Aim
To investigate associations between muscle tone measured using the Modified Ashworth Scale (MAS) and pain in children with spastic cerebral palsy (CP).
Method
Children (aged 0–17 years; mean age 10 years 2 months) in the Norwegian Quality and Surveillance Registry for Cerebral Palsy with physical therapy assessments between 2018 and 2021 were included ( n = 1188; 42.3% female) in this population‐based cross‐sectional study. Logistic regression analyses were used to examine the association between MAS scores and pain. Odds ratios (ORs) with 95% confidence intervals (CIs) were reported.
Results
Pain was experienced by 41.4% ( n = 492). Risk of pain was highest in children classified in Gross Motor Function Classification System level V (OR = 1.81, 95% CI = 1.21–2.70) and Manual Ability Classification System level V (OR = 2.00, 95% CI = 1.25–3.16), children with bilateral spastic CP (OR = 1.34, 95% CI = 1.06–1.68) and increasing age (OR = 1.12, 95% CI = 1.09–1.15). Pain interfered with sleep in 24.8% ( n = 122) of children and activities of daily living (ADLs) in 53.0% ( n = 261) of children. An MAS score of 2 or greater was recorded in 42.3% ( n = 502), most often in the ankle plantar flexors. Adjusted analyses showed that the highest MAS score was associated with pain interfering with ADLs (adjusted OR = 1.40, 95% CI = 1.00–1.96), and an MAS score of 2 or greater with higher pain intensity (adjusted OR = 1.2, 95% CI = 1.01–1.42).
Interpretation
Pain is common in children with spastic CP, frequently disrupting sleep and ADLs. Higher MAS scores are modestly associated with increased pain intensity and interference, highlighting the need for targeted pain screening strategies.

When standards fall short: The use of discretion and just allocation of long-term care services in Norway.
BMC Health Services Research.
Fulltekst i vitenarkiv

Background: Care administrators in long-term care (LTC) are ‘gatekeepers’ to the services and handle
applications, investigate care needs, and decide if, what and how much services applicants are entitled
to receive. Various models of service allocation have been in operation in the Nordic countries since the
inception of LTC services in the 1970s. The main drivers of the development of allocation models have
been concerns about service inequality among users, quality of care, patient safety, and the overall
efficiency within healthcare settings. While care administrators have several laws and guidelines to aid
their decision-making, the use of professional discretion is known to be widespread but less
investigated. In the context of Norway, this article seeks to explore: What guides care administrators in
reaching decisions? How and in which circumstances is the use of discretion needed in making
decisions?
Method: Qualitative in-depth interviews with 5 managers in care administration and 13 care
administrators in 5 Norwegian municipalities. The material was analysed using thematic analysis.
Results: Two main themes were identified. These are Negotiating standards with discretion and collegial
discussion, and Complexities in deciding needs and services.
Conclusion: Care administrators hold significant responsibility in ensuring fair service allocation.
Discretion is a necessary and valuable tool that can strengthen the integrity and trustworthiness of care
services. However, increased reforms and standardisation place heavy pressure on administrators,
limiting their discretion through stricter allocation criteria. This may reduce their ability to act in the best
interests of service applicants in line with the fiduciary principle.

Study Objectives: To examine the prognostic value of the apnea-hypopnea index (AHI), desaturation severity parameters, and cardiac troponins alone and combined for major cardiovascular events (MACEs).
Methods: MACE data were retrieved in 2021 from the Norwegian Patient Registry for 518 participants in the Akershus Sleep APnea (ASAP) cohort. Baseline polysomnography and fasting blood samples were collected between June 2006 and January 2008. Desaturation duration (DesDur) and severity (DesSev) were calculated using Automatic Blood Oxygen Saturation Analysis software. Cox regression models estimated hazard ratios (HRs) for MACE. Predictive properties of combining troponins and obstructive sleep apnea severity were calculated by comparing established clinical thresholds for cardiac troponin I (cTnI) and T (cTnT) with AHI clinical thresholds of ≥15 and ≥30, respectively.
Results: High AHI, DesDur, DesSev, cTnI, and cTnT were associated with increased MACE risk. However, only cTnI independently predicted MACE after adjustment (HR: 1.74, 95% CI: 1.32–2.29). The HR for MACE was 2.68 (95% CI: 1.03–6.97) in patients with both high cTnI and AHI ≥30 events/h.
Conclusion: In this 15-year follow-up, cTnI was independently associated with risk of MACE, whereas the AHI, desaturation parameters, and cTnT were not independent predictors. cTnI, especially when combined with AHI, was a stronger MACE predictor than cTnT. Provided our findings are validated in clinical obstructive sleep apnea populations, the measurement of cTnI may be considered for cardiovascular risk stratification.

Background As many newly graduated nurses lack proficiency in practical nursing skills, understanding their development during education and placements is important for delivering high-quality patient care. This study explored first-year nursing students’ reflections on practical skill performance during clinical placements, using Bjørk’s model of practical skill performance as a framework.
Methods A qualitative content analysis was employed, analysing reflection notes from 22 first-year nursing students through deductive content analysis, guided by Bjørk’s model, with additional inductive content analysis to explore qualitative variation in the depth of reflection.
Results The results revealed that students demonstrated patient-centred care, preparation, and the integration of technical, relational, and contextual elements. Further, that students’ reflection notes varied in how descriptively and analytically they applied Bjørk’s model.
Conclusions Variability in the depth of reflections using Bjørk’s model highlights the need for tailored educational strategies to promote reflective thinking when learning and performing practical skills.

Background: Clinical placements are critical components of nursing education. Assessment tools have been developed to assess students’ competencies against preset learning outcomes (LOs), yet assessment remains challenging due to the complexity of clinical learning environments.
Aim: To generate knowledge on the coherence between LOs and assessment criteria in clinical placements.
Methods: A qualitative, descriptive design was applied, using practice-oriented document analysis on study plans across four nursing programmes and one assessment tool.
Findings: Variations in the coherence between the documents were found. Across 23 subject plans, LOs varied widely in number and were largely general in scope. Use of the different parts of the Respons tool, and thus number of criteria, differed across placements and institutions.
Conclusion: The study highlights systemic and practical challenges in assessing students’ competencies and underscores the need for continued development of assessment tools that ensure coherence between LOs and criteria, customised for the placement context.

Background: Multiligament knee injuries (MLKIs) are serious and heterogeneous injuries that often lead to long-term functional limitations. There is limited literature regarding objective muscle strength and functional recovery after surgical treatment of MLKIs. Purpose: To investigate long-term quadriceps and hamstring muscle strength and functional recovery after surgical treatment of MLKI and to compare postoperative outcomes between single cruciate MLKI and bicruciate MLKIs. Study Design: Cross-sectional study; Level of evidence, 3. Methods: Patients surgically treated for an MLKI at a single level 1 trauma center between January 2013 and December 2020 were included. Isokinetic quadriceps and hamstring muscle strength were assessed using a Biodex dynamometer, and physical function was measured by single-leg hop, triple hop, crossover hop, and 6-m timed hop tests, with the limb symmetry index (LSI) calculated. Results: Of 124 eligible patients, 90 (72%) participated at a mean follow-up time of 7 years (SD, 2.1) since surgery to evaluation. The median age at the time of testing was 49 years (range, 21-72) and 59% were male. In total, 54% of the patients had not regained full quadriceps strength (LSI <90%), and 46% had persistent hamstring deficits for peak torque/body weight (PT/BW) at 60 deg/s. Across the entire cohort, there was a significant difference between the uninvolved and involved sides for quadriceps and hamstrings for PT/BW ( P = .01). The mean LSI for quadriceps PT/BW was 88% at 60 deg/s and 90% at 240 deg/s, while for the hamstrings, the mean LSI was 93% at 60 deg/s and 97% at 240 deg/s. Median LSIs for the hop tests ranged from 93% (range, 41%-187%) for single leg to 99% (42%-141%) for crossover; however, 44% of patients failed to reach the 90% threshold for the single-leg hop. The 6-m timed hop test and quadriceps muscle strength were significantly lower for patients with bicruciate injuries than those with single cruciate injuries ( P < .05). Conclusion: Persistent deficits in quadriceps and hamstring muscle strength were observed 7 years after MLKI surgery, with a substantial proportion of patients failing to regain full functional symmetry in muscle strength and hop test performance. Patients with bicruciate injuries demonstrated significantly more pronounced deficits in muscle strength and hop test scores as compared with those with single cruciate injuries.

Aims
Limited information exists on characteristics associated with methadone intoxication deaths. We therefore aimed to describe and explore differences in demographics, methadone prescription status, and drug concentrations in deaths with methadone, comparing intoxications with non-intoxications.
Methods
1092 forensic autopsy cases with methadone detected in blood analyzed at Oslo University Hospital (2004–2021) were linked to the Norwegian Cause of Death Registry and the Norwegian Prescribed Drug Registry. Cases were grouped as methadone intoxication (n = 638), other intoxication (n = 252), and non-intoxication (n = 202). Non-parametric tests (Chi-squared, Mann–Whitney U) and logistic regression analyses were used to compare demographic variables, prescription status, and drug concentrations (opioids, benzodiazepines, stimulants, ethanol) between intoxications and non-intoxications.
Results
Methadone prescription was less common in methadone intoxications and other intoxications than in non-intoxications (28.8% and 27.4% vs. 59.9%, respectively; P < 0.001 for both). Methadone intoxications were associated with younger age (adjusted odds ratio [aOR] 0.96; 95% confidence interval [CI] 0.95–0.98) and no recent methadone prescription (aOR 3.01; 95% CI 2.04–4.44) compared with non-intoxications. Adjusted analyses revealed no significant difference in methadone concentration between these groups. However, among cases with prescription, methadone intoxications were associated with higher methadone concentration (aOR 1.20; 95% CI 1.09–1.32). Intoxications also involved more psychoactive drugs and more frequent detections of benzodiazepines and stimulants.
Conclusion
Our results support that the determination of fatal methadone intoxications cannot be based solely on interpretation of postmortem methadone concentrations in blood but is dependent on information about prescription status and degree of tolerance.

Objective: This qualitative study aimed to investigate how patients with severe dental anxiety perceived cognitive behavioral therapy (CBT) delivered by interdisciplinary teams consisting of psychologists and dental personnel.
Material and methods: Twelve individual semi-structured interviews were conducted at the Oral Health Center of Expertise in Eastern Norway, using an exploratory qualitative design. The analysis was guided by Braun and Clarke’s thematic analysis. Self-determination theory (SDT) was applied as the theoretical and analytical framework.
Results: The qualitative analysis identified five themes shaping patients’ experiences with CBT and progress in managing dental treatment: Establishing positive relationships with the psychologist and dental personnel, Experiencing enhanced competence in managing one’s anxiety, Discovering the opportunity to use one’s own capacities, Identifying factors nurturing plans for future dental visits, and Experiencing additional effects on quality of life. These results underscore the importance of autonomy-supportive treatment that fulfills patients’ basic psychological needs (BPNs) for autonomy, competence, and relatedness, thereby fostering more autonomous motivation and desired behavioral changes.
Conclusions: The findings indicate that CBT provided by interdisciplinary teams may benefit from adopting SDT as a framework, integrating autonomy-supportive principles to fulfill patients’ BPNs. This approach may enhance CBT’s patient-centered effectiveness and potentially improve patients’ quality of life.

Objective: To investigate the occurrence and consequences of medical complications during inpatient rehabilitation of children with central nervous system (CNS) injury.
Design: A prospective observational study conducted at a tertiary rehabilitation centre.
Subjects: Participants aged 2 to 17 years with acquired CNS injury requiring multidisciplinary inpatient rehabilitation.
Methods: Data were collected from medical charts and questionnaires on admission and discharge, assessing predefined complications and associated impacts on the rehabilitation process.
Results: Of the 64 participants, 59 experienced a total of 183 instances of medical complications, comprising 19 distinct complication types. Most common were sleep disturbances, pain, -constipation, anaemia, infections, spasticity, and malnutrition. No significant correlation was observed between the occurrence of complications and age, gender, or diagnosis group. In total, 74% of the complications were found to have no impact on the rehabilitation process; however, malnutrition, infections, and spasticity could notably lead to interrupted or prolonged stays, altered rehabilitation goals, and worsened functional prognoses.
Conclusion: Medical complications frequently occur during inpatient rehabilitation. While many have no significant consequences, malnutrition, infections, and spasticity in particular warrant further attention clinically and in future research.

Many young women seek physiotherapy for long-term health disorders. To address the complexity of such conditions, physiotherapists are encouraged to use a biopsychosocial model of care that includes biological, emotional, and social aspects of patients’ lives. However, knowledge of how the complexity of these disorders is addressed in psychomotor physiotherapy practice remains limited. By exploring clinical encounters between psychomotor physiotherapists and young women with long-term health disorders, we aimed to generate knowledge about therapeutic practices that may inform improvements to treatment. We conducted a narrative analysis of 10 observed clinical encounters between psychomotor physiotherapists and young women in primary healthcare in Norway, alongside individual interviews with the participants involved. Our findings highlight how the patient’s life becomes interwoven with treatment through embodied therapist–patient collaboration and co-creation of meaning. Inspired by Mattingly’s concept of clinical emplotment, we developed the encounters into clinical plots. We present three illustrative examples. The first plot highlights how a patient’s way of standing is connected to how she relates to others. The second highlights how exploring a patient’s breathing is connected to her search for recognition of her illness experiences. The third plot highlights how subtle acts of care are connected to a patient’s former experiences of neglect and abuse. Overall, the findings emphasize the potential of inviting patients to actively participate in making meaning of physiotherapy and using their bodily experiences as a foundation for dialog. This may aid physiotherapists in addressing the complexities of long-term health disorders in clinical practice.

Denne kvalitative studien undersøker hvordan førstelinjetjenester i kommunal helse- og omsorgstjeneste i Norge håndterer «aging in place»-politikk (hjemmeboende eldre).

Background
Few instruments exist to capture interdisciplinary healthcare professionals' (HCPs) health literacy (HL) sensitivity, such as knowledge, skills, and attentive attitudes, to pursue HL when communicating with patients and their caregivers effectively.
Objective
This study aims to report on the psychometric properties of The Instrument of Health Literacy Competencies (IOHLC) using confirmatory factor analysis (CFA) procedures on data from a sample of HCPs working in specialist healthcare services in Norway.
Methods
The nine scales of the original Chinese IOHLC were translated and adapted from English into Norwegian using a forward-backward translation procedure and cognitive interviews. For the psychometric evaluation, a cross-sectional survey of 288 HCPs was conducted in a medium-sized hospital in Norway from July 2022 to September 2022. The statistical analysis included correlation analysis (Persons R), internal consistency (Cronbach's alpha), and CFA.
Results
The cognitive interviews led to changes in wording and the inclusion of an additional item in the survey. Nine one-factor models resulted in an acceptable fit (i.e., >0.6), comprising 36of the 40 items in the original version. Four items were excluded due to a low loading. In the final version, comprising 36 items, the alpha for internal consistency was 0.7 and above.
Conclusions
The CFA analysis yielded 36 items from the original IOHLC with an acceptable model fit. Further studies on the psychometric properties of the Norwegian version in other settings and with a larger sample are needed.

Urban blue and green spaces, such as streams and adjacent riparian areas, constitute important biodiversity refuges in highly anthropogenically -modified areas, but this depends on their ecological characteristics, preservation and management. Birds are useful indicators of ecosystem quality due to their relatively high trophic position. They may also disseminate and control pathogenic organisms, being good indicators for human and animal health risks. In our study, we used presence/absence data of avian species, and a wide environmental characterization of 102 urban streams across five European cities with a broad latitudinal range (Portugal, France, Italy, Belgium, and Norway), to evaluate how the impacts of urbanisation on ecosystem health impacted avian biodiversity. Benevento and Oslo differed most strongly from the remaining cities. The most representative bird species were different among the cities, which indicated that there is not a full homogenization of in European riparian zones. Yet, the number of insectivorous bird species was negatively associated with increasing urbanisation, including some of its proxies such as imperviousness, artificial light at night and population density. Streams with higher diversity of vegetation types and habitats tended to favour insectivorous birds like the Eurasian Wren Troglodytes troglodytes and the Common Chiffchaff Phylloscopus collybita. This is relevant in the context of One Health because this feeding guild may control the abundance of potential disease vector-mosquitoes. Management strategies should focus on preserving and rehabilitating riparian zones with structured vegetation and promoting habitat diversity through the maintenance of hedgerows and patches of natural green areas.

Background
Practical nursing skills are complex actions performed daily in nursing practice. Structured learning and repeated practice help students develop clinical competence. Clinical placements are pivotal in learning, with factors such as educational context, preceptor availability, and time constraints shaping students’ learning opportunities.
Objectives
To describe nursing students’ opportunities to perform practical skills during clinical placements.
Design and Setting
A quantitative cross-sectional study conducted in 2023 involved 706 nursing students from Denmark, Norway, and Sweden. Data were collected via a questionnaire and analyzed using descriptive statistics.
Results
Students reported substantial variations in the practical skills performed during clinical placements. Overall, the frequency of skills performance was low across all years and domains, including practical nursing skills concerning respiration, circulation, elimination, nutrition, activity, skin care, and medication. Supervision was generally available, though some skills lacked adequate preceptor involvement.
Conclusions
The study highlights disparities in nursing students’ opportunities to practice essential skills during clinical placements. The lack of practice across a wide range of skills may compromise the quality and thoroughness of nursing education, affecting the students’ professional preparedness.

Abstract Background Parkinson’s disease (PD) is a common neurological disease worldwide. Management of PD is complex, requiring person-centred care to address the physical, emotional, social, and economic impacts of the disease. The OPTIM-PARK intervention was developed to enhance the process of living with PD for people affected and their family carers (FCs) by designing inter- and multisectoral collaboration and incorporating a coordinator role in four European countries: Denmark, Norway, Spain, and the United Kingdom. The aim of this cross-national study was to evaluate the feasibility and acceptability of the OPTIM-PARK intervention in community settings. Methods A mixed-methods convergent design was applied, including quantitative analysis of registrations, outcome measures and an Acceptability scale, as well as qualitative analysis of Logs and Mapping of Resources, and semistructured interviews. The focus of the three months intervention was a PD coordinator who undertook a personalized needs assessment and provided information and support, strengthened multi- and intersectoral collaboration, and optimized access to and use of community resources and other third sector support systems. Results A total of 130 participants were included in the intervention, 72 people with PD and 58 of their FCs. The intervention was typically provided in three consultations. The PD coordinator focus was on individualized support through active listening in the delivery of healthcare interventions. Inclusion of FCs were highly valued. Completion of the outcome measures was feasible, 105 (81%) participants were able to complete outcomes at both time points with <5% missing data. Most participants (92%) found it easy to answer the questionnaires, the use of digital tools for data collection was found beneficial if the participant were comfortable with the digital format. Conclusions To the best of our knowledge this is the first cross-national European study exploring the feasibility of a PD coordinator focussed support to enhance the process of living with PD. The findings suggest that the OPTIM-PARK intervention was feasible and well-received in a community setting across different countries, suggesting potential for wider application and further evaluation in a larger-scale trial.

Purpose
Health literacy influences surgical outcomes by affecting patients’ ability to understand information, manage their health and navigate the healthcare system. Yet, little is known about health literacy in patients with colorectal liver metastases. This study assessed health literacy across different domains and explored associations with sociodemographic and clinical factors aiming to identify knowledge gaps within this population.
Design
Cross-sectional.
Methods
A total of 122 patients with colorectal liver metastases were recruited between December 2021 and December 2024. Health literacy was assessed using the Health Literacy Questionnaire. Descriptive statistics were used to characterize the sample and to present the overall health literacy profiles. Associations between health literacy and selected sociodemographic and clinical variables were examined using, both univariate and multivariate linear regression analyses.
Results
Participants reported the greatest challenges in the Health Literacy Questionnaire domains “ability to find good health information” (mean score 3.98 ± 0.56, range 1-5) and “appraisal of health information” (mean score 2.63 ± 0.55, range 1-4). Lower health literacy scores were significantly associated with male gender, lower educational attainment, living alone, and the presence of comorbidity.
Conclusion
This study provides novel insights into the health literacy of patients with colorectal liver metastases and highlights the importance of identifying and addressing health literacy challenges in clinical care. Tailored communication and support may enhance treatment outcomes, particularly to vulnerable patient groups. Further research is needed to explore how health literacy impacts decision-making and treatment adherence in complex cancer care.

Chronic obstructive pulmonary disease (COPD) is commonly regarded as an ambulatory care-sensitive condition (ACSC), for which targeted outpatient and primary care can improve patients' outcomes and reduce their risk of hospitalization. Yet little is known about how COPD patients' use of such services evolves in real-life settings. Using national health registry data from Norway (2008–2020), linked to demographic and socioeconomic registers, we explore how COPD patients' combined use of primary and specialist services evolves in the first four years after an acute respiratory hospital episode, and assess whether these patterns align with clinical guidelines and vary by sociodemographic characteristics.
A two-stage clustering approach was employed to identify individuals with similar care trajectories. The 13 057 patients were first grouped by the number of respiratory hospital episodes after the index episode (0, 1–2, or 3+), then K-medoid clustering was performed within each group to identify similar patterns of respiratory-related outpatient and primary health service utilization. Four distinct care trajectories emerged within each group, with similar combination patterns: (1) predominantly GP services (most common), (2) GP and outpatient services, (3) GP, out-of-hours primary care and infrequent outpatient services, and (4) frequent physiotherapist, GP and outpatient services (rarest). Notably, groups 2 and 4 had higher income and education than groups 1 and 3, and many patients do not follow care trajectories according to current guidelines for COPD treatment. Understanding the mechanisms that explain these variations in health services use, and their implications for care quality and equity, is essential for improving COPD management in universal healthcare systems.

Purpose
Fear of pain and anxiety are common among children undergoing needle procedures. Actively involving the child in communication can help reduce their distress and foster a stronger nurse–child relationship. Positive emotions have the potential to strengthen the patient–nurse connection and may also play an important role in managing children’s distress, though further research is needed to fully understand their impact. This study aimed to identify expressions of positive emotions in procedural communication between children, parents, and nurses, using the seven categories of the Positive Emotion Communication (PEC) coding system.
Design
A secondary analysis of nine previously collected video recordings exploring fear and pain during needle injections among children recently diagnosed with rheumatic diseases. Positive emotions were identified using the PEC coding system, and the findings were analyzed using descriptive statistics.
Results
We identified 328 expressions of positive emotions (60% from nurses, 30.5% from parents and 9.5% from children). All participants widely used the category Positive Focus, while nurses and parents favored the category Praise or Support. Children used the category Humor more frequently than other participants. Communication was mainly directed at children, with parents also receiving encouragement.
Conclusion
Nurses frequently used positive utterances in their communication with children and parents during this challenging healthcare procedure. The PEC coding system proved useful for identifying positive emotions in pediatric communication research; however, it lacks the ability to capture nonverbal communication and responses.
Clinical Implications
Using positive emotions in medical conversations with children has significant potential to enhance their healthcare experience. However, further research is needed to refine the PEC coding system to better capture nonverbal communication and responses, ensuring a more comprehensive understanding of positive communication in pediatric care.

Objective: To map dental health service utilization (DSU) and associated factors among older adults receiving home care services (HCS) in south-eastern Norway, using Andersen’s model.
Materials and methods: In this cross-sectional study of older adults receiving HCS aged ≥ 65 years, data on oral health practices, general health, and predisposing, enabling, and need-based factors were collected via interviewer-administered questionnaire.
Results: Of 116 participants (mean age 83, 53.6% female), 67.2% reported annual DSU, while 24.1% used dental health services only for acute problems. Although 95.6% were entitled to free dental care in the public dental service (PDS), 66 were aware of this, and 56.5% made use of their rights. Awareness of entitlements was associated with more frequent PDS use. A higher self-reported number of teeth and using private dental services were also linked to higher DSU.
Conclusions: The present study showed underutilization of dental health services among older adults receiving HCS and unawareness of entitlement of their rights in PDS. However, higher self-reported number of teeth and use of private dental services were linked to more frequent DSU. To address underuse, the PDS should improve awareness of entitlements, enhance information delivery, and further investigate reasons for non-utilization.

Abstract:
Objectives This study aims to examine preoperative sociodemographic, clinical, and psychological factors associated with acute worst pain intensity following open carpal tunnel release (CTR).
Methods:
This prospective observational study included 83 patients between 18 and 79 years undergoing primary outpatient CTR. All patients filled in consent forms and a questionnaire that assessed demographics, comorbidities, pain and function, sleep quality and pain catastrophizing. The primary outcome was worst pain intensity during the first three postoperative days. Linear regression analyses were done to estimate associations between age, sex, education, comorbidity, pain catastrophizing and sleep quality, and worst postoperative pain intensity.
Results:
The patients mean age was 60 years and most were women (64 %). Patients rated their worst preoperative pain to 4.5 on a scale from 0 to 10, and worst pain intensity of 5.3 during the first three postoperative days. Postoperatively, 47 % of the patients experienced moderate pain and 32, 5 % reported severe pain in the three first days. In the univariable analyses, poorer sleep quality, more pain catastrophizing, more PRWHE hand pain and disability, and higher preoperative pain were associated with more severe postoperative pain. In the multivariable analysis, only higher level of pain catastrophizing remained associated with more severe postoperative pain.
Conclusion:
Higher preoperative pain catastrophizing was independently associated with higher postoperative pain intensity after CTR. This factor may be useful as part of preoperative screening to identify patients at higher risk for more severe postoperative pain. REC approval # 2018/1562.

Abstract:
Background The use of eHealth resources is rapidly expanding within the healthcare services, representing a significant digital transformation. The success of this transformation depends on healthcare providers’ motivation and eHealth literacy capacity. The eHealth Literacy Questionnaire has been modified to assess healthcare providers’ own perception of eHealth literacy in their interactions with patients (Staff eHealth Literacy Questionnaire [SeHLQ]). This study aimed to investigate the reliability (internal consistency), discriminant validity, and construct validity (structural validity) of the SeHLQ in a self-report format administered to nurses employed in Norwegian hospitals.
Methods:
A cross-sectional survey was administered to 260 hospital-based registered nurses educated at the baccalaureate level or higher across various hospitals, university colleges, and universities in Norway. Discriminant validity was investigated by assessing the inter-domain correlation (Pearson r) between the seven SeHLQ domains. The structural validity of the predefined seven dimensions of the Norwegian SeHLQ was examined using confirmatory factor analysis (CFA), and the internal consistency was evaluated using Cronbach’s alpha.
Results:
Participants had a mean age of 36 years (range 22–65 years), with the majority being female. The CFA showed that seven one-factor models were acceptable after one to two model adjustments, demonstrating an acceptable structural validity. The discriminant validity between the seven dimensions in the SeHLQ was acceptable. Cronbach’s alpha for the 7 domains ranged from 0.71 to 0.85, indicating good internal consistency.
Conclusions:
The Norwegian version of the SeHLQ demonstrates acceptable structural validity, discriminant validity, and internal consistency. The modified version of the eHealth Literacy Questionnaire has the potential to be a useful measurement for various purposes.

Objective
To systematically map existing evidence on health literacy among people with non-communicable diseases (NCDs) in primary healthcare.
Methods
An interdisciplinary research group conducted a scoping review following the Arksey and O’Malley framework, refined by Levac and Peters, and reported according to PRISMA-ScR. The review was registered in OSF and applied the Population–Concept–Context framework to define inclusion criteria. Searches were conducted in Medline, APA PsycInfo, Scopus, and Cochrane for studies published between 2000–2025. Peer-reviewed studies were screened in Rayyan, and data were charted and thematically analyzed through team-based consensus.
Results
The search identified 7952 studies, of which 106 studies met the inclusion criteria. Participants’ mean age was 59 years; 56% were female. Studies originated from 29 countries, mainly the USA, China, and Canada, with an increase after 2013. Three main themes were identified: (1) health literacy, social status, and ethnicity, (2) health literacy, disease, and health outcomes, and (3) health literacy, NCD knowledge, and self-management. Many studies reported associations between health literacy, and self-management, quality of life, and some clinical outcomes, but findings across designs and outcomes were heterogeneous and not consistently positive.
Conclusions
Improved communication, greater cultural competence, and tailored health literacy interventions are associated with better self-management and disease outcomes in non-communicable diseases, but most evidence is associative rather than causal, intervention effects are mixed, and research on health literacy experiences remains limited.
Practice Implications
More rigorously designed intervention studies and broader conceptualization and measurement of health literacy with multidimensional instruments are needed to strengthen the evidence and clarify how health literacy relates to social status, disease outcomes, and self-management in primary healthcare. Training healthcare professionals to assess and address health literacy is essential to promote equity and effective patient engagement.

Background
Recruiting participants for clinical trials in primary care settings remains challenging, not least for interventions targeting individuals with chronic obstructive pulmonary disease (COPD). Clinical outcomes and the effects of interventions receive considerable attention compared to factors influencing recruitment success. Identifying and addressing possible barriers and facilitators is essential for optimizing recruitment strategies and ensuring the feasibility of trials.
Aim
This study aimed to assess uncertainties regarding recruitment for a digitally based Guided Self-Determination (GSD) follow-up program for people with COPD, prior to a fully powered cluster randomized controlled trial (cRCT), and to explore characteristics of primary care practices and patients willing to be included in the study.
Methods
The intervention was a 9-month digitally based follow-up program performed with the GSD counseling method, conducted by nurses in primary care practices. Eligible primary care practices were invited to participate, and participants were recruited through their respective practices. The primary feasibility outcome was recruitment, assessed at both the practice (cluster) and patient (participants) level. Eligible participants were people between 35 and 80 years diagnosed with COPD. Baseline characteristics included demographic data, COPD Assessment Test, Dyspnea-12, WHO-5 Well-being Index, and Health Education Impact Questionnaire domains. Progression criteria Go , Amend , and Stop were applied to assess feasibility.
Results
Recruitment occurred from May to December 2024. Of 16 eligible practices, 12 (75%) responded, of which ten (83%) agreed to participate, corresponding to 63% of the eligible practices. A total of 64 participants with COPD were recruited, 28 in the intervention and 36 in the control clusters. The overall median recruitment rate was 6.5 participants per cluster, 7 in the intervention clusters and 6 in the control clusters. One of three progression criteria, cluster recruitment, met the target Go, while cluster sizes and participant recruitment met the amended criteria.
Conclusion
Recruitment was deemed feasible. However, successful cluster-level recruitment does not necessarily ensure effective patient enrollment, highlighting the necessity for amendments before a fully powered cRCT. Future research should integrate qualitative data obtained from patients and primary care practitioners to enhance the understanding of recruitment strategies.

Background
Cancer presents challenges for patients due to its complexity and treatment demands, resulting in a low quality of life. Patients must manage amounts of information, and make care decisions. Health Literacy (HL) may be key to managing these challenges. Existing research suggests an association between health-related quality of life (HRQOL) and HL in cancer patients. However, there remains a limited understanding of how dimensions of HRQOL associate with aspects of HL.
Aims
Investigate associations between domains of HRQOL and multidimensional HL in cancer patients while controlling for demographic and clinical variables.
Methods
A cross‑sectional study was conducted at a hospital in Oslo, Norway (2020 to 2022). Patient‑reported measures included HLQ (HL), FACT‑G (HRQOL), EORTC‑OH15 (oral QOL), and sociodemographic data. Clinical data were obtained from questionnaires and medical records. Multivariable linear regression analyses were performed
Results
We included 54 men and 63 women with cancer at a hospital outpatient clinic (mean age 61 years). Multivariable analysis on the association between HRQOL and HL, controlling for demographic and clinical variables, was performed. Significant associations were found between multiple HL domains, younger age, and fewer comorbidities (St. β ranged −0.20 to −0.39, p ranged .04 to <.001), explaining 6–20% of the variance. No associations with gender, living arrangements, education level, or treatment setting. Associations were observed between higher social well‑being scores and higher HL scores across most HLQ domains; higher emotional, physical, and functional well‑being scores were also associated with higher scores in several HLQ domains. All HLQ domains except appraisal of health information (HLQ5) were associated with HRQOL, HLQ7 (navigation) showed most associations (St. β ranged 0.19–0.53, p ranged 0.04 to <.001, explaining 11–46% of the variance). No associations between oral HRQOL and HL.
Discussion
This study reveals insights into how aspects of HRQOL associate with different HL dimensions in cancer patients. Higher scores on multidimensional HL—especially social support, patient–provider communication, information access, and navigation—relates to better social, emotional, physical, and functional QOL. The article highlights the need for clinician engagement, information appraisal, and navigation support to improve outcomes and guide HL interventions, and underscores the need to measure both concepts multidimensionally.
Plain Language Summary
Cancer can create many problems for people and can lower their quality of life. Patients often have to learn a lot about cancer, and manage their treatment and symptoms. They also have the stress of having cancer. This requires health literacy, which means being able to understand health information. Our study looked at how health literacy relates to quality of life in cancer patients. We found that people with higher health literacy tended to have better quality of life in many areas.

Introduction
Guidelines recommend large decompressive craniectomies (DC) in traumatic brain injury (TBI), yet the optimal size remains debated. Real-world practice often differs from guideline recommendations and generalizability of prior evidence to a broader TBI population is uncertain.
Research question
What is the comparative effectiveness of DC size on 12-month functional outcome in TBI?
Material and methods
We selected participants enrolled in the CENTER-TBI diagnosed with TBI who received a hemicraniectomy. Effect of DC size on functional outcome was evaluated with random-effects logistic regression, associating center case-mix adjusted DC sizes to GOSE. Center preference was quantified with the median odds ratio (MOR).
Results
Among 4509 patients enrolled in CENTER-TBI, 295 underwent a hemicraniectomy. DC size varied from 37 cm2 to 165cm2 (IQR 96 cm2-123cm2, ellipsoid calculation), with a two-times higher probability of receiving a 27 cm2 larger (IQR increase) DC for a similar patient in one center versus another random center (adjusted MOR for DC size 1·7). Only 4 patients received a DC ≥ 12 × 15 cm (cm) or 15 cm in diameter, while 0 patients received a DC ≤ 6 × 8 cm. Larger DC size was not associated with more favorable 12-month GOSE scores (aOR 0.73 for 27 cm2 increase in DC size, 95%CI 0.47-1.1).
Discussion and conclusion
DC size varied widely across European centers. Recommended DC sizes were rarely reached, as were very small DC sizes. Larger versus smaller DC was associated with similar outcomes, however heterogeneity in DC indication may have attenuated observable treatment effects. Neurosurgeons may continue to prefer larger over smaller decompressions.

Isolating the effects of outpatient care on hospitalisation in observational data is challenging. We estimate the
effects of factors believed to influence hospital admissions in patients with COPD. The factors were related to
outpatient care and social factors, motivated by recommendations in COPD guidelines and the literature. We
applied linked national and municipal registry data on 25,369 COPD patients in Oslo and Trondheim 2008–2019.
Yearly frequency and type of healthcare contacts (respiratory and non-respiratory, across providers), together with
comorbidities, were used as proxies for patients’ health status. We constructed samples matched on the full set of
proxy variables to estimate the effect of each factor on future COPD hospital admissions. Overall, 9.8% of follow
up years had at least one COPD hospital admission. Findings indicated beneficial effects of several of the factors.
Care interaction between general practitioner (GP) and other health providers, follow-up by GP within 30 days
after COPD hospital discharge and counselling/psychotherapy by GP showed significant reductions of 2.9, 4.9
and 1.4% points in the rate of next-year COPD hospital admissions, respectively. The effects are noteworthy when
extrapolated to the entire Norwegian COPD population. A reduction of 1% point could result in up to 1,500 fewer
patients experiencing one or more COPD hospital admissions during a year. In the absence of detailed clinical
information, creating samples that are multidimensionally matched on health service use and comorbidities as
pragmatic proxies for health status could be a viable approach for estimating the effects of other factors.

Background: Neuropsychiatric symptoms (NPS) are highly prevalent in nursing home residents. The main aims of this study were to examine whether music-based care (MBC) had sustained effects on NPS in nursing home residents with dementia and chronic pain and to describe the distribution of NPS.
Methods: A secondary analysis of a cluster-randomised controlled trial with intervention and control groups was performed. The 8-week MBC intervention included daily individualised prerecorded music integration. A large sample of nursing home residents with dementia and chronic pain in 12 nursing homes in Norway were screened for pain and dementia by experts and included in the study. The Neuropsychiatric Inventory Nursing Home Edition (NPI-NH) was completed for the residents at pre-test and after the 8-week MBC intervention. Descriptive statistics were used to characterise the sample, and multilevel mixed model analysis assessed the difference in change in NPI-NH scores before and after the intervention between the groups.
Results: The sample (n = 232) had a mean age of 86 years (SD 8.8), with 71% being female. Overall, 41%, 38% and 21% had severe, moderate or mild dementia, respectively, while 77% had moderate pain and 23% severe pain. Over two-thirds (68%) of the sample had at least one clinically important (≥ 4) NPI-NH symptom at pre-test, with mean NPI-NH total score of 19 (range 0–89). The most commonly identified NPI-NH subcategories were agitation (44%), affective symptoms (34%) and psychosis (25%) at pre-test. There was no significant difference in changes either comparing the NPI-NH total score (p = 0.396) (confidence interval: CI [–2.6 to 6.6]) between the intervention group (n = 108) and the control group (n = 124) or comparing any of the three NPI subcategories: psychosis (p = 0.203) (CI [–0.5 to 2.3]), agitation (p = 0.830) (CI [–2.3 to 1.8]) or affective symptoms (p = 0.447) (CI [–0.9 to 2.1]).
Conclusions: No statistically significant sustained effect of the MBC intervention on NPI symptoms was found among residents with dementia and chronic pain. Future studies should include measurement points closer to the intervention to evaluate short-term effects of MBC.

Background
Thermal ablation of colorectal liver metastases (CRLM) is traditionally reserved for patients who are not candidates for surgical resection, but it is increasingly applied as a parenchyma-sparing alternative. This study investigates long-term outcomes following laparoscopic resection and thermal ablation in a parenchyma-sparing treatment strategy.
Methods
Cohort study of patients with CRLM treated with laparoscopic resection or thermal ablation at Oslo University Hospital between 2007 and 2020, identified from two prospectively maintained databases. The primary endpoint was the average treatment effect on survival, estimated using inverse probability weighting to adjust for prognostic variables. Secondary endpoints included overall survival and progression-free survival. In case of recurrence of liver metastases after the study treatment, both ablation and resection were used.
Results
A total of 207 ablation procedures and 499 resection procedures were eligible for analysis. Five-year overall survival was 55.2% in the ablation group and 55.1% in the resection group. The estimated average treatment effect of ablation versus resection was − 0.34 months (95% confidence interval − 7.29 to 6.57; P = 0.919), indicating that treating all patients with ablation would yield an average survival of 0.34 months shorter than treating all patients with resection.
Conclusions
In a parenchymal-sparing paradigm, thermal ablation yields long-term survival comparable to resection for CRLM.

Objective
Explore treatment trajectories over a two-year follow-up among patients with musculoskeletal disorders (MSDs) in Norway and investigate the associations with education and country of origin.
Design
Register-based cohort study.
Settings
Data were obtained from three national registers, the Norwegian Control and Payment of Health Reimbursements Database (KUHR), the Norwegian Patient Registry (NPR) and Statistics Norway (SSB).
Subjects
Patients diagnosed with MSDs in 2015 in primary healthcare according to the International Classification of Primary Care, second edition (ICPC-2).
Main outcome measures
Treatment trajectory classes of healthcare use over a 2-years follow-up were generated using group-based multi-trajectory modelling, for all patients registered with MSDs and three common diagnoses: spine pain, osteoarthritis and fibromyalgia.
Results
From the analysis, four treatment trajectory classes of healthcare use were identified for MSD patients overall; low (75%), stable (10%), descending (9%) and high use (5%). The pattern was almost similar for the three diagnostic groups. High use was primarily caused by consistent high use of physiotherapy. For MSD patients overall, low education increased the probability of being in the low and stable use classes, while high education increased the probability of being in the descending use class. Patients with Norway as country of origin, were less likely to be in the class with low use and more likely to be in the class with descending use.
Conclusion
Using nationwide register data and group-based multi-trajectory modelling show that about 75% of all patients with MSD, consulting primary healthcare in Norway, were classified as low users of healthcare services and 5% were classified as high users. Overall, both education and country of origin were associated with the treatment trajectory classes. However, high use was not associated with neither education nor country of origin.

Background: Bullying may have a considerable negative impact on health and well-being. Nevertheless, there is a lack of studies investigating exposure to bullying in the general population. Thus, we aimed to assess the prevalence of lifetime exposure to bullying in the adult Norwegian population and to compare health-related quality of life (HRQoL), and physical and psychological health problems between bullied and non-bullied.Methods: We measured lifetime exposure to bullying in the general Norwegian population using a self-report questionnaire. The study was part of a cross-sectional national health survey (n = 1733) conducted in 2014–2015, to which a probability sample of people in the general population was recruited. Reports on socio-demographic data, HRQoL and a wide range of mental and somatic health problems provided the basis for a comparison of those who were bullied with those who were not. Chi square tests and logistic regression analyses were used to assess associations with lifetime exposure to bullying. Results: Across age groups, 28.2% reported exposure to bullying during their lifespan. Adjusted for other variables, the odds of bullying were higher in younger age groups, for those who lived as singles, and for those who were not working or in education. Respondents who had been bullied reported more mental problems such as depression, anxiety, insomnia, eating disorder, psychosis, self-harm, and suicide attempts, and they had lower HRQoL. They also reported more chronic pain, fibromyalgia, rheumatoid arthritis, diabetes mellitus, obesity, and musculoskeletal-, respiratory- and gastrointestinal diseases than people who had not been exposed to bullying. Conclusion: Bullying is common in the general Norwegian population, where more than a quarter of respondents had experienced exposure to bullying during their lifespan. Substantial more mental and somatic health problems among those exposed to bullying require a broad effort against bullying in central arenas in society.

Nurses’ Experiences with Visitor Restrictions in Intensive Care Wards for Adults During the COVID-19 Pandemic: A Qualitative Systematic Review
Background: The COVID-19 pandemic emerged in March 2020 and led to strict visitor restrictions in intensive care units. In contrast, guidelines highlighted the importance of involving relatives in intensive care units. Nurses had to implement this new practice.
Aim: This qualitative systematic review explored the experiences of nurses in adult intensive care units during the COVID-19 pandemic regarding visitor restrictions.
Methods: This qualitative systematic literature study employed the framework of Bettany-Saltikov and McSherry for systematic reviews and was reported according to the Enhancing Transparency in Reporting the Synthesis of Qualitative Research checklist. A systematic literature search was performed in Cinahl, MEDLINE, and Embase for the period 2019–2025. Studies reporting qualitative data collected using interview or free text answers in survey about nurses in adult intensive care units’ experiences with visitor restrictions during COVID-19 pandemic were included. Pairs of authors individually performed study selection, appraised methodological quality and extracted data. Data were analyzed using qualitative content analysis.
Results: Ten studies were included. Three themes were identified: “Bridging the gap between the patients and their families,” “An additional emotional and mental burden” and “Being a gatekeeper led to moral distress.” Nurses maintained the relationship between patients and their relatives by finding new creative ways to provide care, such as using mobile phones and tablets for video calls. The fact that relatives were not allowed to visit dying patients became an additional emotional and mental burden. Denying relatives the opportunity to visit and being loyal to the visitation guidelines, along with the fear that relatives might bring infection, led to moral distress. Nurses also made independent assessments and adjustments regarding the visitor restrictions.
Conclusion: The visitor restrictions during the COVID-19 pandemic posed many challenges for nurses in adult intensive care units. Many nurses experienced high emotional burden and moral distress during the pandemic.

Introduction
Post-stroke fatigue (PSF) is common and impacts stroke rehabilitation. Dopaminergic treatment may have beneficial effects on PSF. This study investigated whether levodopa, compared with placebo, was associated with a lower frequency or severity of PSF during in-hospital rehabilitation.
Patients and methods
Enhancement of Stroke Rehabilitation with Levodopa (ESTREL)-Fatigue was an exploratory analysis of secondary outcome data obtained in the multicentre, randomised, placebo-controlled ESTREL trial. Participants with acute stroke received levodopa 100 mg/carbidopa 25 mg or placebo 3 times daily for 39 days to enhance motor recovery. Participants who (i) reported fatigue at 5 weeks and who (ii) took at least 80% of the study medication were included in ESTREL-Fatigue. No adjustments for confounding were made. The primary endpoint was the presence of PSF at 5 weeks, defined as a T-score of ≥ 55 on the Patient-Reported Outcomes Measurement Information System (PROMIS) Fatigue-Short-form-4a. As secondary endpoints, T-score cutoffs of ≥ 60 (moderate fatigue) and ≥ 70 (severe fatigue) were used. Binary logistic regression was used to compare PSF at 5 weeks between treatment groups. Results are presented as odds ratios (ORs) with 95% CI.
Results
A total of 456 of 505 (90.3%) participants were included (levodopa/placebo 235/221, median age 73 years, 41% female). Post-stroke fatigue at 5 weeks was present in 63/235 (26.8%) levodopa-treated participants and in 65/221 (29.4%) placebo-treated participants (OR: 0.88; 95% CI, 0.58–1.32; risk ratio 0.91; risk difference − 2.6%). For cutoffs of ≥ 60 and ≥ 70, ORs were 0.78 (95% CI, 0.43–1.41) and 0.8 (95% CI, 0.25–2.44), respectively. A sensitivity analysis as per intention-to-treat with all 610 randomised ESTREL participants also showed no significant difference in fatigue presence between levodopa and placebo groups (OR: 0.95; 95% CI, 0.65–1.39) and a sensitivity analysis using a mixed-effects logistic regression showed no evidence of centre-related clustering.
Conclusion
In ESTREL-Fatigue, levodopa, compared to placebo, was not associated with less PSF during in-hospital rehabilitation.

Objective
Fibromyalgia is a chronic illness that is poorly understood by healthcare professionals (HCPs), and HCPs may express uncertainty due to limited available knowledge. Our objective was to explore what HCPs tell about their present confidence in delivering self-management programs to patients with fibromyalgia.
Methods
A narrative research approach was applied, and interviews were conducted with nine HCPs from various professional backgrounds. The HCPs were experienced in the management of fibromyalgia, and one year earlier, they had revised their self-management program to ascertain person-centredness. The interview data were subjected to a narrative analysis of what and how they storied their clinical experiences of delivering the programs.
Results
The new confidence of the HCPs was grounded in a cohesive, collective platform with an explicit purpose linked to a defined content and an explorative educational approach. This was articulated by a shared clinical mindline emphasizing stress, patients’ experiences of life stress, and facilitating patients to find solutions to repair their disrupted daily lives. The cohesive, collective platform fostered mutual respect and shared responsibility for the wholeness of the program. Further the collective as well as discipline-specific clinical mindlines and skills were bolstered through the team’s engagement and reflective learning on episodes in practice.
Conclusions
The findings indicate that clinical confidence arises from a shared clinical mindline that articulates a theoretical rationale linked to the actions required and how to deliver the program, together with interdisciplinary collaboration and ongoing reflective practice.

Implementing Digital Antenatal Care Registries at Public Health Care Facilities in Ethiopia and Uganda: A Comparative Study of Barriers and Enablers.
Fulltekst i vitenarkiv

Background: Digital registries can support clinical decision-making, longitudinal tracking of patient care and resource allocation decisions. Yet, questions around how to implement registries remain unsolved. We identified barriers, enablers and implementation strategies for digital antenatal care registries (ANC eRegistry) in public primary health facilities in Ethiopia and Uganda.
Methods: We utilized data from research projects on ANC eRegistries in sub/urban Ethiopia (Addis Ababa, Adama, Harar, Jimma) and Uganda (Mukono). Interviews (n=35) and focus groups (n=18, 90 persons) with ANC providers, health facility managers, and sub-national and national leaders, identified through purposive sampling were English-translated and coded (NVivo) through deductive thematic analysis mapped onto Consolidated Framework for Implementation Research domains, identifying barriers and enablers in common and unique to each country. CFIR-ERIC (Expert Recommendations for Implementing Change) tool matched barriers and enablers to implementation strategies.
Results: Among common barriers were financial constraints exacerbating infrastructure insufficiencies (computers, servers, unreliable networks), and heavy provider workloads constraining time towards meticulous data entry. Enablers in both settings were high motivation to switch to digital records, perceived ease of use of the eRegistry, simplified tasks and improved workflows, and existing peer-learning networks. Unique barriers in Ethiopia were lack of interoperability with existing health information systems and providers prioritizing clinical care over data entry: strong leadership commitment to digitalization was an enabler. In Uganda, limited proactive leadership and difficulties learning new systems were unique barriers; computer-literate young providers was an enabler. Six common implementation strategies were: Conducting ongoing training, preparing champions, formal implementation blueprints, provider performance data, coalition building, and conducting cyclical small change tests.
Conclusions: Despite different contexts, identifying implementation determinants and strategies in common and unique to different contexts can facilitate adoption of digital registries. Health systems should capitalize on provider motivation to digitalize while ensuring training, resources and leadership.

Background: Older adults (≥65 years) receiving municipal home health care services (HHCS) in Norway face substantial oral‑health challenges. One important reason is that HHCS and public dental services (PDS), organized at the county level, operate in separate organizational structures and use different digital systems. Currently, no integrated digital communication solution exists to support collaboration, and there is a clear need for improved cross‑sector coordination. Recent cost‑of‑illness evidence shows that poor oral health in this population entails considerable economic burden, including direct costs related to oral disease, indirect costs from increased risk of systemic illness, and productivity losses among caregivers.
SYNCORAL is a digital platform embedded in electronic patient records designed to streamline patient identification, consent, and information exchange between HHCS and PDS. By enabling better coordination and earlier intervention, SYNCORAL may help shift care from costly acute treatment toward prevention. However, the key effects and costs of SYNCORAL remain uncertain. A Value of Information (VOI) analysis is therefore needed to assess whether additional evidence is required before large‑scale implementation.
Aim: The aim of this VOI analysis is to quantify remaining decision‑critical uncertainty in the expected effects and costs of SYNCORAL, and to assess whether collecting additional evidence would be worthwhile.
Method: We developed a Markov model (see Figure 2) and performed a probabilistic sensitivity analysis (PSA) using Monte Carlo simulation with 10 000 iterations to quantify parameter uncertainty. The model applied a lifetime time horizon, following older adults receiving HHCS from the age of 65 years old to death. Two separate analyses were conducted for adults aged 65–79 years and 80+ years. Data were obtained from epidemiological studies, national health registries, reimbursement databases, and expert assessments.
Preliminary results: Substantial uncertainty across several key parameters is already evident while the VOI analysis remains in progress. Figure 3 shows the preliminary incremental cost‑effectiveness scatterplot of SYNCORAL versus current practice. Each point is one PSA simulation; the dashed line indicates a 275 000 NOK/QALY threshold, with points below the line considered cost‑effective. In these preliminary results, SYNCORAL appears cost‑effective in around 60% of simulations at this threshold.
Further research: Our planned VOI work will estimate Expected Value of Sampling Information (EVSI) to quantify the value of reducing the uncertainty through new data, and Expected Net Benefit of Sampling (ENBS) to assess whether additional evidence would justify the cost of further research

Background & Objectives: The International Cerebral Palsy DAG Consortium recently developed a comprehensive directed acyclic graph (DAG) mapping causal pathways to cerebral palsy (CP). Structural analysis of the CP-DAG can help rank risk factors by their relative contribution to CP. This information is critical for researchers and clinicians to prioritize prevention efforts. The objective is therefore to prioritize modifiable causal factors in the CP-DAG by ranking their capacity to disrupt causal pathways using a multi-metric network analysis framework.
Study participants & setting: This study uses the network structure of the CP-DAG, a causal directed acyclic graph model of the causal factors and pathways to CP.
Materials & Methods: Adapting a framework originally pioneered by NASA for its Human System Risk Network, we computed three metrics for each causal factor: out-degree (immediate causal reach), inverse α-centrality (downstream systemic influence), and edge connectivity (minimum edges required to disconnect the cause from the outcome). We scaled and combined these measures using principal component analysis, deriving two component scores for each causal factor, termed systemic importance and mechanistic complexity.
Results: We evaluated 7 unique causal factors. Five factors exhibited slightly below-average systemic importance (as much as 1 SD below the mean), but slightly above-average in mechanistic complexity (up to 0.5 SD above the mean). Two factors stood out: socio-economic status (SES) and social determinants of health (SDH). SES was average on mechanistic complexity but high on systemic importance (3.7 SD above the mean), while SDH was average on systemic importance and moderately low on mechanistic complexity (1 SD below average).
Conclusions: Among the independent and modifiable causes of CP examined here, several are of roughly equal systemic importance and mechanistic complexity, signaling they are of comparatively low influence in the causal pathways of CP. SDH, though often touted as important overall, shows little promise beyond its direct influence on SES. To intervene on SES, specific components of this greater concept should be mapped explicitly into the network.
Overall, network analysis can aid the prioritisation of opportunities to interrupt pathways to CP, both through clinical care and the prioritization of new research.

Economic Burden of Reduced Oral Health Among Older Adults Receiving Home Health Care Services.
Fulltekst i vitenarkiv

Introduction:
Older adults (65+) receiving home health care services (HHCS) represent about 2% of
Norway’s population. By 2050, the population aged 80+ is projected to double, which is
expected to increase the demand for HHCS. Old age is associated with a high prevalence of
oral diseases and poor oral health, which in turn is associated with elevated risks of systemic
conditions. Despite these implications, the societal economic cost of oral health among
older adults receiving HHCS is insufficiently documented.
This study aimed to estimate current and future societal costs of reduced oral health
among older adults receiving HHCS in Norway.
Method:
A cost-of-illness analysis was conducted. Direct costs included routine examinations and
dental treatment, while indirect costs covered caregiver productivity losses and systemic
diseases associated with reduced oral health. Data were obtained from epidemiological
studies, national health registers, reimbursement registries, and expert elicitation.
Results/conclusion:
Older adults receiving HHCS accounted for 4.8% of dental health expenditures (NOK 1.1
billion) in 2026. In 2026, adults aged 80 years and more made up about 60% of total costs (NOK 919 million), compared with 40% for those aged 65–79 years (NOK 619 million). By 2060, this distribution is projected to shift to roughly 70% (NOK 2.3 billion) versus 30% (NOK 1 billion)
Poor oral health among older adults receiving HHCS constitutes a substantial economic
burden.

In this thesis, healthcare personnel’s well-being and mental health were researched in combination with health literacy sensitivity in clinical practice. Health literacy sensitivity is the knowledge, skills, and attitudes required to effectively meet patients’ health literacy needs (health literacy: how individuals access, understand, and use information in ways that promote and maintain good health). Health literacy-sensitive care plays a critical role in patients’ long-term health outcomes, care quality, and patient safety.
The overall aim of this thesis was to explore experiences and potential associations among healthcare personnel’s health literacy sensitivity, work-related factors, and mental well-being in a general hospital setting, and to use this knowledge to develop and evaluate a health literacy intervention.
A multi-method sequential design was applied. Paper 1 used focus group interviews with interdisciplinary healthcare personnel, analyzed using thematic analysis. Paper 2 was a cross-sectional study (n=288) examining associations, analysed using hierarchical multiple regression. Paper 3 was a pre–post feasibility study, using quantitative measures and qualitative interviews to evaluate a six-month health literacy intervention for registered nurses.
Findings show that higher health literacy sensitivity is associated with better teamwork, higher work engagement, and fewer mental health challenges, and the intervention resulted in statistically significant improvements in 7 of 9 domains of health literacy sensitivity and improved nurse–physician collaboration. Qualitative findings highlighted increased awareness, strengthened routines, and enhanced interdisciplinary collaboration, but also identified health literacy responsibilities as potentially demanding.
In conclusion, health literacy sensitivity is closely linked to both organizational conditions and healthcare personnel’s mental well-being. While it can be experienced as demanding, it can be strengthened through structured interventions, improved teamwork, and integration into everyday clinical practice.

This thesis investigates medical complications in children with moderate to severe acquired central nervous system (CNS) injury within pediatric rehabilitation medicine.
The thesis explores what is known from the literature about medical complications after acquired CNS injury in children, how frequently such complications occur during and after inpatient rehabilitation, and what consequences they have for the rehabilitation process and for patients and caregivers after discharge.
The work comprises one scoping review, one questionnaire-based survey, and one prospective cohort study. The survey included children who sustained CNS injury between 2 and 17 years of age and had received inpatient rehabilitation at Sunnaas Rehabilitation Hospital. The cohort study followed children aged 2-17 years during inpatient rehabilitation.
The findings show that the literature mainly describes single complications in isolation, while comprehensive rehabilitation-oriented studies are scarce. In the patient studies, medical complications were common both during rehabilitation and in the months and years after injury. Frequently reported complications included sleep disturbances, pain, spasticity, constipation, infections, anemia, malnutrition, and overweight. Their occurrence was associated with longer length of stay. Although most complications had limited impact on the rehabilitation process, pain, malnutrition, infections, and spasticity were more often associated with interrupted or prolonged stays, altered rehabilitation goals, and poorer functional prognosis. After discharge, patients and caregivers reported that complications affected daily functioning, participation in leisure activities, and psychological well-being, and were time-consuming to manage.
Overall, the thesis demonstrates that medical complications are common after acquired CNS injury in children and may have important consequences for rehabilitation, long-term functioning, and family life.
