Implementation of advance care planning in the routine care for acutely admitted patients in geriatric units
The overall aim of the project is to improve health services, user involvement and patient-centered care for frail older adults and their next of kin, in an efficient, sustainable, and coordinated way, through better implementation of advance care planning.
