Rehabilitering etter traumatisk hjerneskade (TBI) (avsluttet)

Mål

Hensikt med prosjektet er å fremskaffe kunnskap om kognitiv, emosjonell og funksjonell utkomme, samt å forstå hvilke faktorer kan forutsi god helse og yrkesutkomme.  I tillegg, å beskrive kost-effekt, evaluere forskjellige rehabiliteringsstrategier og kartlegge bruk av helsetjenester fra tidlig til sen fase.

Bakgrunn

En av de store utfordringene etter alvorlige skader inkludert TBI er å bidra til forskningsbasert kunnskap om hvordan man kan fremme helse, funksjon og deltakelse på kortere og lengre sikt. På grunn av sammensatte problemstillinger mangler det ofte validerte målemetoder, effekt- og kostnadsvurderinger er i liten grad gjennomført. Bruk av den internasjonale klassifikasjonen av funksjon, funksjonshemning og helse (ICF) som et rammeverk for forskning kan bidra til å belyse det biopsykososiale, funksjons- og kontekstperspektivet. Resultatene blir da sammenlignbare på tvers av studier og populasjoner og gjør det mulig for klinikere å målrette behandlingsstrategier for spesifikke funksjonsutfall. Prosjektet fokuserer også på rehabilitering etter TBI, organisering av oppfølgingen i forløpet etter skaden, bruk av helsetjenester og hvordan disse tilbudene dekker pasientens langsiktige behov.

Metode

Prosjektet har hatt en multimetode tilnærming og inkluderer metaanalyser, tverrsnitts analyser og longitudinelle analyser samt helseøkonomiske beregninger. ICF-modellen er benyttet for å få en bedre forståelse av utkomme etter TBI og kompleksiteten og interaksjoner av forskjellige forhold knyttet til yrkesutkomme etter TBI.

Resultat

Prosjektet viste at tidlig rehabilitering og en kontinuerlig behandlingskjede resulterte i bedre funksjon og helserelatert livskvalitet både 1 og 5 år etter alvorlig traumatisk hjerneskade. I samarbeid med Avdeling for helseledelse og helseøkonomi, UiO, og professor Lu ved Virginia Commonwealth University, USA, har vi bidratt til utvikling av metodologi og modeller for helseøkonomisk evaluering av TBI rehabilitering, og vist at tidlig rehabilitering og kontinuerlig behandlingskjede er kostnadseffektivt.  
I prosjektet fant vi også at personer som har gjennomgått hjerneskade sliter med sanseproblemer inkludert tap av luktesans og har redusertfunksjon, helserelatert livskvalitet og arbeidsdeltakelse over mange år. Mange rapporterer også store udekkede rehabiliteringsbehov.

Konklusjon

Traumatisk hjerneskade medfører funksjonsproblemer på mange områder over tid. Tidlig igangsatt rehabilitering og sammenhengende rehabiliteringskjeder er effektive. Deltakerne rapporterer imidlertid om mange ulike funksjonsproblemer og udekkede rehabiliteringsbehov lang tid etter gjennomgått hjerneskade. Det er derfor et stort behov for videreutvikling av effektive rehabiliteringstiltak i ulike faser til denne pasientgruppen.
