The excess demand for health professionals persists in many countries in spite of systematic increases in the education capacity in order to meet the demand. Regulations through quotas have been widely used in countries with a national health service (NHS). With the deregulation of health markets, incentives such as the health professional’s pay and practice income play a relatively larger role in the implementation of health policies. This thesis aims to explore the short-term impact of increased wages on the working hours of health personnel and their practice choice. An additional objective is to identify existing compensating differentials in the job market for health personnel.

To analyze this question a structural labor supply model with utility maximizing individuals is applied. The utility maximization problem is solved by discretizing the budget set and choosing the optimal job package from a finite set of alternatives. The econometric framework allows for non-convex budget sets, nonlinear labor supply curves and imperfect markets with institutional constraints.

2005: 2 Regulation versus practice The impact of accessibility on the use of specialist health care in Norway

In Norway specialized health services are provided both by public hospitals and by privately practicing specialists who have a contract with the public sector. Patients’ co-payment is the same irrespective of the type of provider they visit. The ambition of equity in the allocation of medical care is high among all political parties. The instruments for auditing whether these goals are fulfilled are not equally ambitious. The objective of the present study is to explore whether laws and regulations that govern the allocation of specialist health care resources in fact are fulfilled. Panel data from the Survey of Living Conditions are merged with data on capacity and spatial access to primary and specialist care. We find that accessibility and socio-economic variables play a considerable role in determining both the probability of at least one visit and the number of visits to a private specialist. A person with a higher university degree living in a municipality with the highest value of the geographical accessibility index has a 46%-points higher probability of at least one visit to a private specialist compared with a person with junior high living in a municipality with the lowest value of the accessibility index. With regard to visits to a hospital outpatient department these variables are not found to have significant effects.

The authors conclude that public ambitions and regulations are fulfilled for specialist services provided by public hospitals. With regard to the provision of services provided by publicly financed private specialists they find a discrepancy between public goals and surveyed practice.

2005: 3 Utviklingen i fastlegenes listelengder, driftsinntekter og takstbruk

I denne rapporten undersøkes utviklingen av fastlegenes listelengder, driftsinntekter og takstbruk, fra utprøving av fastlegeordningen (1993-1996) til etter at fastlegereformen ble innført sommeren 2001. Undersøkelsen viser at fastlegene i de tidligere forsøkskommunene for fastlegeordningen (Lillehammer, Åsnes, Trondheim og Tromsø) hadde fått kortere lister i 2003 enn de hadde i 1993. Listene til legene er imidlertid fremdeles lengre enn ellers i landet. Data viser også at gjennomsnittslegen i forsøkskommunene i 2003 hadde færre ledige plasser på sin liste enn fastlegene ellers i landet, noe som kan skyldes at forsøkskommunene fortsatt har en dårligere legedekning enn landsgjennomsnittet for norske kommuner.

I perioden etter fastlegereformen har gjennomsnittslegen i forsøkskommunene nedjustert sine listetak. På landsbasis derimot hadde omtrent 21 % av fastlegene oppjustert sitt listetak fra 2001 til 2003, mens 22 % hadde nedjustert sine listetak. I perioden 1994 til 2003 hadde brutto driftsinntekter for fastlegene i de tidligere forsøkskommunene økt. Veksten i driftsinntekter per person har vært høyere enn vekst i total driftsinntekt. Før den landomfattende reformen hadde legene i forsøkskommunene betydelig høyere driftsinntekter enn i landet forøvrig, noe som kan forklares med den lave legedekningen i kommunene. Fastlegene i forsøkskommunene hadde fortsatt noe høyere driftsinntekter enn ellers i landet, men forskjellene er mindre i perioden etter 2001.

Undersøkelsen viser vider at bruk av ordinær konsultasjonstakst, bruk av enkel pasientkontakt og samlet bruk av laboratorietakster per person på fastlegens liste har gått ned i perioden 2001-2003. Legenes bruk av tidstakst er imidlertid på et litt høyere nivå i 2003 enn i 2001. Det kan tyde på at befolkningen får noen færre konsultasjoner, men at konsultasjonene i gjennomsnitt har blitt lengre.

2005: 4 Strategier for bedre helse og funksjonsevne blant eldre

Eika, K.H., Lurås, H., Økonomisk institutt, Institutt for helseledelse og helseøkonomi og HEROForebyggende arbeid i helse og omsorgstjenester er spesielt viktig ovenfor eldre. Men tjenestene som tilbys må være stabile og av god kvalitet.

Hvilke forebyggingstiltak overfor eldre bør prioriteres? I rapporten går forfatterne gjennom forebyggende arbeid, med vekt på tiltak som med sikkerhet eller trolig er samfunnsøkonomisk lønnsomme. Med utgangspunkt i denne gjennomgangen er det like nærliggende å spørre hvorfor forebyggingstiltak som en vet har stor samfunnsmessig nytte ikke har et større omfang.

Kvaliteten på behandlende helse- og omsorgstjenester er særlig viktige i det forebyggende arbeid overfor eldre. Det gjelder ikke minst i forhold til mentalt eller fysisk svekkede eldre. Svekkede eldre vil ofte ikke ha tilstrekkelig kompetanse eller autoritet til å sikre kvalitet i tjenesterelasjonen. Samtidig er god skjønnsutøvelse viktig, og forutsetter, i tillegg til menneskelig innsikt, ofte også betydelig geriatrisk kompetanse. Slike kvalitetsaspekter kan i liten grad måles eller veies, og det gjør ytre kontroll med kvalitet og ressursutnytting vanskelig. Forebygging krever også et langsiktig og overordnet perspektiv, samtidig som gode tiltak lett kan bli underbudsjettert i konkurranse med mer akutte og livsnødvendige tiltak, og med tiltak der effektene er lettere å dokumentere.

Disse utfordringene vil være tilstede i en hver organisasjonsform. Økt satsing på forebygging forutsetter at det tilføres ressurser og kompetanse til de tjenestene som er viktige for forebygging, men dette er ikke alene en garanti for at forebygging faktisk gjennomføres. Det må samtidig være et fokus på om de institusjonelle og organisatoriske rammevilkår har en utforming som bidrar til nødvendig stabilitet i tjenestetilbudet, og til god kvalitet.

2005: 5 Aktivitetsbasert finansiering av pleie- og omsorgstjenestene

En sterk økning i eldre stiller krav til omfang og kvalitet i pleie- og omsorgstjenestene. I Norge i dag mottar mer enn 200 000 personer pleie- og omsorgstjenester. Kostnaden for dette er rundt 46 mrd kroner. Den tradisjonelle modellen for produksjon av velferdsgoder i Norge og Norden for øvrig, er kjennetegnet ved at staten har delegert ansvar for tjenesteproduksjonen til kommuner eller fylkeskommuner, at staten gjennom finansieringsordninger og reguleringer forsøker å påvirke kommunenes prioriteringer, og at tilbudet er tilnærmet gratis for brukerne. I dag mener mange aktører at det er behov for et bedre utbygd tjenestetilbud på mange områder. Det stilles også krav om økt kostnadseffektivitet, og brukerne ønsker rett til selv å velge tjenesteprodusent. I denne rapporten stilles det spørsmål om aktivitetsbasert finansiering av pleie- og omsorgstjenestene representerer en løsning. Hvordan kan eventuelt en aktivitetsbasert finansiering av pleie- og omsorgstjenestene tilpasses i Norge og hvilke hensyn vektlegges i forhold til dagens rammefinansieringsmodell av kommunesektoren.

Det gis det en grundig gjennomgang av finansieringen av pleie og omsorgstjenestene i utvalgte OECD-land utenom Norden; Tyskland, Japan og USA med vekt på tre forhold: organiseringen av forsikringsordningen, tildelingskriterier og kontrakter mellom forsikringsselskap og tjenesteyter. Viktige spørsmål er om det finnes former for aktivitetsbasert finansiering i disse landene og hvordan dette eventuelt er organisert. Rapporten konkluderer med at det er mulig å innføre statlig aktivitetsbasert finansiering av kommunenes pleie- og omsorgstjenester, men at en slik modell kanskje ikke vil representere en løsning som vil være stabil over tid. En ikke usannsynlig utvikling er at modellen vil skape ansvarsuklarheter mellom kommuner og stat. En vil da komme i en situasjon der en må velge mellom tilbakegang til dagens rammefinanseiringsmodell og statlig overtakelse av pleie- og omsorgstjenestene.

Rapporten er skrevet på oppdrag av Helse- og omsorgsdepartementet og utgjør deler av departementets grunnlagsarbeid i forbindelse med kommende stortingsmelding om eldreomsorgen.

2005: 6 I skyggen av Fastlegeordningen: Hvordan har det gått med det offentlige legearbeidet?

Godager, GC., Lurås, H., Institutt for helseledelse og helseøkonomi og HEROEtter innføringen av fastlegeordningen (FLO) rapporteres det at offentlige legestillinger i mindre grad enn før reformen står ledige. Samtidig er det et gjennomgående tema fra helsetilsynet i fylkene at det offentlige legearbeidet ikke har kommet styrket ut etter innføringen av FLO. På bakgrunn av innsamlede data fra 35 representativt valgte kommuner beskrives i en ny rapport ulikheter i kommunenes organisering av det offentlige legearbeidet, samt i hvilken grad fastlegene deltar i arbeidet.

Kommunene har ansvar for å skaffe leger som kan utføre arbeid ved sykehjem, helsestasjon og skolehelsetjeneste, samt samfunnsmedisinske oppgaver som planlegging og beredskapsarbeid i forbindelse med ulykkeshåndtering, miljørettet helsevern, smittevern og administrative oppgaver. Det offentlige legearbeidet er et viktig tilbud til innbyggerne i en kommune. Det har tidligere vært vanskelig å få privatpraktiserende leger til å utføre denne typen arbeid fordi det har blitt regnet som dårlig betalt i forhold til privatpraktiserende praksis. Leger som er fast ansatt i kommunene har derimot generelt vært mer positive til å påta seg offentlig legearbeid.

Med innføringen av fastlegeordningen (FLO) i 2001 ble det åpnet for at kommunene kunne pålegge fastlegene offentlig legearbeid inntil 7,5 timer per uke. En ny rapport fra Institutt for helseledelse og helseøkonomi, viser at det er stor variasjon i hvor mye fastlegene deltar i dette arbeidet etter reformen. Totalt er det kun 17,3 % av fastlegene som arbeider mer enn 7,5 timer i uka for kommunen. I 2002 arbeidet fastlegene i gjennomsnitt 5,55 timer per uke med offentlig legearbeid. Det virker som kommunene ikke utnytter potensialet påleggsklausulen gir for å få leger til å arbeide med oppgaver i det offentlige, sier Hilde Lurås en av forskerne bak rapporten. Dersom alle fastlegene i utvalget som i dag jobber mindre enn 7,5 timer, arbeidet 7,5 timer per uke, kunne det samlede offentlige legearbeidet økes med 40 %. Spørsmålet er imidlertid om det er ønskelig at det offentlige legearbeidet skal organiseres ved at alle fastleger pålegges å delta eller om det kan tenkes andre ordninger.

Typically, the participation rate is below 100 per cent. In this paper pecuniary compensation is used to increase the participation rate. In a postal questionnaire to 5,000 people invited to screening for colorectal cancer, those not participating were asked "would you participate if you were given NOK X in compensation?"

The results show that compensation increases participation and that the participation probability systematically varies with travel expenses, income, age, county, native country, marital status, use of health care services, genetic predisposition, expected benefit from the screening, subjective health status, and education. The estimated costs per additional screening are increasing.

How will a general practitioners income per listed person be influenced by the composition of the patient list according to indicators of need for services, and of accessibility according to the GP density in the municipality? In the Norwegian capitation system each general practitioner (GP) has a personal list of patients. The payment system is a mix of a capitation fee and fee-for-service. From a model of a GP’s decisions we derive the optimal practice profile contingent on whether a GP experiences a shortage of patients or not. We also find the conditions for whether a GP, who experiences a shortage of patients, is likely to increase the number of services he provides to his patients. Data give us the opportunity to reveal patient shortage, i.e. a positive difference between a GP’s preferred and actual list size, at the individual practice level.

From the analysis of 2587 Norwegian GPs (out of a total 3650) the main result is that patient shortage has a positive effect on a GP’s intensity of service provision and hence, on the income per listed person. We also find that a GP’s income per listed person is influenced by the composition of the list according to indicators of need for services, and of accessibility according to the GP density in the municipality. These results are also valid when possible selection bias is accounted for, although the magnitude of the effects is then smaller.

In this report the authors show that recent attempts to reorganise and cut costs in the Norwegian health care and social services sectors have had the unintended side effects of raising the level of sickness absence and disability among the employees, and that these effects have persisted several years after completion of the reorganisation processes. Since a substantial proportion of the resulting costs are external to the decision-makers, we suspect that the pace of change may have been excessively high. Changes that were efficient from each service provider’s point of view may have been inefficient from a social and a public-finance point of view.

What aspects of quality are important to consumers of hospital services? Do quality indicators, such as readmission rates and waiting time, capture the quality that consumers demand? The main empirical finding is that hospitals’ readmission rates have a negative and significant effect on inpatients’ experiences. Patients admitted to hospitals with low readmission rates are more content with the care, treatment, and information they receive from hospital personnel.

In 2002 the state took over ownership of the Norwegian hospital sector and organized it through five regional semiautonomous companies. The motivation behind this was more efficient use of hospital resources, equal access despite geographical differences and a higher quality of health services. Cost efficiency, measured as total activity relative to total costs, decreased during the 1990s. Part of the decrease can be explained by increased labour costs. It is often assumed that decreasing costs lead to lower quality. If this is the case in the health sector, one would expect to see a higher level of quality when costs per patient increase. But what aspects of quality are important to consumers of health services? Do quality indicators, such as readmission rates and waiting time, capture the quality that consumers demand? This report is an attempt to answer these questions.

The data on patient satisfaction with hospital services were taken from an anonymous survey among patients admitted to somatic hospitals, the Norwegian Patient Register. They received the surveys two to three weeks after discharge. The survey consisted of 50 questions that were grouped into seven category variables, according to the type of service the different questions concerned. Patients were also asked about their gender, age, health status, education level, number of admissions last two years, and whether their first language was Scandinavian. The main empirical finding is that hospitals’ readmission rates have a negative and significant effect on inpatients’ experiences. Patients admitted to hospitals with low readmission rates are more content with the care, treatment, and information they receive from hospital personnel. They are also more content with hospital facilities and sanitary conditions and organization of hospital staff. Patients’ age, health status, number of previous admissions, and education level significantly affected their satisfaction with hospital services. The results for waiting time were more ambiguous. Patients’ impression of hospitals’ facilities and sanitary conditions was better at hospitals with longer waiting time.

Using register data for the entire Norwegian population aged 50-89 in 1980-1999, in which there are ¾ million deaths, it is estimated how the proportions who are divorced or never-married in the municipality affect all-cause mortality, net of individual marital status. The data include individual histories of changes in marital status and places of residence, and provide a rare opportunity to enter municipality fixed-effects, capturing the time-invariant unobserved factors at that level, into the models. The positive health externality of marriage that has been suggested in the literature is supported by some of the estimates for women, while other estimates, and especially those for men, point in the opposite direction. These findings may indicate that a high level of social cohesion is not as beneficial as often claimed, at least not for both sexes, that marriage perhaps undermines rather than strengthens social cohesion, or that other mechanisms are involved, for example related to people’s perceptions of their health relative to that of others. Estimates from models without such municipality fixed-effects are markedly different, but these also shed doubt on the notion that a high proportion not married generally increases individual mortality.

The focus of this study is the effect of a laboratory analysis and socioeconomic variables on choosing medical actions in a specific situation (a clinical vignette - a young woman, Mrs Hansen, with dyspepsia - presented to GPs). We assume that the GP’s decision depends on what he or she thinks is best for the patients, based on the best clinical evidence available. Significant variables associated with the choice of medical actions are: the result of the Helicobacter pylori (HP) test, the GP’s stated importance of HPRT, the location of the general practice, the GP recommending sick leave, the GP’s stated probability that Mrs Hansen’s symptoms are due to a H.pylori infection after the HP-result is known, and how the GP follows up the patient. Our results show that the HP-analysis has a significant and major influence on the GPs choice of medical actions. Therefore the quality of the analysis is likely to affect the patients’ health and social costs. Hence institutions for quality monitoring and improvement are important elements of health care reforms. Such institutions should balance cost and benefits of quality improving measures, and will be the focus of closer studies in our future research.

In health economics, cost-effectiveness is defined as maximized health benefits for a given health budget. When there is a private alternative to public treatments, care must be taken when using costeffectiveness analysis to decide what types of treatments should be included in the public program. The correct benefit measure is in this case the sum of health benefits to those who would not be treated without the public alternative and the cost savings to those who would otherwise choose private treatment. In the socially optimal ranking of treatments to be included in the public health program, treatments should be given higher priority the higher are costs per treatment for a given ratio of gross heath benefits to costs.

Rapporten utgjør sluttdokumentasjon for et oppdrag utført i 2004 av HERO for Riksrevisjonen. Notatet ble endelig overlevert 31.1.2005 men har vært underlagt utsatt offentlighet fram til 21.10.2005. Riksrevisjonens undersøkelse av tjenester fra laboratorie- og røntgenvirksomheter”, Dokument nr. 3:3 (2005-2006). Undersøkelsen viser at private røntgeninstituttene står for om lag en tredel av den samlede røntgenaktiviteten i Norge, og private laboratorier står for om lag en sjettedel av aktiviteten innen klinisk kjemi. Andelen private er høyest i Helse Øst og lavest i Helse Nord. Det er forskjeller i sammensetning av tjenester mellom offentlige sykehus og private enheter. En rekke omfattende undersøkelser gjøres utelukkende i det offentlige. De offentlige sykehusene har i større grad døgnkontinuerlig vakt, og forskning er konsentrert til noen få offentlige enheter. Det har vært problematisk å overføre informasjonen fra sykehusenes røntgensystemer til kodene i takstsystemet. Det mangler også en standard for analysenavn innen medisinsk biokjemi. Dette innebærer trolig at mange sykehus ikke har tilstekkelig god informasjon om omfanget og sammensetningen av egen virksomhet. Variasjonen i bruk av analysenavn i laboratorievirksomheten innebærer også at det vanskelig å se at refusjonsmyndighetene vet hvilke om de betaler riktig refusjon. Undersøkelsen viste også at offentlige enheter i liten grad har informasjon om deres andel av kostnader knyttet til sykehusenes felles drift, og derfor ikke gode anslag over hva deres tjenesteproduksjon koster.
