseline, were significantly higher in the delirium group compared to the no delirium group (13.38 (11.21–17.93) vs. 10.83 (9.03–14.64) P = .004) and (17.23 (11.80–24.22) vs. 13.49 (10.19–17.98) P = .031), respectively. When stratified by dementia status, the associations of CCL2 with delirium were maintained in both groups, but CXCL2 was only significantly higher in delirium in the subgroup without dementia. Discussion CCL2 and CXCL2 are chemokines involved in immune cell recruitment and markers of accelerated immune ageing. Future work should elucidate the role of immune cell recruitment and brain infiltration in delirium, to identify treatment targets. Furthermore, work should explore whether targeting immune ageing can reduce delirium risk.

ABSTRACT To address data gaps around metabolic liver disease in Africa, within the Hepatitis B in Africa Collaborative Network (HEPSANET), we evaluated participants with chronic hepatitis B virus infection (CHB) for steatotic liver disease (SLD) using transient elastography with controlled attenuation parameter (CAP). Using multivariable logistic regression, we identified sociodemographic, anthropometric, and clinical associations with SLD (CAP ≥ 248 dB/m), and evaluated whether SLD severity was independently associated with fibrosis. Short‐term impacts of SLD on mortality were also assessed with Cox proportional hazards analysis. We also evaluated the diagnostic performance of B‐mode ultrasound for SLD, with CAP as reference. Across 6 countries and 1601 participants (age range, 12–80 years; 39.5% female; 4% with diabetes; 11% unhealthy alcohol use; median body mass index, 24.4), 421 (26.3%) had SLD; 219 (13.7%) had severe steatosis (CAP ≥ 280 dB/m). Older age, male sex, overweight and obesity, and HIV‐negative status were associated with SLD. After adjusting for age, sex, HIV status, HBV treatment, and country, severe steatosis, but not mild and moderate forms (i.e., 248–279 dB/m), was associated with a 8.4% (95%, 1.9%–14.9%) increased probability of fibrosis. Over limited follow‐up (median 2.69 years) and small number of deaths, SLD was not associated with all‐cause or liver‐related mortality. B‐mode ultrasound had low sensitivity for SLD, overall or when severe, but specificity was 95%. In conclusion, SLD was common in people with CHB in Africa and its severe form was associated with liver fibrosis. These data support screening for metabolic comorbidities as part of HBV care and treatment in Africa.

Abstract Background Most studies of delirium occurrence are based on selected hospitalised populations or administrative data. We aimed to estimate the incidence rate of in-hospital delirium in older adults and to assess use of the International Classification of Diseases, 10th Revision (ICD-10) delirium code (F05) in electronic health records. Methods This population-based cohort study included 9956 participants aged ≥70 years from HUNT4 70+. Participants were followed from the date of study participation (2017–19) until 31 December 2023. In-hospital delirium was identified by retrospective chart review of all admissions during follow-up, using Diagnostic and Statistical Manual of Mental Disorders, fifth edition criteria. Incidence rates were estimated using the first delirium episode per participant, weighted for non-participation. Results During follow-up, 1507 participants (15.1%) experienced ≥1 in-hospital delirium episode. The weighted incidence rate was 33.1 (95% CI 31.5–34.9) per 1000 person-years and increased with age from 11.4 (age 70–74) to 109.0 (age ≥90) per 1000 person-years. Men had higher incidence rates than women across age groups. Among admissions with delirium, 11.9% had an ICD-10 discharge delirium diagnosis code (F05). Conclusion In-hospital delirium was common, increased markedly with age and was rarely captured by diagnostic coding. Low coding rates suggest that routine health-care data substantially underestimate delirium burden. Under-recognition of delirium may limit opportunities for follow-up in a group at high risk of adverse outcomes. With an ageing population, the burden of delirium is likely to increase for individuals and health-care systems. Our findings support systematic approaches to improve identification and management.

Background
Lung neuroendocrine tumours (NETs, also known as carcinoids) are rapidly rising in incidence worldwide but have unknown aetiology and limited therapeutic options beyond surgery. The current WHO classification, based on mitotic count and presence or absence of necrosis, divides lung NETs into grade-1 typical, and grade-2 atypical tumours. This dichotomous classification however does not account for recently described molecular entities nor is it sufficient for clinical management.
Methods
Here we conducted integrative multi‐omic analyses on over 300 lung NETs including whole‐genome sequencing, transcriptome profiling, and DNA methylation arrays, followed by archetype analysis, to identify and characterise molecular groups. We further investigated molecular groups using spatial RNA sequencing and proteomics, and deep learning analysis of whole slide images.
Results
The integration of multi-omic data provided definitive proof of the existence of four strikingly different molecular groups that vary in patient characteristics, genomic and transcriptomic profiles, microenvironment, and morphology. Among these, we identified a new molecular group, enriched for highly aggressive supra‐carcinoids that displayed an immune‐rich microenvironment linked to tumour-macrophage crosstalk. We uncovered an undifferentiated cell population within supra-carcinoids and show the transcriptomic similarities between supra-carcinoids and the recently identified atypical small cell lung cancer tumours, further demonstrating their molecular link to high-grade lung neuroendocrine carcinomas. Multi-regional genomic analyses identified distinct evolutionary trajectories, suggesting that molecular groups are determined early in tumourigenesis by genomic events, and that transitions between groups, though infrequent, are possible for supra-carcinoids. Deep learning models accurately identified these groups based on morphology alone, outperforming current histological criteria. Together with the validation of a panel of immunohistochemistry markers, we demonstrated that these molecular groups can be accurately identified based on morphological features, facilitating their future implementation in the clinical setting. Our proposed morpho-molecular classification highlights potential group-specific therapeutic opportunities, with differences in expression to DLL3, EGFR, FGFR and TERT inhibitor targets.
Conclusions
Overall, our findings unify previously proposed molecular classifications and refine the lung cancer map by revealing novel tumour phenotypes with potential implications for prognosis and therapeutic management.

Background: Advance care planning can support frail older adults, their next of kin, and healthcare professionals by clarifying patients’ values and preferences and improving communication toward the end of life. Despite these benefits, advance care planning remains underutilized, particularly among frail older adults with complex needs. Limited research has explored healthcare professionals’ experiences with initiating it in acute geriatric settings. As part of a broader research project focused on implementing advance care planning for older adults admitted to acute geriatric hospital units, this study aimed to explore healthcare professionals’ experiences with initiating these conversations.
Methods: A qualitative sub-study, nested within a cluster randomized controlled trial, was conducted with implementation teams and other healthcare professionals from the six geriatric units in the intervention arm. They had recently started engaging in advance care planning with frail, acutely ill older patients and their next of kin. Data were collected through four focus groups and four dyadic interviews, involving a total of 24 participants. The data was analysed by using reflexive thematic analysis.
Results: Four themes developed as perceived benefits: (1) Amplifies the patients’ voice, (2) Improves communication with next of kin, (3) Influences treatment decisions and may avoid overtreatment (4) Contributes to meaningful work. Two themes developed as perceived challenges: (1) Uncertainty regarding the ACP documentation, (2) ACP conversations require dedicated time and focus.
Conclusion: This study revealed that advance care planning conversations in acute geriatric hospital units can amplify patients’ voices and promote their participation in decision-making. Discussions about existential topics appear beneficial and rewarding for patients, next of kin and healthcare professionals, both for in-moment and future decisions. Time constraints may have led to missed opportunities to initiate ACP conversations with patients before the end of life, while documentation issues may have limited their impact on continuity of care.

Abstract Background Point prevalence surveys (PPS) are a core tool of the WHO Global Action Plan on antimicrobial resistance (AMR), yet their implementation in low- and middle-income countries (LMICs) remains limited by weak prescribing and surveillance systems. We conducted a multiseasonal mixed-method PPS, integrating quantitative prescribing data with qualitative ward observations, and the behavioural and operational factors shaping antibiotic prescribing and use of culture testing in a tertiary-care hospital in northern India.
Methods A hospital-wide PPS was conducted across autumn (November 2023), summer (April 2024), monsoon (August 2024), and winter (January 2025), with each phase comprising a two-week data collection episode, following WHO Global-PPS methodology. Data were collected from five inpatient departments (Medicine, Surgery, Obstetrics-Gynaecology, adult, paediatric, and neonatal intensive care units (ICUs) using standardised forms. Quantitative data on indications, routes, and AWaRe categories were supplemented with ward observations and inpatient follow-up to assess culture testing, antibiotic sensitivity test (AST) use, and treatment modifications.
Results A total of 1,680 inpatients were surveyed. Ceftriaxone (30–33%), Piperacillin–tazobactam (9–20%), and Doxycycline (8–16%) were the top prescribed antibiotics. Azithromycin use dropped sharply after the first phase. Amikacin (6–9%) and Meropenem prescribing (4–7%) remained low. Empirical prescribing dominated (63–67%), while culture-guided therapy remained ≤ 6%. Over 80% of antibiotics were given parenterally. Watch antibiotics accounted for 46–56% of prescriptions, Access 35–51%, and Reserve ≤ 4%. Prophylactic use ranged from 24–30%, and combination therapy was common in the ICU. Clinical diagnoses showed seasonal variation, with gastrointestinal (20–22%) and chronic conditions (17–23%) most frequent, and respiratory infections (3–9%) peaking in monsoon and winter. Antibiotic modification following AST occurred in only 4.7–6.5% of cases. Qualitative findings highlighted stock-outs of oral antibiotics, delays in culture sampling, and documentation gaps during patient transfers, limited stewardship activities, collectively reinforcing broad-spectrum empirical use.
Conclusions This PPS found high empirical and broad-spectrum antibiotic use, limited culture-based prescribing, and systemic gaps hindering stewardship. Strengthening diagnostic access and use, ensuring drug availability, and embedding multidisciplinary stewardship teams with real-time feedback are essential to promote evidence-based prescribing in resource-limited settings.

Abstract Defective mitophagy is a key pathological risk of Alzheimer’s disease (AD), yet available pharmacological agents capable of restoring mitophagy remain clinically limited, largely due to the lack of tools for drug screening and target identification. To address this, we introduce an integrated platform with three novel artificial intelligence (AI)-enabled approaches—the phenotype-based screening model ‘CellAutoFM’, the drug-likeness predictor ‘ADMET Ranker’, and the target deconvolution method ‘PROCAT’. Using this platform together with wet lab validation, Crassifolin A (JGX-1) is identified as a potent natural product-derived mitophagy inducer; structurally, JGX-1 directly binds to ATP-binding cassette sub-family A member 1 (ABCA1) and sub-family G member 1 (ABCG1), triggering enhanced cholesterol efflux which activates PINK1/Parkin-dependent mitophagy. In both in vitro and in vivo AD laboratory models, JGX-1 reduces amyloid-β and Tau pathologies in an autophagy/mitophagy-dependent manner as these benefits are dramatically blunted with key mitophagy genes knocked down. Collectively, we provide an open-access, reproducible AI-assisted platform for rapid compound screening, drug-likeness evaluation, and potential target prediction to advance drug discovery. Through this paradigm, we unveil modulating the ABCA1/ABCG1-mediated cholesterol-mitophagy pathway as a promising anti-AD strategy, with a novel drug candidate JGX-1 identified.

Acute sinusitis – quo vadis?
Full text in Research Archive
Show summary
Acute sinusitis (AS) is a frequent reason for primary care visits and has historically been a major cause
of unnecessary antibiotic prescribing, contributing significantly to antimicrobial resistance (AMR). This
symposium presents findings from the Norwegian BASIC (Better treatment for Acute Sinusitis In primary
health Care) project, which addresses AMR and promotes sustainability by tackling diagnostic
uncertainty, evaluating treatment alternatives, and studying patient and prescriber expectations. The
central theme is the shifting landscape of AS management: while Norway has succeeded in substantially
reducing antibiotic use, clinical decision-making remains complex, driven by patient-reported symptoms
rather than clear guidelines.
Aim of the symposium
The overarching purpose of this symposium is to contribute to an improved understanding and
management of acute sinusitis (AS) in Nordic general practice. The symposium will provide clinically
useful information by presenting the newest evidence regarding appropriate AS management, thereby
enhancing patient care. For the academic audience, the symposium offers academically valuable
insights by detailing the innovative research methodology utilized in the BASIC project.

Implementing Digital Antenatal Care Registries at Public Health Care Facilities in Ethiopia and Uganda: A Comparative Study of Barriers and Enablers.
Full text in Research Archive
Show summary
Background: Digital registries can support clinical decision-making, longitudinal tracking of patient care and resource allocation decisions. Yet, questions around how to implement registries remain unsolved. We identified barriers, enablers and implementation strategies for digital antenatal care registries (ANC eRegistry) in public primary health facilities in Ethiopia and Uganda.
Methods: We utilized data from research projects on ANC eRegistries in sub/urban Ethiopia (Addis Ababa, Adama, Harar, Jimma) and Uganda (Mukono). Interviews (n=35) and focus groups (n=18, 90 persons) with ANC providers, health facility managers, and sub-national and national leaders, identified through purposive sampling were English-translated and coded (NVivo) through deductive thematic analysis mapped onto Consolidated Framework for Implementation Research domains, identifying barriers and enablers in common and unique to each country. CFIR-ERIC (Expert Recommendations for Implementing Change) tool matched barriers and enablers to implementation strategies.
Results: Among common barriers were financial constraints exacerbating infrastructure insufficiencies (computers, servers, unreliable networks), and heavy provider workloads constraining time towards meticulous data entry. Enablers in both settings were high motivation to switch to digital records, perceived ease of use of the eRegistry, simplified tasks and improved workflows, and existing peer-learning networks. Unique barriers in Ethiopia were lack of interoperability with existing health information systems and providers prioritizing clinical care over data entry: strong leadership commitment to digitalization was an enabler. In Uganda, limited proactive leadership and difficulties learning new systems were unique barriers; computer-literate young providers was an enabler. Six common implementation strategies were: Conducting ongoing training, preparing champions, formal implementation blueprints, provider performance data, coalition building, and conducting cyclical small change tests.
Conclusions: Despite different contexts, identifying implementation determinants and strategies in common and unique to different contexts can facilitate adoption of digital registries. Health systems should capitalize on provider motivation to digitalize while ensuring training, resources and leadership.

No One to Lose – Workplace: A Brief Training to increase Employees’ Confidence in Responding to Suicide Risk.
Full text in Research Archive
Show summary
Background:
Workplaces can play a crucial role in suicide prevention — colleagues are often the first to notice when something’s off. “No One to Lose – Workplace” is a short, team-based training program developed to increase the employees’ confidence in responding to possible suicide risk. The program mixes short films with guided group discussions. Organizations receive some initial guidance by phone but run the sessions themselves, making it a flexible option that fits into everyday work routines.
Method:
A total of 20 organizations with approximately 800 employees were offered the training. The employees were invited to participate in anonymous online surveys before and after the training, and N=399 and N=197 responded to the pre- and post-tests, respectively. Participants rated their confidence (on a 1–10 scale) on recognizing suicide risk, talking directly and offering support to a person with suicidal thoughts. We compared confidence levels before and after the training, using independent samples t-tests. Additionally, implementation was assessed in terms of completion rates, teamwork, and leadership involvement.
Results:
Most participants completed the training, joined the discussions, and many managers took part. Overall, scores increased towards higher confidence in the posttest, and most markedly with regards to talking openly about suicide and knowing how to offer support. Qualitative feedback was overwhelmingly positive — participants highlighted the program’s relevance, practicality, and impact on reducing stigma. Some wanted more time, clearer crisis routines, and extra tools. Many also expressed interests in further training.
Conclusion:
A short team-based program can strengthen employees’ confidence in responding to suicide risk — especially by encouraging open dialogue and supportive action. Manager participation and collective reflection seem to boost the effect. While self-report data and the lack of a control group limit causal conclusions, the findings suggest that simple, discussion-based training can make workplaces more prepared to prevent suicide.

Medical students admitted by grade point average present low personality diversity in Norway.
Full text in Research Archive
Show summary
Background ---- Norway admits medical students solely by GPA. Combined with free education and low attrition, future doctors are largely a result of self-selection. ---- Aim ------ Characterise personality traits of medical students at the University of Oslo and compare them with the Norwegian population and medical students in other countries. ---- Methods ---- Across 2022-2026, five newly admitted medical student cohorts completed the BFI-20S (n=535, 89%) and six the MBTI (n=632, 95%) personality tests (mean age = 22.1, 68% female). T-tests compared BFI-20S with Norwegian workers (n=6,372), French (n=1132) and German (n=880) medical students. Chi-squared tests compared MBTI sixteen personalities between general Norwegians (n=159,468), U.S. (n=83) and South Korean (n=171) medical students. ---- Results & Conclusion ---- On BFI (1–5), Norwegian medical students scored higher on Agreeableness, mean 4.3 (SD=0.5) and Extraversion, 3.7 (0.89), though lower in Neuroticism, 2.5 (0.8) than French (3.8, SD = 0.6; 3.1, SD = 0.8; 3.0, SD = 0.8) and German (4.2, SD = 0.6; 3.6, SD = 0.8; 2.9, SD = 0.8) medical students (p < 0.001). These also differed from Norwegian workers (p < 0.001). Most medical students were MBTI Extraverted (70%), Intuitive (59%), Feeling (66%), and Judging (78%). The ENFJ (22%) was overrepresented compared with U.S. (2%) and South Korean (3%) medical students, and Norwegians (6%) (p < 0.001). Norwegian medical students display high Conscientiousness and Agreeableness with gross overrepresentation of ENFJ. Although these are considered well-suited for medical practice, the limited personality diversity might implicate coping mechanisms and problem-solving strategies in the physician workforce.

Hos barn kan diagnosen stilles uten gastroskopi dersom antistoffnivået er høyt nok i to prøver (over 10 ganger øvre normalområde).

Corticotroph tumors, a subtype of pituitary neuroendocrine tumors (PitNETs). Functioning corticotroph adenomas (FCA) secrete excess biologically active adrenocorticotropic hormone (ACTH), causing Cushing’s disease, whereas silent corticotroph adenomas (SCA) are typically diagnosed as non-functioning pituitary adenomas (NFPAs) and present with mass-related symptoms. Despite their shared lineage, SCA tend to be larger, more aggressive, and more prone to recurrence. Their hormonal silence remains poorly understood, but may involve impaired proopiomelanocortin (POMC) processing, defective secretory pathways, and reduced activity of the prohormone convertase PC1/3 (PCSK1).
This PhD project investigated factors that may contribute to hormonal silence and aggressiveness in corticotroph tumors, focusing on the PC1/3 inhibitor PCSK1N, the cytoskeletal regulator PDZ and LIM domain protein 1 (PDLIM1), and endoplasmic reticulum (ER) stress. We quantified PCSK1N and corticotroph marker expression in patient-derived FCA and SCA samples and assessed associations with clinical features. We induced ER stress in mouse corticotroph tumor cells and primary human FCA cultures to examine its effects on cell survival and ACTH production. Lastly, PDLIM1 was overexpressed in mouse corticotroph tumor cells to assess its effects on cell viability, migration, and morphology related to invasive behavior.
We found that corticotroph cells rely on effective ER function for ACTH production and secretion. ER stress increased PCSK1N expression and reduced secreted ACTH, suggesting a potential mechanism contributing to hormonal silence in SCA. PDLIM1 expression was higher in SCA than in FCA, and its overexpression promoted cell viability and migration in vitro, indicating a possible role in tumor aggressiveness. These findings highlight ER stress, PCSK1N, and PDLIM1 as potential contributors that influence hormonal silence and invasive behavior in corticotroph tumors.
